Facial nerve monitoring during parotidectomy: a systematic review and meta-analysis.

OBJECTIVES: To determine the effectiveness of intraoperative facial nerve monitoring (FNM) in preventing immediate and permanent postoperative facial nerve weakness in patients undergoing primary parotidectomy. DATA SOURCES: PubMed-NCBI database from 1970 to 2014. REVIEW METHODS: A systematic review and meta-analysis of the literature was conducted. Acceptable studies included controlled series that evaluated facial nerve function following primary parotidectomy with or without FNM (intraoperative nerve monitor vs control). Primary and secondary end points were defined as immediate postopera

[Ultrasonographic evaluation of the diameter of the optic nerve sheath for medication of intracranial pressure: a case report].

The main complication associated with acute brain injury is the elevation of intracranial pressure (ICP) and it is associated with high morbidity and mortality. In these patients, multimodal neurological monitoring has emerged as a fundamental tool in the intensive care unit, with the minimally invasive trend seen in recent years. We report the case of a patient in which ICP monitoring was based on the measurement of the diameter of the optic nerve sheath (DONS), a procedure that has shown a good correlation with the ICP, as well as a high specificity, sensitivity, and low cost.

Wake-up test in total hip arthroplasty with high-riding developmental dysplasia.

Total hip arthroplasty (THA) for patients with Crowe type IV developmental dysplasia of the hip is technically challenging. This group of patients has a higher incidence of nerve injury during THA. Although neurophysiologic intraoperative monitoring has been developed to provide nerve monitoring, it is not always available. The wake-up test has been used for intraoperative spinal cord monitoring during major spinal surgery, but no study has reported the use of the wake-up test for neurologic monitoring during THA in patients with severe developmental dysplasia of the hip. The authors retrospec

Does D-wave predict laterality? A case report and video demonstrating use of D-wave intraoperative neuromonitoring in surgery of intramedullary spinal cord tumor to predict laterality of post-operative neurological deficit.

PURPOSE: D-wave monitoring in for intramedullary spinal cord tumors is a necessary surgical adjunct in the current literature. The epidural placement of the D-wave electrode for monitoring does not help in predicting the laterality of the post-operative deficit. We present an illustrative case of how subdural placement of D-wave electrode can help us predicting the laterality of post-operative neurological deficit. CASE REPORT: This is a case of D1-D2 pilocytic astrocytoma. The patient presented with history of dysesthetic pain with paraesthesias in all four limbs. She underwent D1-D2 laminec

Changes in motor evoked potentials after erector spinae block in scoliosis surgery-when to take pre-incision baseline recordings?

Multimodal intraoperative neurophysiological monitoring (IONM) is highly valuable in scoliosis surgeries for monitoring spinal cord function, particularly during instrumentation. Accurate timing of baseline recordings of TcMEP and SSEP is crucial, as any changes observed during surgery and instrumentation are compared to these baseline recordings. However, the impact of ultrasound-guided erector spinae block (USG-ESPB) on SSEP and TcMEP is not well-studied in scoliosis surgery. In this report, we present two cases of scoliosis surgery where bilateral two-level USG-ESPB using different concentr

Intraoperative Spinal Cord Monitoring Does Not Decrease New Postoperative Neurological Deficits in Patients With Cervical Radiculopathy or Spondylotic Myelopathy Undergoing One or Two Level Anterior Cervical Discectomy And Fusion.

BACKGROUND: Intraoperative neurological monitoring (IONM) is commonly used in spine surgery. However, the utility of IONM in anterior cervical decompression and fusion (ACDF) remains a topic of debate. The purpose of the study was to investigate the utility and cost of IONM (both Somatosensory evoked potentials (SSEPs) and Motor Evoked Potentials (Tc-MEPs)) in reducing postoperative neurological deficits in myelopathic and non-myelopathic patients undergoing ACDF. METHODS: Retrospective chart review was performed to include only patients with cervical radiculopathy or myelopathy undergoing on

Motor evoked potential recovery with surgeon interventions and neurologic outcomes: A meta-analysis and structural causal model for spine deformity surgeries.

