Restricted pulmonary diffusion capacity after exercise is not an ARDS-like injury.

Pulmonary diffusion capacity (DLCO) is reduced 2 h after various types of exercise, such as rowing, treadmill running, arm cranking and marathon running. The decrease in DLCO may involve alterations in the alveolar-capillary membrane as well as depletion of the central blood volume. We hypothesized that the reduction in DLCO might also be influenced by oxygen free radicals, acute phase proteins and endotoxin, which are also involved in the adult respiratory distress syndrome (ARDS). Ten competitive male oarsmen performed a 6 min 'all-out' ergometer row. Single breath DLCO was determined before

Unusual consecutive sacral stress fractures in a female distant runner: a case report.

Sacral stress fractures are rare overuse injuries predominantly perceived in female long-distance runners. Our case report describes left-sided sacral wing stress fracture followed by contralateral sacral wing fracture after return to running. A 21-year-old female amateur long-distance runner presented with nonspecific low back and left buttock pain. Plain radiograph at presentation was unremarkable. Magnetic resonance imaging (MRI) revealed extensive bone marrow oedema in the cranial part of the left sacral wing and oblique fracture line. After six weeks of moderate training and two months of

Why do they do it? Long-term follow-up on the motivation to run in ultrarunners: findings of the ultrarunners longitudinal TRAcking (ULTRA) study.

Participants in the ULTRA Study completed surveys in 2014 and again in 2024 to assess long-term changes in motivation to run (Motivations of Marathoners Scale) and perception of success (Perception of Success Questionnaire). Of the 534 ultramarathon runners, 270 were still participating in ultramarathons at 10-year followup. Motivation declined across nearly all dimensions over 10 years, including personal goal achievement, competition, recognition, affiliation, psychological coping, life meaning, and self-esteem (all p<0.001). Health orientation showed no significant change (p = 0.084). Parti

Physical activity enhances metabolic fitness independently of cardiorespiratory fitness in marathon runners.

High levels of cardiovascular fitness (CRF) and physical activity (PA) are associated with decreased mortality and risk to develop metabolic diseases. The independent contributions of CRF and PA to metabolic disease risk factors are unknown. We tested the hypothesis that runners who run consistently >50 km/wk and/or >2 marathons/yr for the last 5 years have superior metabolic fitness compared to matched sedentary subjects (CRF, age, gender, and BMI). Case-control recruitment of 31 pairs of runner-sedentary subjects identified 10 matched pairs with similar VO2max (mL/min/kg) (similar-VO2max). T

Fluid intake and changes in limb volumes in male ultra-marathoners: does fluid overload lead to peripheral oedema?

An increase in body mass due to oedema has been previously described. The aim of this study was to investigate a potential association between both fluid and electrolyte intake and the formation of peripheral oedemas. Fluid and electrolyte intakes and the changes in limb volumes in 50 male 100-km ultra-marathoners were measured. Pre- and post-race serum sodium concentration ([Na(+)]), serum aldosterone concentration, serum copeptin concentration, serum and urine osmolality and body mass were determined. Fluid intake, renal function parameters and urinary output, as well as the changes of volum

The fastest nonprofessional age group IRONMAN triathletes in the world originate from Europe.

It is well known that elite athletes of specific ethnicities and/or nationalities dominate certain sports disciplines (e.g., East Africans in marathon running). However, we do not know the nationalities of the fastest non-professional IRONMAN triathletes. Therefore, this study intended to identify the fastest athletes by country of origin competing in IRONMAN triathlon events, focusing on non-professional age group triathletes. Data from all IRONMAN age group athletes competing worldwide between 2002 and 2022 in all official IRONMAN races were collected. Sex, age group, country of origin of th

Running with Femoral Acetabular Impingement: Operative Vs Nonoperative Treatment.

