Disputes over coverage of emergency department services: a study of two health maintenance organizations.

STUDY OBJECTIVE: We describe the characteristics and outcomes of enrollee-health plan disputes over insurance coverage for emergency department (ED) services at 2 large health maintenance organizations (HMOs) that apply the prudent layperson standard. METHODS: We abstracted information from a stratified random sample of approximately 3,500 appeals of coverage denials lodged by privately insured enrollees between 1998 and 2000 at 2 of the nation's largest HMOs (hereafter referred to as Plan 1 and Plan 2). We describe appeals involving ED services in terms of the timing of visits, patient age,

Middle Ear and Temporal Bone Papilloma: A Clinicopathologic Study and Comprehensive Literature Review of 57 Cases.

Sinonasal papilloma (SP), formerly Schneiderian papilloma, represents a rare group of benign epithelial neoplasms, most commonly identified in the sinonasal tract, while less frequently identified in the pharynx, lacrimal sac, and middle ear. Within temporal bone sinonasal-type papilloma (TBSP), there seems to be a much higher recurrence and malignant transformation risk than those identified in the sinonasal tract. Based on this clinical report and a review of the cases reported in the English literature, 49% of the 57 cases developed in the setting of concurrent or antecedent sinonasal or na

Otogenic pneumocephalus: case report and review of the literature.

We report a case of a 70 year old lady who presented to the accident and emergency department with a three day history of left sided otalgia, drowsiness and confusion. On admission her temperature was 39 degrees C, glascow coma scale was 14/15 and otoscopy revealed a left sided otitis media. There was no sign of CSF otorrhoea or mastoiditis. Neuro-otological examination was normal. High resolution CT scan of temporal bones and brain showed evidence of pneumocephalus near the left cerebello-pontine angle and posterior to the left petrous bone. No focal parenchymal pathology, intra-cranial bleed

Carcinoid tumor of the middle ear: a case report.

Middle ear (ME) carcinoid tumor is exceedingly rare. We present a case of ME carcinoid tumor in a 29-year-old woman whose chief complaints were right otalgia, aural fullness, hearing loss, and facial palsy of 5 days duration. A reddish bulging mass behind the right eardrum and right facial palsy staged as grade IV on the House-Brackmann scale were noted. The high-resolution computed tomography of the temporal bone showed a soft-tissue density mass in the right middle ear and antrum without bony invasion. Although the initial diagnosis was ME adenoma made by the intraoperative frozen section re

Typical carcinoid tumor of the larynx occurring with otalgia: a case report.

Neuroendocrine neoplasms of the larynx comprise a rare group of tumors that have a controversial history with regard to their classification. The World Health Organization categorizes these tumors as typical carcinoids, atypical carcinoids, small-cell neuroendocrine carcinomas, and paragangliomas. Atypical carcinoids and small-cell neuroendocrine carcinomas are the more common. Only 13 cases of typical carcinoid have been reported. Local excision is sufficient for typical carcinoids and paragangliomas, but atypical carcinoids require neck dissection because of their tendency to metastasize. Sm

Diagnosis of acromegaly in orofacial pain: two case reports.

Acromegaly is an uncommon condition, with an annual incidence in the UK of three per million. The gradual onset of the clinical features mean that often friends and relatives are unaware of the underlying pathology. In view of the morbidity, and indeed mortality, arising from undiagnosed cases, general dental practitioners and other healthcare workers should routinely take note of systemic as well as intra-oral changes occurring in their patients when seen on review. The association of paraesthesia, anaesthesia and pain with acromegaly is well documented. However, there appear to be few report

Short-term tympanostomy in conjunction with hyperbaric oxygen therapy.

OBJECTIVE: To determine the most effective method of short-term middle ear aeration in patients unable to tolerate hyperbaric oxygen (HBO) therapy. STUDY DESIGN: Prospective study comparing two methods of short-term tympanostomy in each patient. METHODS: Seventeen adult patients were referred for management of barotitis and inability to tolerate hyperbaric oxygen therapy (HBO). Each patient underwent CO2 laser tympanostomy on the right ear and tympanostomy with T-tube placement in the left ear. The tube was removed on completion of HBO. Patients were asked to rate pain and their satisfaction

Sensation of aural fullness and its treatment with an autonomic nerve blocking agent.

One hundred and eighteen patients (143 ears) complaining of a full sensation in the ear without any evidence of other ear diseases were selected for treatment with an autonomic nerve blocking agent and were analysed clinically. Females were extremely predominant in number, 79% of the cases. Sixty-five of 143 ears (45.4%) showed normal hearing and 54 ears (37.8%) had sensorineural hearing loss in low tone. Four mg of butropium bromide, Coliopan, an anti-cholinergic routinely used as an antispastic agent, was injected intravenously. Subsequently, the aural fullness diminished or disappeared in 7

Pseudoephedrine prophylaxis does not prevent middle ear barotrauma in hyperbaric oxygen therapy.

A common complication of hyperbaric oxygen (HBO₂) treatment is middle ear barotrauma (MEB), which can lead to pain, treatment abandonment, or delay in treatment. Studies have shown that pseudoephedrine decreases MEB for pressure changes in SCUBA divers and airplane travelers. We conducted a randomized, double-blind, placebo-controlled trial to determine if pseudoephedrine effectively decreases MEB rates in patients receiving their first HBO₂ treatment. There was no statistically significant difference between the pseudoephedrine and placebo groups concerning ear pain ratings, tympanic membrane

A case of acute labyrinthitis due to intra-aural tick infestation: a case report.

BACKGROUND: Intra-aural tick infestations, though uncommon, pose a serious clinical challenge owing to the risk of acute labyrinthitis-an inflammatory condition of the inner ear. This inflammation can lead to severe complications such as sensorineural hearing loss, vertigo, and facial nerve palsy. Prompt recognition and management are crucial to prevent these adverse outcomes. CASE PRESENTATION: A 21-year-old female patient of Limbu ethnicity from Dharan presented with sudden-onset right-sided otalgia, accompanied by vertigo, emesis, and auditory impairment. Otoscopic examination revealed a t

Herpes zoster oticus associated with varicella zoster virus encephalitis.

Ramsay-Hunt syndrome, herpes zoster oticus (HZO), derived its name from James Ramsay Hunt, who first described it in 1907. It is classically characterized by acute peripheral facial paralysis, herpetic eruptions on the auricle, and vestibulocochlear dysfunction due to the reactivation of varicella zoster virus (VZV). In this Case Report, the authors describe an HZO patient with simultaneous VZV encephalitis. To date, only eight cases of HZO associated with VZV encephalitis have been reported in the English literature. Therefore, the authors discuss all the aspects of this rare entity, includin

Pneumocephalus as a delayed mastoidectomy complication.

We report the case of a 43-year-old woman who presented with a spontaneous pneumocephalus, 37 years after a mastoidectomy. Clinical examination showed a cerebrospinal fluid leak, meningeal herniation in the superior part of the middle ear, and an audible noise from her ear when she stood up due to the entrance of air into the cranium. A computed tomography scan and magnetic resonance imaging showed the complete destruction of the tegmen tympani and the pneumocephalus in the temporal lobe. The patient underwent an emergency operation via a double middle-ear and subtemporal approach. The meningo
