Why monitor perilymphatic pressure in Menière's disease?

A cohort of 39 patients with confirmed raised intracranial pressure was investigated. Of these patients, 24 (62%) complained of tinnitus and 11 (28%) suffered from paroxysmal rotary vertigo. Intracranial hypertension can occur without the usual headache and visual symptoms. In such cases, the patient may be referred to the otolaryngological clinic and the condition may be mistaken for Menière's disease or a labyrinthine disorder. The Tympanic Membrane Displacement (TMD) technique now provides a non-invasive method of monitoring the intracranial and perilymphatic pressures. This study provides

The impedance bridge and ENG controlled fistula test: results in children with unilateral deafness and comparison with normal controls.

The diagnosis of a perilymph fistula is normally made on clinical grounds. Thus in most cases suspicion is raised by the clinical history and the presence of a fistula confirmed at operation. In children such fistulae may be relatively silent and the diagnosis difficult to establish. This paper reports the results of using an impedance bridge and ENG-controlled fistula test in the routine screening of 24 children with unilateral sensorineural deafness, and compares the results obtained to those found from testing 21 normal and asymptomatic children of comparable age and sex. These tests have s

The metabolic reaction of the cochlea to unphysiological noise exposure.

Guinea pigs were exposed to continuous white noise of 120 dB SPL. The performance included a recording of CM, pO2, and metabolites in the perfusate of the perilymphatic spaces. The metabolic reactions observed following the exposure are limited. No evidence of an overstimulation or of an insufficiency of the energy metabolism in the cochlea could be found. The results suggest the metabolic processes to be secondary to the sound-dependent damage of the organ of Corti after irreversible mechanical destructions in the hair cells.

10 years of Vertigo Clinic at National Hospital Abuja, Nigeria: what have we learned?

The clinician's major role in management of the dizzy patient involves determining what dizziness is vertigo, and what vertigo is of central or peripheral origin. These demand attention to details of history, otolaryngological workup including vestibular assessment, and often use of diagnostic and management algorithms. There is paucity of published reports of the management outcomes of peripheral vestibular diseases from Africa. Two tertiary care otologist-led dedicated vertigo clinics are located in Abuja, Nigeria. A prospective, non-randomized study of patients presenting with features of p

Preservation of Inner Ear Function in a Case of Vestibular Fistula Caused by Cholesteatoma Using the Underwater Technique.

A 70-year-old male presented with intermittent vertigo, dizziness, and left facial spasms. Otoscopic examination and temporal bone computed tomography revealed extensive cholesteatoma with lateral semicircular canal erosion and a vestibular fistula. Preoperative audiometry demonstrated severe mixed hearing loss with partial preservation of bone conduction thresholds. The patient underwent canal wall-down tympanomastoidectomy using the underwater technique, which involved continuous saline irrigation to stabilize the inner ear environment and minimize perilymph leakage. The fistula site was sea

Auditory nerve function following cochleitis.

We present a case of temporary cochlear nerve dysfunction due to endocochlear inflammation with subsequent recovery. Retrospective case review at a pediatric tertiary care hospital. A nine-year-old male presented seven years post-cochlear implantation with an electrode array extruded into the external auditory canal. Upon exploration in the operating room, turbid perilymph from the scala tympani was discovered. A new electrode array was implanted. The patient had no discernible neural response telemetry (NRT) responses or auditory perception immediately following the procedure. Continuous but

Direct inner ear infusion of dexamethasone attenuates noise-induced trauma in guinea pig.

The protective effect of dexamethasone (DEX) against noise-induced trauma, as reflected in hair cell destruction and elevation in auditory brainstem response (ABR) sensitivity, was assessed in guinea pigs. The animals were administered DEX (1, 10, 100, and 1000 ng/ml) or artificial perilymph (AP) via a mini-osmotic pump directly into scala tympani and, on the fourth day after pump implantation, exposed to 120 dB SPL octave band noise, centered at 4 kHz, for 24 h. Animals receiving DEX demonstrated a dose-dependent reduction in noise-induced outer hair cell loss (significant at 1, 10 and 100 ng

Fragile bones and fragile ears.

Conductive, sensorineural and mixed hearing loss occur in osteogenesis imperfecta in autosomal dominant inheritance pattern. Hearing loss is generally due to the middle and inner ear pathology of osteogenesis imperfecta and only occasionally to the coincidental association of otosclerosis and osteogenesis imperfecta. Two lesions cause the conductive hearing loss of osteogenesis imperfecta: (1) functional ossicular discontinuity due either to stapes superstructure fracture or fibrous replacement, or (2) thick, crumbly, lightly fixed stapes footplate. Cochlear hair cell loss, stria vascularis at

Close Packing of Cells in Vestibular Epithelia Supports Local Electrical Potentials that Reduce Latency of Action Potential Generation.

In the vestibular system, upon transduction of head motion, ionic currents from type I sensory hair cells alter [K+] and electrical potentials in an extended synaptic cleft formed by a calyx terminal of the associated afferent neuron. During excitatory stimuli, these changes in turn modulate post-synaptic currents across the calyx inner face to depolarize the afferent and initiate action potentials. Within the tightly packed columnar vestibular sensory epithelium, electrical currents from the hair cell and calyx must also traverse non-synaptic extracellular spaces and generate local extracellu

Subclinical diagnosis of cisplatin-induced ototoxicity with biomarkers.