A 31-year-old female lawyer presents with right hip pain starting approximately 3 years ago and worsening over the past 2 to 3 months. She is an avid runner, running 20 to 30 miles weekly, and noticed that her pain started after a running program training for the marathon. Her pain is primarily over the lateral aspect of her hip with some radiation into the groin. The pain is made worse with impact exercises such as running and made better with walking and cycling. She has done physical therapy for the past 3 months without any significant improvement. She has not had any injections or taken a

The Neglected Leg Lymphatic Vascular Changes in the Pathomechanism of Delayed Onset Muscle Soreness in Runners.

Background: Delayed onset muscle soreness (DOMS) in runners is classified as a leg muscle strain injury and presents with tenderness or stiffness to palpation and movement limitation. Most attention is directed at muscles but not at the mass of other limb soft tissues, including their lymphatic vasculature, although they undergo mechanical stress and bruises, edema, nail destruction, and pains contributing to symptoms. Methods and Results: The study was done on lower limbs of long-distance runners suffering from DOMS complaints. There were 16 runners, 11 males and 5 females, age 22-28, practic

Post ambulatory swollen hands (POTASH): a case report.

BACKGROUND: Post ambulatory swollen hands (POTASH) is an acquired condition that presents as asymptomatic swelling of the hands and their digits during ambulatory activities such as walking, hiking, and running. Affected individuals cannot clench their fingers into their palm to make a fist and thereby have a positive fist sign. The benign condition typically resolves spontaneously within 2 hours with complete resolution of the swelling; the individual can make a tight fist and has a negative fist sign. CASE DESCRIPTION: A 65-year-old man developed POTASH, when he was rapidly walking during a

End-tidal CO2 and ventilation: Novel markers for assessing performance levels in elite long-distance runners.

Well-trained individuals, compared to less well-trained individuals, exhibit a lower minute ventilation (V̇E) and higher end-tidal partial pressure of CO2 (PETCO2) at a given work rate. This study investigated whether such breathing adaptations seen in well-trained individuals also applied to elite long-distance runners. Forty-one long-distance runners were categorized into high (Long-High, consisting of Tokyo-Hakone College Ekiden [relay marathon] runners and Olympic athletes, n = 23), or low performance-level group (Long-Low, n = 18) according to their race times. Exercise, immunology and upper respiratory tract infections.

The literature reveals a paradoxical response of the immune and host defense systems to endurance exercise apparent stimulation following long-term regular training and suppression in response to acute exposure to exhaustive endurance exercise. Several epidemiological surveys have confirmed a clinical manifestation of immunosuppression in the form of increased incidence of upper respiratory tract infection symptoms following participation in competitive marathon and ultramarathon running events. Prerace training status and racing intensity have been related to the incidence of this symptomatol

Influence of circadian preference, sleep inertia and their interaction on marathon completion time: A retrospective, cross-sectional investigation of a large mass-participation city marathon.

Burgeoning interest in marathons necessitates an understanding of performance determinants. Research has highlighted the importance of diet, training and sleep, yet relations of circadian preference and sleep inertia with marathon performance remain largely unexplored. Because marathons generally start early-to-mid morning, these characteristics may have relevant impact. This study investigates relationships of circadian preference, sleep inertia and their interaction with marathon completion time. Consenting participants in a 2016 large mass-participation city marathon completed self-report q

Peripheral blood mononuclear cell number and paracrine function in responses to a 50-km trail race: An exploratory study.

Peripheral blood mononuclear cells (PBMCs) represent a heterogeneous mix of cells with paracrine functions that may be altered following prolonged exercise. We determined the effect of ultramarathon running on PBMC paracrine function and PBMC subtype number. Recreational athletes participated in a 50 km ultramarathon. Blood was sampled from N = 7 at baseline, 10 km, 50 km, and 24 h post-race. PBMCs were isolated and cultured, and conditioned media was used for a HUVEC-based proliferation assay. CD31+, CD3+, and CD31+/CD3+ PBMCs were quantified at each time point. Proliferation increased from b
