Association between ambient temperature and emergency psychiatric consultations: A case-crossover study in a South American emergency setting (2021-2023).

BACKGROUND: Although environmental factors have been studied for their impact on mental health, the link between ambient temperature and mental disorders remains insufficiently explored. AIM: This study aimed to explore the relationship between ambient temperature and the frequency of emergency psychiatric consultations at the sole university hospital in Paraguay from 2021 to 2023. METHODS: This investigation adopted a quantitative, observational, cross-sectional design, using a case-crossover approach. Data collection entailed reviewing patient records and meteorological data, focusing on t

Gestational weight gain charts for Latin American adolescents.

Monitoring gestational weight gain (GWG) throughout pregnancy among adolescents is important for detecting individuals at risk and timely intervention. However, there are no specific tools or guidelines for GWG monitoring of this group. We aimed to construct GWG charts for pregnant adolescents (10-19 years old) according to pre-pregnancy body mass index (BMI) using a pooled dataset from nine Latin American countries. Datasets from Argentina, Brazil, Chile, Colombia, Mexico, Panama, Paraguay, Peru, and Uruguay collected between 2003 and 2021 were combined after data cleaning and harmonization.

Serum biomarkers and anti-flavivirus antibodies at presentation as indicators of severe dengue.

BACKGROUND: Dengue is the most common vector-borne viral disease worldwide. Most cases are mild, but some evolve into severe dengue (SD), with high lethality. Therefore, it is important to identify biomarkers of severe disease to improve outcomes and judiciously utilize resources. METHODS/PRINCIPAL FINDINGS: One hundred forty-five confirmed dengue cases (median age, 42; range <1-91 years), enrolled from February 2018 to March 2020, were selected from an ongoing study of suspected arboviral infections in metropolitan Asunción, Paraguay. Cases included dengue virus types 1, 2, and 4, and severi

Fluid Resuscitation and Inotropic Support in Patients With Septic Shock Treated in Pediatric Emergency Department: An Open-Label Trial.

Introduction Fluid resuscitation and inotropic support are essential interventions to improve cardiovascular function in patients with septic shock. However, the optimal volume of fluids and the timing of inotropic support to achieve the resolution of shock are controversial. They may depend on the availability of critical care support services.  Aims To compare early versus the delayed start of epinephrine administration after fluids bolus in children with septic shock.  Methods  We conducted an open-label randomized trial in which patients under 18 years of age diagnosed with septic shock an

Identification of a non-host semiochemical from tick-resistant donkeys (Equus asinus) against Amblyomma sculptum ticks.

Amblyomma sculptum is a tick affecting animal and human health across Argentina, Bolivia, Paraguay and Brazil. Donkeys, Equus asinus, are known to be resistant to A. sculptum, suggesting that they can produce non-host tick semiochemicals (allomones), as already demonstrated for some other vertebrate host/pest interactions, whereas horses, Equus caballus, are considered as susceptible hosts. In this study, we tested the hypothesis that donkeys produce natural repellents against A. sculptum, by collecting sebum from donkeys and horses, collecting the odour from sebum extracts, and identifying do

MLST and Whole-Genome-Based Population Analysis of Cryptococcus gattii VGIII Links Clinical, Veterinary and Environmental Strains, and Reveals Divergent Serotype Specific Sub-populations and Distant Ancestors.

The emerging pathogen Cryptococcus gattii causes life-threatening disease in immunocompetent and immunocompromised hosts. Of the four major molecular types (VGI-VGIV), the molecular type VGIII has recently emerged as cause of disease in otherwise healthy individuals, prompting a need to investigate its population genetic structure to understand if there are potential genotype-dependent characteristics in its epidemiology, environmental niche(s), host range and clinical features of disease. Multilocus sequence typing (MLST) of 122 clinical, environmental and veterinary C. gattii VGIII isolates

Lonomia genus caterpillar toxins: biochemical aspects.

In 1967 we reported for the first time five cases of an acquired bleeding disorder in humans which developed after contact with saturnidae caterpillars. Since that time, other cases have been reported in Brazil, French Guyana, Peru, Paraguay and Argentina. The caterpillars have been identified as Lonomia achelous (LA) in Venezuela and northern Brazil and as Lonomia obliqua (LO) in southern Brazil. All patients present pain and a burning sensation at the site of contact. Within a few hours hematomas and hematuria are seen in combination with intracerebral and intraperitoneal hemorrhage (in some

Purifying selection constrains the evolution of Juquitiba virus in wild Oligoryzomys nigripes communities.

Juquitiba virus (JUQV) is endemic in Oligoryzomys nigripes across several South American countries and causes hantavirus pulmonary syndrome when transmitted to humans via infectious saliva or excreta. We developed a next-generation sequencing (NGS) pipeline to generate the first complete reference genome for assessing the genetic diversity of JUQV in Oligoryzomys populations inhabiting the Mbaracayú Biosphere Reserve within the Atlantic Forest of Paraguay. From 32 additional Oligoryzomys specimens, we obtained 17 S- and M-segment viral RNA (vRNA) genomes from lungs with 94-100% sequence covera

Population Heterogeneity of Diabetes in Indigenous Peoples of the Americas: A Systematic Scoping Review of the Existing Literature.