A mouse model with cisplatin-induced ototoxicity was used in addition to human samples from the ITMAT Biobank at the University of Pennsylvania. Mouse auditory brainstem responses (ABR), inner ear histology, perilymph cisplatin sampling, and measurement of serum prestin via ELISA were performed. Human serum prestin level was measured via ELISA in patients with otological issues after cisplatin treatment and compared to matched controls. Serum prestin was significantly elevated before ABR threshold shifts in mice exposed to cisplatin compared to control mice. Prestin concentration also correlat

Successful hearing improvement with direct acoustic stimulation in a patient with schizophrenia.

OBJECTIVE: A direct acoustic cochlear implant provides its power directly to the inner ear by vibrating the perilymph via a conventional stapes prosthesis. Our experience with a patient with severe mixed hearing loss due to otosclerosis is described.Case reportThe patient, a 47-year-old male, had a pre-operative speech recognition score of 10 per cent and had been treated for many years for schizophrenia, both of which made him a poor candidate for a direct acoustic stimulation device. Nevertheless, the surgery was performed, which preserved the pre-operative bone conduction level and signific

[Surgical treatment of vestibular vertigo: methods and indications].

After the failure of medical treatment of dizziness or vertigo, surgical treatment is chosen according to functional diagnosis and duration and severity of symptoms. Surgery is contraindicated in patients with incomplete vestibular compensation, central nervous disorders and bilateral vestibular deficits. Surgery is obligatory in cases of sudden loss of labyrinthine function with traumatic perilymph fistula, middle ear and temporal bone processes. Endolymphatic hydrops and pathological endolymph pressure of other etiology as impaired middle ear ventilation, EVAS or superior semicircular canal

Noise-induced hearing loss in chinchillas pre-treated with glutathione monoethylester and R-PIA.

The protective effects of glutathione monoethylester (GEE) and GEE in combination with R-N6-phenylisopropyladenosine (R-PIA) were evaluated in the chinchilla when exposed to impulse (145 dB pSPL) or continuous (105 dB SPL, 4 kHz OB) noise. Six groups of 10 chinchillas were used as subjects. Before exposure to noise, the subjects were anesthetized, a 30 microl drop of drug was placed on the round window (GEE [50, 100, 150 mM], GEE 50 mM and R-PIA). Forty minutes later the subject was exposed to either impulse or continuous noise. The 50 mM treatment provided significant protection from impulse

A reexamination of experimental type II collagen autoimmunity: middle and inner ear morphology and function.

Type II collagen autoimmunity has been reported to result in a number of middle and inner ear morphological and functional abnormalities. We investigated the immunological, electrophysiological, and anatomical effects of this autoimmune state in well-established Wistar-Furth rat models of type II collagen autoimmune arthritis. Seventeen animals immunized with bovine type II collagen were divided into short term (1-3 months postimmunization) and long term (9-11 months) groups. Thirty-three experimental ears were tested electrophysiologically and were compared to nine ears of unimmunized Wistar-

Immediate noise effects on cochlear vasculature in the guinea pig.

Nine guinea pigs were exposed to a 4 kHz narrow band noise at 120 dB for 30 minutes. The animals were killed immediately following exposure and the cochlear tissues prepared using a soft-surface preparation technique. Experimental and control animals were mixed and randomly assessed without prior knowledge of the groups of which the specimens belonged. The results were analysed by computer. Statistically significant differences in the vasculature between noise-exposed and control animals of a general nature included: more changes in spiral lamina than external wall vessels, more changes in ves

Hypothetical mechanism for vertigo in Meniere's disease.

Past theories that have been proposed to account for the attacks of vertigo during the course of Meniere disease are reviewed. In the past, vascular theories and theories of perilymph and endolymph mixing due to ruptures or leakages were proposed. Recent research concerning the basic mechanisms of the inner ear anatomy and function cast doubt on these theories. The anatomy, physiology, and pathophysiology of the inner ear, and in particular of the endolymphatic sac and endolymphatic duct are reviewed. Recent studies suggest that in people the endolymph ionic content is replenished without any

Perilymphatic fistula in cabin attendants: an incapacitating consequence of flying with common cold.

A perilymphatic fistula (PLF) is an abnormal communication between the inner ear and the middle ear that leaks perilymph. PLF is considered rare, but is known to occur during childbirth, straining, weightlifting, head trauma, and diving with middle ear equalizing problems. It has also, anecdotally, been described in connection with flying. The symptoms are uncharacteristic vertigo and, in some cases, hearing impairment and tinnitus. This study describes four cases of PLF during a period of 6 mo in a major Scandinavian airline company employing approximately 3000 cabin attendants (CAs). None of

K+-induced motility and depolarization of cochlear hair cells. Direct evidence for a new pathophysiological mechanism in Ménière's disease.

In addition to an endolymphatic hydrops in various models of Ménière's disease, ruptures of the membranes lining the endolymphatic spaces or massive changes in their biochemical permeability are believed to allow large amounts of potassium-rich endolymph to deluge the normally low [K+] perilymphatic fluid. Our clinic has shown that exposure of the lateral and basal membrane parts of living isolated outer hair cells to increasing concentrations of potassium ions resulted in a sustained reversible cellular depolarization. Furthermore, potassium intoxication was accompanied by a strictly longitud

Hypothyroidism and the ear: electrophysiological, morphological, and chemical considerations.

There is both clinical and laboratory evidence that hearing loss can result from congenital and acquired hypothyroidism. The reversibility of this process, however, and its incidence and pathophysiology are not universally agreed upon. Laboratory animals rendered hypothyroid with radioactive iodine 131 or propylthiouracil demonstrated normal perilymph sodium and potassium levels but increased auditory thresholds for N1N2 response and brain stem evoked audiometry as well as a crystallized consistency of the bone of the bullae and cochleae, ossicular abnormalities, obliteration of the oval and r
