New technique for the treatment of fresh bony mallet finger: A retrospective case series study.

BACKGROUND: The bony mallet finger is a tear fracture of the extensor tendon, resulting in a flexion deformity of the finger, which affects both the function of the finger. The classical Ishiguro's method is associated with damage to the cartilage of the distal interphalangeal (DIP) joint and always lead to the joint stiffness. This paper explores a new technique to overcome the shortcomings of the classical Ishiguro's method and achieve better clinical efficacy. METHODS: We examined 15 patients with bony mallet fingers, 9 males and 6 females, from February 2020 to June 2022, ranged from 23 t

[The results of surgical and nonsurgical treatment of mallet finger].

INTRODUCTION: The injury of the hand tendon classified as mallet finger presents the loss of continuity of the united lateral band of the extensor apparatus above distal interphalangeal joint, which consequently leads to specific deformity of distal interphalangeal joint which is called mallet (hammer) finger. OBJECTIVE: Our paper had several research objectives: presentation of the existing results of surgical and nonsurgical treatment of mallet finger deformities and comparison of our findings and other authors' results. METHOD: The study was retro-prospective, and analyzed 62 patients tre

An alternative 6-inch octave keyboard reduces forearm muscle activation level and improves finger postures in expert pianists with smaller hand spans.

A small hand span is a risk factor of musculoskeletal injuries in pianists. Alternatively sized keyboards are recommended to help smaller hand pianists reduce risk of injury. However, in comparison to a 6.5″ octave conventional keyboard, it is unclear if a 6.0" octave keyboard (DS6.0) reduces forearm muscle activations and no research has investigated if keyboard size impacts finger joint kinematics. The objective was to determine the effect of keyboard size and hand size on pianists' right forearm muscle activation and finger joint posture when playing large handspan chords. Smaller hand pian

Mallet finger - management and patient compliance.

BACKGROUND: Mallet finger is a flexion deformity of the finger resulting from injury to the extensor mechanism at the base of the distal phalanx. OBJECTIVE: This article discusses the current clinical assessment and appropriate management of mallet finger injuries. DISCUSSION: Mallet finger usually results from forced flexion of an extended finger. Treatment can be difficult as patient compliance is essential, and if not treated appropriately the injury can lead to permanent deformity. Patients will present with a flexion deformity of, and inability to actively extend, the distal interphalan

[A man with a swollen ring finger].

A 30-year-old man visited the emergency department with a cut in his left forefinger. A coincidental finding during examination was a pre-existing swollen left ring finger. Radiography showed the formation of a new joint in the left ring finger after a volar dislocation of the proximal interphalangeal joint 2 years before.

Accidental needle inoculation of bacillus Calmette-Guerin vaccine in medical worker - TBC arthritis.

Despite the increase of mycobacterial infection, the incidence of tuberculous hand infection remains extremely low. We report a case of a 51-old pediatric nurse, who inadvertently inoculated her index finger via needlestick, while performing BCG immunization of the baby. Two days after the event, painful swelling and erythema of the distal interphalangeal joint region of the index finger appeared. Further diagnostic confirmed rare case of mycobacterial finger joint infection. It is important that hospital stuff should be informed about the possibility of BCG accident transmission to reduce the

Adipofascial, Transposition, and Rotation Flaps.

Soft tissue loss over the dorsum of the finger could potentially expose critical structures like extensor tendon, bone, and joint. These exposed structures often require flap coverage. Local flap is one of the available options to cover most small-sized defects on the dorsum of fingers. One of the primary requisites for any flap used on the dorsum of the finger, especially over a joint, is to enable the full range of motion of the finger. Even though skin over the dorsum of the fingers can be pinched easily in extension, with full flexion such suppleness is not demonstrated.

Post-traumatic reconstruction of digital joints by costal cartilage grafting: a preliminary prospective study.

In digital joint defects, reconstruction is meant to obtain a stable, mobile and pain-free finger. Six patients aged 29 years in average (15-46) and who were prospectively followed-up presented with digital joint defects that affected at least half of either the proximal interphalangeal (PIP) joint or the metacarpophalangeal (MCP) joint. These defects were treated in emergency (four cases) or scheduled for an autograft of costal cartilage harvested from the ninth rib. Four digits showed lesions of the extensor system which were repaired. One digit grafted after complete amputation was no more

The risk factors associated with subluxation of the distal interphalangeal joint in mallet fracture.

