BACKGROUND: Knee varus alignment may increase loading in the medial tibiofemoral compartment, which can increase strain on the articular cartilage. Knee valgus unloader braces seek to reduce loading through the medial femoral compartment, but their effects on cartilage characteristics during dynamic tasks have not been evaluated.
OBJECTIVE: To determine the effects of a knee valgus unloader brace on medial femoral articular cartilage deformation following a single 5000-step walking protocol in individuals with varus-knee alignment.
METHODS: Twenty-four healthy individuals (63% female, BMI =

OBJECTIVES: This hospital-based study evaluated the magnitude and characteristics of musculoskeletal anomalies in children with Down syndrome.
METHODS: Children aged 3 months to 14 years, diagnosed to have Down syndrome by karyotyping, were evaluated for musculoskeletal anomalies. We excluded children diagnosed with another chronic condition affecting musculoskeletal health (e.g., cerebral palsy, muscular diseases); and those with any acute illness, which is likely to affect evaluation for musculoskeletal anomalies. A detailed history was taken, and clinical examination was performed by a ped

BACKGROUND: Mucopolysaccharidosis (MPS) IVA is a lysosomal storage disorder caused by mutations in the gene encoding the galactosamine (N-acetyl)-6-sulfatase (GALNS) enzyme. Children with MPS IVA usually develop pectus carinatum, genu valgum and multiple skeletal abnormalities.
AIM: To establish a patient-derived induced pluripotent stem cell (iPSC) disease model to investigate the effects of two GALNS missense mutations.
METHODS: The medical history and clinical manifestations of a patient with MPS IVA were first inspected. The effects of the identified GALNS mutations were predicted throug

The objective of this study is to identify musculoskeletal manifestations associated with Stickler syndrome from an inclusive literature review and to perform the first population study to establish its incidence. We reviewed all for us retrievable publications linked to Stickler syndrome listed in PubMed and publications quoted within these publications going back to 1954. From our medical record data bases, we identified all patients with the diagnosis of Stickler syndrome out of a population of consecutive live births born in our maternity hospital between 2000 and 2016. We identified 504 p

PURPOSE: Congenital femoral deficiency is characterized by limb length discrepancy and genu valgum. Lengthening of the femur along its anatomic axis increases valgus alignment by medial knee translation. Pairing limb lengthening with simultaneous medial distal femoral hemiepiphysiodesis can simultaneously correct two limb deformities.
METHODS: All skeletally immature patients with congenital femoral deficiency who underwent antegrade femoral lengthening and concomitant guided growth over a 4-year period were reviewed. Length and alignment data were quantified during lengthening, consolidation

PURPOSE: To describe several cases of recurrent femorotibial subluxation episodes in pediatric patients, to review the existing literature about this rare entity, and to characterize its different clinical presentations.
MATERIALS AND METHODS: The study included a series of three cases seen at our center. All patients underwent a structured anamnesis and complete physical examination, as well as a basic radiological study. One underwent magnetic resonance imaging. A literature search was performed in the main databases under the terms "Snapping knee" and "Femorotibial subluxation in child" to

OBJECTIVE: Studies have detected differences in various measures of bone health between individuals with autism spectrum disorder (ASD) and their peers. However, these measures do not amount to direct clinical evidence of increased orthopedic pathology in this population. Some of the most compelling evidence to this effect comes from case reports of nutritional rickets in children with ASD. We report on 1 such case that, to our knowledge, is the first report of nutritional rickets in ASD necessitating corrective surgery.
METHODS: Case report, review of relevant literature, and implications fo

Twenty years ago, we described kindred of 105 individuals with isolated GH deficiency (IGHD) in Itabaianinha County, in northeast Brazil, carrying a homozygous mutation in the GH-releasing hormone receptor gene. These subjects exhibit markedly reduced GH responsiveness to stimulatory tests, and anterior pituitary hypoplasia. Serum concentrations of IGF-I, IGF binding protein type 3 and the acid-labile subunit are markedly reduced, with a lesser reduction of IGF-II. The most striking physical findings of these IGHD individuals are the proportionate short stature, doll facies, high-pitched voice

Type IV mucopolysaccharidosis (Morquio A syndrome; MPS IVA; OMIM 253000), is a multisystemic, severe and very disabling disease, also life-threatening; MPS IVA is due to a deficiency of the enzyme N-acetylgalactosamine-6-sulfate sulfatase (GALNS), a lysosomal enzyme responsible for the degradation of keratan sulfate (KS) and chondroitin-6-sulfate (C6S). The disease is characterized by respiratory, pulmonary manifestations and also causes bone involvement with progressive spondyloepimetaphyseal degradation and mild and late-onset ophthalmologic, hearing and cardiac complications. These manifest

OBJECTIVE: The introduction of hematopoietic stem cell transplantation (HSCT) has significantly improved the life-span of Hurler patients (mucopolysaccharidosis type I-H, MPS I-H). Yet, the musculoskeletal manifestations seem largely unresponsive to HSCT. In order to facilitate evidence based management, the aim of the current study was to give a systematic overview of the orthopaedic complications and motor functioning of Hurler's patients after HSCT.
METHODS: A systematic review was conducted of the medical literature published from January 1981 to June 2010. Two reviewers independently ass

