Sleep-related violence in parasomnias: Forensic and clinical insights for criminal responsibility assessments.

BACKGROUND: Parasomnias are undesirable physical events during sleep that can manifest as complex, violent behaviors, posing significant challenges for forensic psychiatry. The central legal dilemma involves "automatism," where actions lack the necessary mens rea for criminal liability. This study analyzes the prevalence, clinical characteristics, and adjudication of parasomnia defences within the Turkish legal system. MATERIAL AND METHODS: A retrospective analysis was conducted using the nationwide archives of the Council of Forensic Medicine through the National Judicial Network Project dat

Antidepressant treatment outcomes of psychogenic movement disorder.

BACKGROUND: Psychogenic movement disorder (PMD) is a subtype of conversion disorder. We describe the outcomes of a series of PMD patients following antidepressant treatment. METHOD: Twenty-three outpatients with chronic PMD, diagnosed using Fahn and Williams' criteria, underwent psychiatric assessment. The patients were referred for assessment and management from January 2003 to July 2004. Fifteen agreed to be treated with antidepressants. Patients received citalopram or paroxetine; those who did not respond after 4 weeks of taking an optimal dose were switched to venlafaxine. Concurrently, 3

The reliability of frequency-doubling technology (FDT) perimetry in a pediatric population.

PURPOSE: Frequency-doubling Technology IFDT) Perimetry was introduced as a rapid, easy method for detection of visual-field defects in adults. Its reliability, howev, has never been tested among pediatric patients. The purpose of this study was to determine if children could complete a screening program of FDT Perimetry reliably and to define the age range over which it might be most applicable. METHODS: Two hundred fifty-nine normal children (mean age, 10.7 years; range, 4 to 17 years) were asked to complete the FDT C-20-1 screening test as part of their general eye examination. The visual-f

An examination of the relation between early childhood lead (Pb) exposure and cognitive intraindividual variability in a medicolegal sample.

OBJECTIVE: Lead (Pb) exposure, particularly during early childhood, is a major health concern associated with diminished cognitive performance. Most prior research has focused on Full-Scale IQ (FSIQ). This study examined relations between childhood blood Pb exposure and cognitive intraindividual variability (IIV) in a medicolegal sample. METHOD: Two-hundred sixteen adolescent participants (50% female; 82.4% Black; age M = 20.47, SD = 2.05 years; education M = 10.94, SD = 1.56 years) completed a neuropsychological battery more than a decade after documented Pb exposure. All participants passed

Medical Disqualification Following Concussion in Collegiate Student-Athletes: Findings from the CARE Consortium.

BACKGROUND: The absence of evidence-based guidelines make medical disqualification (MDQ) following concussion one of the most challenging decision-making processes faced by sports medicine professionals. OBJECTIVE: We aimed to compare premorbid and postmorbid factors between student-athletes that were and were not medically disqualified from sport following a concussion. METHODS: Among 1832 student-athletes diagnosed with concussion within the CARE Consortium, 53 (2.9%) were medically disqualified (MDQ +) and 1779 (97.1%) were not medically disqualified (MDQ-). We used contingency tables and

Performance validity test failure predicts suppression of neuropsychological test results in developmentally disabled children.

There is increasing awareness of the need to use Performance Validity Tests (PVTs) when assessing cognitive abilities in children. Since 1996, there has been an ongoing clinical study of the use of three PVTs with a consecutive series of 1,285 developmentally disabled children. In this study, we report on the results of these PVTs in children of many diagnostic categories. Failure rates on all three tests were very low. The mean scores on the effort measures in those passing the PVTs were extremely high. Failure on each PVT was found to be associated with a significant and widespread suppressi

Motor dysfunction profiles in traumatic brain injury and postconcussion syndrome.

Motor measures are sensitive to central lesions, but they are also affected by peripheral injury and motivation. The motor skills profiles of proven brain injury clients were compared with the profiles of healthy postconcussion patients. The chief result was a double dissociation: The traumatic brain injury (TBI) group produced a motor dysfunction gradient consistent with upper motor neuron disease, while the compensation-seeking postconcussion group produced a nonphysiologic pattern. Objective measures of behavioral pain and emotional distress did not correlate with the findings. Motor skill

Psychological correlates of psychogenic seizures.

Psychological correlates of psychogenic seizures were studied. The MMPI, Portland Digit Recognition Test (PDRT; a forced choice measure of motivation), disability status, Face-Hand Test, and Finger Agnosia were compared in 53 patients with medically intractable seizure disorders who underwent intensive EEG monitoring. Using conventional neurologic criteria, 64% had diagnoses of epileptic seizures (ES) and 36% had psychogenic seizures (PS). PS patients were significantly higher in number of somatoform MMPI profiles and likelihood of applying for financial benefits, and significantly lower on th

Differential diagnosis of orofacial pain.

No attempt has been made to deal with all of the various diseases that the dentist has to consider when diagnosing orofacial pain. One of the most important causes of facial pain--so-called temporomandibular joint or myofascial pain dysfunction (MPD) syndrome--has not been mentioned, yet it accounts for many of the patients attending facial pain clinics. There are several textbooks devoted to the MPD and the reader is referred to these for a full description of the condition. Whenever a patient presents for evaluation of orofacial pain the dentist must remember: (1) to take a detailed history

Use of the Multiple Errands Test-Simplified Version in the assessment of suboptimal effort.

