Fistulae involving rectum and urethra: the place of Parks's operations.

Twenty-five rectovesical and rectourethral fistulae were treated over an 18-year period in which various methods for closing the rectal side of the fistula were developed, ultimately with two techniques using the full thickness of intestine. As a result of these operations the management of the urethral and vesical side of the fistulae became a relatively simple problem to which standard techniques could be applied.

Fistuloclysis can successfully replace parenteral feeding in the nutritional support of patients with enterocutaneous fistula.

BACKGROUND: Use of total parenteral nutrition (TPN) in patients with acute intestinal failure due to enteric fistulation might be avoided if a simpler means of nutritional support was available. The aim of this study was to determine whether feeding via an intestinal fistula (fistuloclysis) would obviate the need for TPN. METHODS: Fistuloclysis was attempted in 12 patients with jejunocutaneous or ileocutaneous fistulas with mucocutaneous continuity. Feeding was achieved by inserting a gastrostomy feeding tube into the intestine distal to the fistula. Infusion of enteral feed was increased in …

[Fistulas between bowel and urinary tract (author's transl)].

From 1968 to 1977, 21 patients were observed who had fistulas between the bowel and the urinary tract. Except in five patients, fistulas were located vesicointestinally. The main etiologic factors were inflammatory lesions of the bowel, especially diverticulitis and appendicitis. All patients had symptoms relating to the urinary tract, mainly recurrent urinary infection with dysuria. On the average, nine months passed until the fistula was diagnosed. Urethrocystoscopy proved to be the most valuable aid in diagnosing intestinovesical fistulas, the fistulous opening was thus seen in 11 patients.

[Renal stone ileus in xanthogranulomatous pyelonephritis. A case report].

A peculiar case of intestinal occlusion caused by a renal stone in a patient with nephroduodenal fistula due to previous xanthogranulomatous pyelonephritis is reported. Only few cases of nephroduodenal fistula are described in the literature, generally as a single case report or in small series. A nephroduodenal fistula as a result of chronic renal inflammatory disease such as xanthogranulomatous pyelonephritis, is usually associated with renal stones, recurrent urinary tract infections or endocrine disorders. Finally, renal stone as a cause of ileus is an event rarely described in the literat

Beyond Rigler's Triad: Gallstone Ileus in the Absence of a Bilioenteric Fistula.

Gallstone ileus is an uncommon cause of mechanical intestinal obstruction, classically associated with bilioenteric fistula formation that enables gallstone migration into the gastrointestinal tract. Its incidence increases among elderly patients and is associated with significant morbidity due to delayed diagnosis. Obstruction typically occurs at the terminal ileum, and diagnosis traditionally relies on Rigler's triad: intestinal obstruction, pneumobilia, and an ectopic gallstone. Gallstone ileus without a bilioenteric fistula is an exceptionally rare presentation that challenges conventional

Delayed diagnosis of anorectal malformations in neonates.

BACKGROUND: Anorectal malformations (ARM) are common congenital abnormalities of the terminal hindgut. Ideally, ARM should be diagnosed at, or shortly following, birth by careful physical examination of the perineum. Delayed diagnosis has been implicated as a risk factor for complications, including intestinal perforation. This study aimed to determine the rate of delayed diagnosis and associated intestinal perforation in ARM. METHODS: A retrospective review was performed for all ARM patients managed at The Royal Children's Hospital over a 16-year period (2000-2015). Data collected included A

Gracilis muscle repair of rectovaginal fistula after restorative proctocolectomy. Report of two cases.

Gracilis muscle interposition flaps have been used to treat two patients with rectovaginal fistulas. The fistulas occurred following restorative proctocolectomy with a J-shaped ileal reservoir and ileoanal anastomosis. Attempts at local repair of the fistulas had failed. A diverting loop ileostomy was constructed simultaneously. Anterior sphincteroplasty was performed in one patient for associated incontinence. Excellent results were achieved in both patients. The fistulas have healed, and intestinal continuity has been re-established. This procedure can be useful to salvage a pelvic pouch com

[Parenteral nutrition using an arteriovenous fistula].

Seventeen children with extended resection of the small intestine, chronic intestinal adynamia or Crohn's disease were given prolonged parenteral nutrition through an arteriovenous fistula, exclusively of associated with enteral nutrition. This new technique allowed the patients to lead a normal life, with a normal growth. In more than 22 patient-years no infectious complication occurred. Thrombosis of the fistula remains the most important complication; il can be prevented thanks to a controlled infusion technique, including adjunction of platelet aggregation inhibiting drugs and the maintena

Spontaneous enterocutaneous fistula due to femoral hernia.

Spontaneous enterocutaneous fistula is a rare entity. We encountered a case of spontaneous enterocutaneous fistula in the groin region due to femoral hernia. A 60-year-old man presented with spontaneous enterocutaneous fistula in the left groin region without signs of peritonitis. He was kept on conservative treatment, but on third postadmission day, he developed a swelling in his right groin, which became firm and irreducible with signs of intestinal obstruction. On exploratory laparotomy, bilateral femoral hernias were noted with formation of enterocutaneous fistula on the left side. Reducti

Gastric ischemia secondary to intestinal occlusion due to gallstone ileus.

Gallstone ileus is a rare complication of cholecystoduodenal fistula. Gastric ischemia due to intestinal obstruction is an extremely rare event that, if not diagnosed in time, can have a fatal outcome. We present the case of a patient with intestinal occlusion due to a gallstone obstructing the middle jejunum in which gastric ischemia was diagnosed intraoperatively due to the distension caused by the intestinal obstruction.