Overuse injuries develop when repetitive stress to bone and musculotendinous structures damages tissue at a greater rate than that at which the body can repair itself. A combination of extrinsic factors, such as training errors and environmental factors, and intrinsic or anatomical factors, such as bony alignment of the extremities, flexibility deficits and ligamentous laxity, predispose athletes to develop overuse injuries. Malalignant of the lower extremity, including excess femoral anteversion, increased Q angle, lateral tibial torsion, tibia vara, genu varum or valgum, subtalar varus and e

We report a 20-year-old female with Hallervorden-Spatz syndrome (HSS). This patient is the product of consanguineous parents. She developed genu valgum, tendency to fall and mental deterioration at the age of 6, decrease of the number of spoken words at the age of 14, dysarthria, unsteady gait, postural tremor of the upper extremities, dystonic posture of hands and double incontinence at the age of 16. Her disease progressed slowly. Neurological examination on admission revealed severe mental retardation, optic atrophy, forced grasping, hyperactive tendon reflexes in the upper extremities and

BACKGROUND: Slipped capital femoral epiphysis (SCFEs) can be idiopathic or atypical (associated with renal failure, radiation therapy, and endocrine disorders). It is important to estimate the likelihood that atypical SCFE shows different clinical and radiographic characteristics and complications with typical SCFEs.
METHODS: We retrospectively reviewed 53 patients (66 hips) with SCFEs managed between 1995 and 2016. Sex, age, weight, height, the duration of symptoms the laterality of the slip, and complications were recorded. Bilateral hip and lower extremity long bone radiographs were utiliz

Multiple epiphyseal dysplasia (MED) is a genetically diverse skeletal disorder characterized by abnormal and delayed epiphyseal ossification, early-onset osteoarthritis, joint pain and mild short stature. MED is primarily caused by pathogenic variants in genes involved in cartilage and bone development, including COMP, MATN, COL9A1-3, CANT1 and SLC26A2. FGFR1 was suggested as a cause of MED in a study published in 2020, which reported a frameshift variant in the last exon of this gene. We assembled an international cohort of 13 individuals with MED carrying C-terminal frameshift variants in FG

BACKGROUND: Physical fitness predicts health outcomes during childhood and later life. While the influence of obesity on physical fitness is well established, the impact of lower limb alignment (such as knee alignment, foot posture, and plantar footprint) on physical fitness has been less explored.
OBJECTIVE: To evaluate the relationship between lower limb alignment, body mass index (BMI), and physical fitness in children aged 10-12 with a sex-specific analysis.
METHODS: Fifty-nine children (33 girls, 26 boys) participated in this cross-sectional study. Knee alignment (genu valgum, normal),

This systematic review examines the intersection of rickets and autism spectrum disorder (ASD), highlighting clinical insights from 10 studies involving 13 patients. Rickets, a pediatric bone disorder resulting from vitamin D, calcium, and/or phosphate deficiencies, often manifests through skeletal deformities, muscle weakness, and bone pain. ASD is a neurodevelopmental condition characterized by deficits in social communication and repetitive behaviors. The coexistence of these conditions, although uncommon, is notable due to overlapping factors such as selective eating habits and sensory sen

Elastosis perforans serpiginosa is a perforating dermatosis that morphologically presents as serpiginous, annular, or curved papules and plaques whose pathologic examination demonstrates the transepidermal elimination of elastic fibers. The perforating dermatosis can be idiopathic or induced by drug exposure: most commonly, D-penicillamine. Occasionally, elastosis perforans serpiginosa is associated with a connective tissue disease. Individuals with Marfan syndrome have a defect in the FBN1 gene, which produces fibrillin. The connective tissue disease can occur spontaneously or is usually inhe

BACKGROUND: The effect of total knee arthroplasty (TKA) on the ankle joint is not entirely clear. The purpose of this study is to assess postoperative changes in the coronal alignment of the ankle joint in patients undergoing TKA for various degrees of knee deformity.
METHODS: This retrospective study included 107 patients who had undergone TKA for primary osteoarthritis. In all cases, preoperative coronal alignment deformity of the knee was corrected in an attempt to restore the native mechanical axis of the knee. Patients were stratified into 3 groups according to the degree of knee coronal

Smith-McCort dysplasia 2 (SMC2) is a rare spondylo-epiphyseal-metaphyseal dysplasia caused by biallelic RAB33B variants. Short trunk dwarfism and radiological findings including the lacy ilia appearance and double bumps of the vertebral bodies are typical features. To date, only eight patients with SMC2 had been reported. The aim of this study is to evaluate the follow-up findings of seven patients from five families with SMC2 and to present four novel variants in RAB33B. The age of diagnosis of the patients was between 4 and 18 years. All patients had variable degrees of short trunk dwarfism

INTRODUCTION: Isolated congenital ACLD is a rare condition with limited literature on the optimal management approach. At present, patients with instability symptoms have been managed with ACL reconstruction in case reports. We present a case report of symptomatic isolated congenital anterior cruciate ligament deficiency (ACLD) managed effectively with gradual correction of biplanar proximal tibial deformity alone.
CASE PRESENTATION: This was a case of bilateral isolated congenital ACLD in a 15-year-old girl with chronic bilateral knee instability, bilateral mild genu valgum, and positive Lac