Background: In the Americas, the number of people living with diabetes is expected to rise from 92 million in 2024 to 120 million by 2050. Indigenous populations may experience distinct biological, environmental, and sociocultural risk factors; however, they are often treated as a homogeneous group in epidemiological research, and consolidated evidence on diabetes prevalence across diverse Indigenous populations remains limited. This scoping review examines the prevalence of diabetes among Indigenous populations in the Americas. Methods: Following PRISMA-ScR guidelines, we conducted a systemat

Association between risk-adjusted mortality and severe morbidity in very low birth weight infants: a multicenter study from the NEOCOSUR network.

BACKGROUND: Very low birth weight infants (VLBWI) are at high risk of mortality and severe morbidities. While many neonatal networks monitor risk-adjusted outcomes separately, the relationship between mortality and major morbidities across centers remains unclear. OBJECTIVE: To evaluate the association between risk-adjusted in-hospital mortality and six major morbidities in VLBWI across 20 South American NICUs participating in the NEOCOSUR Network between 2011 and 2023. METHODS: Retrospective cohort study including 14,100 VLBWI (500-1500 g) from 20 NICUs in Argentina, Chile, Paraguay, Peru,

Quadruple therapies show a higher eradication rate compared to standard triple therapy for Helicobacter pylori infection within the LEGACy consortium. A multicenter observational study in European and Latin American countries.

INTRODUCTION: Gastric cancer (GC) is one of the most lethal malignancies worldwide. Helicobacter pylori is the primary cause of GC; therefore, its eradication reduces the risk of developing this neoplasia. There is extensive evidence regarding quadruple therapy with relevance to the European population. However, in Latin America, data are scarce. Furthermore, there is limited information about the eradication rates achieved by antibiotic schemes in European and Latin American populations. OBJECTIVE: To compare the effectiveness of standard triple therapy (STT), quadruple concomitant therapy (

Fluvial avulsions influence soil fertility in the Pantanal wetlands (Brazil).

River avulsions drive important changes in the Pantanal wetlands, owing to their role in the hydro-sedimentology of the region. Although relevant to numerous ecosystem services, few studies have analyzed the influence of river avulsions on soil fertility in the Pantanal. Here, we use the largest ongoing avulsion in the Taquari River (Caronal region) to evaluate the effects on soil fertility, considering two factors: avulsion stage (1) and aquatic-terrestrial succession (2). Since both factors are influenced by macrophyte abundance, an incident map was created through tasseled cap indices from

Ecological Niche Modelling Predicts Southward Expansion of Lutzomyia (Nyssomyia) flaviscutellata (Diptera: Psychodidae: Phlebotominae), Vector of Leishmania (Leishmania) amazonensis in South America, under Climate Change.

Vector borne diseases are susceptible to climate change because distributions and densities of many vectors are climate driven. The Amazon region is endemic for cutaneous leishmaniasis and is predicted to be severely impacted by climate change. Recent records suggest that the distributions of Lutzomyia (Nyssomyia) flaviscutellata and the parasite it transmits, Leishmania (Leishmania) amazonensis, are expanding southward, possibly due to climate change, and sometimes associated with new human infection cases. We define the vector's climatic niche and explore future projections under climate cha

Leptospira spp. in Naturally Infected Dairy Cow from a Brazilian Border Region.

Leptospirosis is an important infectious disease, which can generate large economic losses, especially in the dairy herd. The pathogen that causes this disease may have its entry in Brazilian herds facilitated by the existence of a large extension of land borders. Therefore, the objective of this work was to investigate the presence of DNA and antibodies against Leptospira spp. in samples of vaginal mucus and serum from naturally infected bovine females from small rural dairy farms in a border region. Blood and vaginal mucus samples were collected from 70 Holstein cows, from small rural dairy

Influence of mate drinking, hot beverages and diet on esophageal cancer risk in South America.

To estimate the effects of consuming hot beverages, including mate (an infusion of the herb Ilex paraguayensis), tea, coffee and coffee with milk, and other food items on esophageal cancer risk, we analyzed data from 830 cases and 1,779 controls participating in a series of 5 hospital-based case-control studies of squamous-cell carcinoma of the esophagus conducted in high-risk areas of South America. After adjusting for the strong effects of tobacco and alcohol consumption, both heavy mate drinking (>1 l/day) and self-reported very hot mate drinking were significantly associated with esophagea

[Mitochondrial 2-methylacetoacetyl-CoA thiolase deficiency in Argentina].

From the description of two pairs of siblings belonging to unrelated families, one Argentine family with a history of consanguinity and Irish ancestry and the other family native of Paraguay, in whom mitochondrial 2-methylacetoacetyl-CoA thiolase deficiency, commonly known as beta-ketothiolase deficiency (beta-KTD, McKusick 203750; EC 2.3.1.9) was recognized. We tried to outline through this experience the clinical and biochemical consequences of this genetic defect in the 6th step of the isoleucine catabolism. The phenotyoic expression presented by the patients belonged to the classical form