Most measures of suboptimal effort focus on short-term learning; fewer studies have considered non-memory feigned cognitive impairment. This study investigated the utility of the Multiple Errands Test - Simplified Version (MET-SV) in the detection of feigned executive functioning impairment. Performance of simulating malingerers (N = 47) was compared to acquired brain injury (N = 46) and neurologically healthy control groups (N = 50). Although simulating malingerers were successful at feigning a realistic level of impairment compared to the brain injury group, there were significant difference

Trauma-induced dissociative amnesia in World War I combat soldiers.

OBJECTIVE: This study relates trauma-induced dissociative amnesia reported in World War I (WW I) studies of war trauma to contemporary findings of dissociative amnesia in victims of childhood sexual abuse. METHOD: Key diagnostic studies of post-traumatic amnesia in WW I combatants are surveyed. These cover phenomenology and the psychological dynamics of dissociation vis-à-vis repression. RESULTS: Descriptive evidence is cited for war trauma-induced dissociative amnesia. CONCLUSION: Posttraumatic amnesia extends beyond the experience of sexual and combat trauma and is a protean symptom, whic

The meaning of modern audiological tests in relation to noise-induced deafness: a review.

Wilmot, T. J. (1972).Brit. J. industr. Med.,29, 125-133. The meaning of modern audiological tests in relation to noise-induced deafness: a review. If noise-induced deafness becomes a prescribed disease, it is inevitable that a very large number of workers with hearing difficulties will be discovered and will pose serious problems for both industrial medical officers and otologists working in the National Health Service. The two main problems are detection of deafness and a decision whether the hearing loss is attributable to noise damage. This paper is concerned largely with the second problem

The Symptom Checklist-Revised (SCL-90-R) is able to discriminate between simulation and fibromyalgia.

This study assessed the ability of the Symptom Checklist-Revised (SCL-90-R) to discriminate between two groups of fibromyalgia patients (those who were about to begin a treatment including the explicit aim of returning to work and those who were initiating a legal procedure to obtain permanent disability compensation) and two groups of healthy volunteers (medical students and psychology graduates), who were asked to produce a symptomatic resemblance to a chronic pain disorder. Logistic regression analyses were applied to the SCL-90-R subscales and individual probabilistic indices of simulation

Detecting dissimulation: profiles of simulated malingerers, traumatic brain-injury patients, and normal controls on a revised version of Hiscock and Hiscock's Forced-Choice Memory Test.

A computer-administered memory test was given to normal subjects instructed to feign brain damage, normal controls, and traumatic brain-injured (TBI) patients with complaints of memory dysfunction. The test, a revised version of an instrument developed by Hiscock and Hiscock (1989) employed forced, two-choice recognition of previously presented five-digit numbers. Two levels of item difficulty and three retention intervals were used. Control and TBI subjects performed at near ceiling level on easy items and items with short retention intervals. Feigning subjects performed worse than TBI and co

Postural orthostatic tachycardia syndrome (POTS): association with Ehlers-Danlos syndrome and orthopaedic considerations.

BACKGROUND: Postural orthostatic tachycardia syndrome (POTS) is the most common of several types of dysautonomia, characterized by dysfunction of the autonomic nervous system manifesting with symptoms of orthostatic intolerance with or without associated orthostatic hypotension and excessive autonomic excitation. Given the numerous presenting musculoskeletal symptoms of POTS and its known associations with other clinical entities like Ehlers-Danlos syndrome, POTS constitutes an unusual treatment challenge of which the orthopaedic surgeon and other related healthcare providers should be aware.

Compensation and Pension Exams for Military Sexual Trauma-Related Posttraumatic Stress Disorder: Examiner Perspectives, Clinical Impacts on Veterans, and Strategies.

BACKGROUND: It is estimated that in one in three women veterans experience military sexual trauma (MST), which is strongly associated with posttraumatic stress disorder (PTSD). A 2018 report indicated the Veterans Benefits Administration (VBA) processed approximately 12,000 disability claims annually for PTSD related to MST, most of which are filed by women. Part of the VBA adjudication process involves reviewing information from a Compensation and Pension (C&P) exam, a forensic diagnostic evaluation that helps determine the relationship among military service, diagnoses, and current psychosoc

The "sunglasses sign" predicts nonorganic visual loss in neuro-ophthalmologic practice.

OBJECTIVE: The aim of our study was to evaluate whether wearing sunglasses (the "sunglasses sign") can be used by neuro-ophthalmologists to predict nonorganic visual loss (NOVL) in their patients. METHODS: We prospectively collected information on all new patients seen by us over 13 months. We included all patients who ultimately received a diagnosis of NOVL, and all patients wearing sunglasses in our clinic. We recorded demographics, iris color, number of positive review of systems, ocular examination, precipitating event or trauma, workers' compensation claims, disability and lawsuit relate