OBJECTIVE: To improve estimates of motor evoked potential (MEP) performance during spine deformity surgeries by accounting for potential confounders. METHODS: A meta-analysis of MEPs for spine deformity surgeries determined the probability of a MEP deterioration which recovered by the end of surgery, P(RSC), and the conditional probability of no new post-operative deficit given an RSC, P(NND|RSC), stratified by category of intraoperative adverse event associated with the MEP deterioration. A structural causal model (SCM) and propensity score matching accounted for intraoperative adverse event

A low-cost solution for quantification of movement during DBS surgery.

BACKGROUND: During the deep brain stimulation (DBS) electrode implantation operation with microelectrode recordings (MER) in awake patients, somatotopic testing and test stimulation are performed to improve electrode placement and provide the most beneficial symptom reduction possible, while minimizing side effects. As this procedure is commonly used to alleviate abnormal movements associated with Parkinson's disease (PD) and Essential Tremor (ET), intraoperative assessment of a patient's movements is critical to optimizing surgical benefit. However, despite its importance, movement assessment

Raising the pressure: Hemodynamic effects of splanchnic nerve stimulation.

A number of cardiovascular and neurological diseases are characterized by a dysregulation of intravascular volume distribution. The veins and arteries of the visceral organs form the so-called splanchnic vascular compartment and are the largest reservoir for intravascular blood. The blood localized in the splanchnic compartment can be mobilized in and out of the compartment via passive compression or active neurohormonal recruitment. We studied the hemodynamic effects of splanchnic nerve stimulation during five cases of irreversible electroporation (IRE) in patients with pancreatic cancer. In

The Interpretation of Muscle Motor Evoked Potentials for Spinal Cord Monitoring.

OBJECTIVE: To provide a summary of the intraoperative monitoring of muscle motor evoked potentials (MEPs) based on the presence-absence concept during neurosurgical operations along the spinal cord. METHOD: Expert review. DISCUSSION: The measurable parameters of MEPs, such as signal amplitudes and thresholds vary considerably both during a single surgery in a single individual patient as well as between individuals and operations. The presence or absence of responses irrespective of stimulus intensity and response amplitude is much more clearly defined. The correlation of intraoperative MEP

Peripheral nerve function estimation by linear model of multi-CMAP responses for surgical intervention in acoustic neuroma surgery.

Nerve function assessments are crucial for surgical intervention during acoustic neuroma surgery. Cranial nerves such as acoustic and facial nerves, can be possibly damaged during tumor dissection. Proper surgical intervention should prevent neurological deficit and achieve total tumor removal. Conventionally, nerve function is qualitatively evaluated by surgeon and neurologist. Facial nerves can be preserved by monitoring the compound muscle action potential (CMAP) response. The differences in the amplitude and latency of CMAP are used as indicators during surgical interventions. However, bas

Conflict processing in juvenile patients with neurofibromatosis type 1 (NF1) and healthy controls - Two pathways to success.

Neurofibromatosis Type 1 (NF1) is a monogenetic autosomal-dominant disorder with a broad spectrum of clinical symptoms and is commonly associated with cognitive deficits. Patients with NF1 frequently exhibit cognitive impairments like attention problems, working memory deficits and dysfunctional inhibitory control. The latter is also relevant for the resolution of cognitive conflicts. However, it is unclear how conflict monitoring processes are modulated in NF1. To examine this question in more detail, we used a system neurophysiological approach combining high-density ERP recordings with sour

[Neurophysiological studies of the neuromuscular junction].

INTRODUCTION: The role played by neurophysiological studies (NPS) in the diagnosis of diseases affecting neuromuscular transmission (NMT) is based on the study of the failure of muscle fibres to achieve a sufficient degree of depolarisation for the junction potential to reach the appropriate threshold and attain a muscular action potential. This totally or partially blocked impulse will give rise to different types of responses in neurophysiological tests. AIMS: To analyse the different NPS as diagnostic methods in diseases that affect NMT. DEVELOPMENT: The article offers a review of the con

[D-wave-guided navigation in spinal cord surgery: From monitoring to decision-making].

The primary goal of spinal cord surgery is to achieve maximal safe resection of lesions while preserving neurological function. Direct wave (D-wave) monitoring, regarded as the gold standard for assessing the functional integrity of the corticospinal tract (CST), has evolved from a passive monitoring tool into an active intraoperative functional navigation system. To systematically review the evolution and clinical application of D-wave-guided navigation, this article discusses 3 key aspects. First, technological advancements, such as subdural recording techniques, have improved signal stabili