Surgical fixation is recommended when a mallet fracture involves more than one-third of the articular surface of the distal phalanx. This recommendation originates from the idea that involvement of more than one-third of the base of the distal phalanx causes subluxation of the distal interphalangeal (DIP) joint. Eighty-six fingers of 85 patients with a mallet fracture involving more than one-third of the articular surface of the distal phalanx were enrolled in this study. Patients were allocated on the basis of subluxation of the DIP joint into a group with no subluxation or a group with sublu

Flexohand: A Hybrid Exoskeleton-Based Novel Hand Rehabilitation Device.

Home-based hand rehabilitation has excellent potential as it may reduce patient dropouts due to travel, transportation, and insurance constraints. Being able to perform exercises precisely, accurately, and in a repetitive manner, robot-aided portable devices have gained much traction these days in hand rehabilitation. However, existing devices fall short in allowing some key natural movements, which are crucial to achieving full potential motion in performing activities of daily living. Firstly, existing exoskeleton type devices often restrict or suffer from uncontrolled wrist and forearm move

Passive Force-Feedback Gloves With Joint-Based Variable Impedance Using Layer Jamming.

Force feedback gloves have a great potential in enhancing the fidelity of virtual reality and teleoperation systems. It is a challenge to develop multifinger and lightweight force feedback gloves. In this paper, we propose a solution using layer jamming sheet (LJS) on each finger joint. In simulating free space, the LJS is soft and easy to deform, which allows the finger joints to move freely with a small resistance force. In simulating constrained space, the LJS becomes stiff, which provides resistance torques to prevent the rotation of finger joints. Possible solutions for mounting the LJS o

Multiple toe transplantations to reconstruct three amputated neighbouring distal fingers by heat press injury--a case report.

Heat press injury to the finger results in severe damage. When it is difficult to recover the function in the damaged finger, amputation may be unavoidable. We present a case of three heat press-injured neighbouring fingers reconstructed by multiple toe transplantations. All transplanted toes survived successfully. Two-point discrimination was 9, 10 and 7 mm in the index, middle and ring finger, respectively. The overall motion of the proximal interphalangeal (PIP) joint was as follows: 30/130 degrees, 30/75 degrees and 30/105 degrees at the index, middle and ring finger. The distal interphala

The fixed lupus hand deformity and its surgical correction.

In a prospective study of 125 patients with SLE, three females developed fixed hand deformities. All had arthritis for more than 10 years, and had been treated with a mean daily dose of 15 mg of prednisone. The hand deformities consisted of irreducible metacarpophalangeal (MCP) subluxation and/or dislocation without erosion or destruction of the MCP joints, resulting in hyperflexion of the fingers in the palm. Two patients had dislocation of the carpometacarpal joints in both hands. On patient developed reducible swan-neck deformities in the second to fifth fingers of both hands and a second p

Anatomic basis of dorsal finger skin cover.

This study describes the anatomy of the dorsal cutaneous vascular system of 180 digits (36 thumbs, index, middle, ring, and little fingers) from 18 pairs of fresh human cadaver hands. The aim of this paper is to incorporate the anatomic data into the current way of designing the homodigital adipofascial turnover flap for cutaneous coverage of the dorsum of the finger. We have carried out an anatomic study in preserved cadaver hands to define the distance between the joint and the origin of the dorsal cutaneous branches of the proper palmar digital artery in the proximal and middle phalanx of t

Automatic detection and quantification of hand movements toward development of an objective assessment of tremor and bradykinesia in Parkinson's disease.

BACKGROUND: Classification of parkinsonian symptoms, including tremor and bradykinesia, require the application of validated clinical rating scales which are inherently subjective. In this study, we assessed an objective measure of parkinsonian symptomology using automated analysis of hand gestures. NEW METHOD: We constructed and evaluated a hand and finger motion capture apparatus and analysis pipeline that recorded hand/finger motion of control subjects and patients with Parkinson's disease. The detailed three-dimensional (3D) motion features of each finger joint was extracted by using Disc
