Pharyngeal reconstruction using a U-shaped pectoralis major myocutaneous flap: an effective technique that should not be forgotten.

INTRODUCTION: Although free tissue transfer is thought the best option for head and neck reconstruction, the pectoralis major myocutaneous flap (PMMCF) remains an important alternative method. The aim of this study was to assess the use of the PMMCF with the prevertebral fascia to close a pharyngeal defect. MATERIALS AND METHODS: This was a retrospective study of 30 patients who underwent circumferential pharyngeal defect reconstruction with a U-shaped PMMCF from 2009 to 2018. The flap was primarily used to reconstruct defects after tumor extirpation. RESULTS: One patient (3.3%) died of an a

Long-term analysis of transorally resected p16 + Oropharynx cancer: Outcomes and prognostic factors.

OBJECTIVE: We observed high survival in a previous report of a p16-positive, oropharyngeal carcinoma (OPC) cohort treated primarily with transoral laser microsurgery (TLM) ± adjuvant therapy and followed for ≥ 12 months. To address long-term outcomes of primary transoral surgery for this unique disease, we present an updated analysis of our cohort with extended follow-up. METHODS: A prospectively assembled TLM cohort of 171 OPC patients was analyzed for disease-free, disease-specific, and overall survival (disease-free survival [DFS], disease-specific survival [DSS], overall survival [OS]) an

Endovascular coiling of a mycotic external carotid artery pseudoaneurysm following pharyngolaryngectomy with a free jejunal graft.

OBJECTIVES: (1) To highlight the significance of carotid artery pseudoaneurysm as a rare complication following neck dissection, and (2) to suggest endovascular coiling as management, in the presence of infection, previous radiotherapy and a grafted blood supply. CASE REPORT: A 66-year-old man diagnosed with squamous cell carcinoma of the hypopharynx and upper oesophagus underwent pharyngolaryngectomy with reconstruction of a neo-pharynx using a free jejunal graft. The patient had previously received radiotherapy for a soft palate squamous cell carcinoma. Two months after surgery, computed to

The surgical dilemma of 'functional inoperability' in oral and oropharyngeal cancer: current consensus on operability with regard to functional results.

OBJECTIVES: If surgical resection of a tumour results in an unacceptable loss of function, this is defined as 'functional inoperability'. The current survey aims to define the borders of functional inoperability in oral and oropharyngeal carcinoma and evaluate its current use by obtaining opinions from the experts in the field. DESIGN: A web base survey. PARTICIPANTS: Dutch head-neck surgeons and radiotherapists. MAIN OUTCOME MEASURES: Assessment of functional results after tumour resection in twenty-two statements and five cases. RESULTS: Response on the survey is 93% and the reactions ar

Lateral upper arm free flap for primary reconstruction of pharyngeal defects in ablative oncological surgery. Report of six consecutive cases.

Free microvascular flaps are an established method for soft tissue reconstruction following ablative oncological surgery in the head and neck. Functional reconstructions of the hypopharynx and the pharyngoesophageal segment (PES) are of particular relevance, as they are highly demanding surgical procedures. So far, the radial forearm free flap (RFFF) and the free jejunal transfer have been the transplants predominantly used for this purpose. The lateral upper arm free flap (LUFF) presents an alternative method for the fasciocutaneous tissue transfer. We report on our experience with the LUFF i

Transoral carbon-dioxide laser resection of hypopharyngeal carcinoma.

In hypopharyngeal carcinoma, open partial resection is rarely feasible because of the high rate of chronic aspiration. Transoral laser surgery, however, is not associated with major swallowing problems. Between 1991 and 1995, a total of 29 patients with cancer of the hypopharynx were treated using laser surgical resection. In 25 patients, a neck dissection was performed, and 26 patients were radiated postoperatively. The 5-year overall survival was 48% and tumor-related survival 58%. The outcome was significantly ( P<0.048) dependent on the preoperative lymph node status (N(0)=74%, N(+)=34% ov

Neck and mediastinal node dissection in pharyngolaryngoesophageal tumors.

BACKGROUND: Specific reports about neck node metastasis in cervical esophageal tumors and mediastinal node metastasis in patients with pharyngolaryngoesophageal tumors are lacking. This study was undertaken to evaluate the need for neck and mediastinal lymph node dissection when dealing with carcinomas of this region. METHODS: A retrospective review of the records of 34 patients who underwent total pharyngolaryngoesophagectomy and gastric transposition (TPLEGT) for squamous cell carcinoma of the pharyngoesophageal junction was done. Sixteen patients had esophageal carcinomas, 14 had hypophary

Current status of pharyngolaryngo-esophagectomy and pharyngogastric anastomosis.

BACKGROUND: Pharyngolaryngo-esophagectomy and pharyngogastric anastomosis (PLO & PGA) is one of the surgical options in the management of tumors arising from the hypopharynx and cervical esophagus. Indications of the operation and the outcome are changing over the years. To examine these, the experience of this operation in one Institute (the Head and Neck Division of the Department of Surgery, The University of Hong Kong at Queen Mary Hospital, Hong Kong) over the last 30 years was reviewed. METHODS: From 1966 to 1995, a total of 317 patients underwent PLO & PGA. The clinical results of 69 p

[Treatment of carcinoma of the piriform sinus].

Sixty patients with carcinoma of the piriform fossa were treated with surgery and postoperative irradiation, and 22 other patients with incurable disease were irradiated. All but 2 patients primarily irradiated died within a year. 87% of the 60 patients who were treated by primary surgery had histologically proven lymph node metastases. Twenty six of these patients suffered a local recurrence, regional metastases or distant metastases within 18 months. They were all dead within 2 years of diagnosis of the primary tumor. 75% of patients in this group had shown capsular rupture by metastases. Th

A novel surgical T staging system for recurrent nasopharyngeal carcinoma: advancing prognostic accuracy and clinical applicability.

OBJECTIVE: The aim of this study was to develop and validate a novel surgical T (sT) staging system for recurrent nasopharyngeal carcinoma (rNPC) that better reflects anatomical barriers to resection and improves prognostic accuracy compared with the American Joint Committee on Cancer (AJCC) staging system. METHODS: Cadaveric dissections and a retrospective analysis of 211 patients with rNPC undergoing endoscopic nasopharyngectomy were conducted. RESULTS: The proposed sT staging system incorporates 3 key anatomical barriers-the pharyngobasilar fascia (PBF), interpterygoid fascia (IPF), and d

Mucosal Flap Reconstruction Enhances Survival in Salvage Endoscopic Nasopharyngectomy for Recurrent Nasopharyngeal Carcinoma: A Propensity-Matched Cohort Study.

Mucosal flap reconstruction can improve the long-term survival rate of patients with recurrent nasopharyngeal carcinoma. Mucosal flap reconstruction also increases the treatment-related risks of ear fullness and dysphagia. This matching study provides preliminary evidence for the use of mucosal flap reconstruction in salvage endoscopic nasopharyngectomy.

Fascia lata graft closure of an enlarged tracheoesophageal puncture (TEP) after laryngectomy/laryngopharyngectomy.

Enlargement of tracheoesophageal puncture (TEP) is seen in 1-29% of laryngectomee/laryngopharyngectomee using tracheoesophageal voice. It predisposes patient for recurrent aspiration pneumonia. Surgical closure of TEP is required in 30-67% of patients not responding to conservative measures. Surgical closure of an enlarged TEP using a flap may be time consuming, complex, and logistically challenging to organize. A novel technique of Fascia lata graft closure of an enlarged TEP is presented here. This technique can be useful in carefully selected patients with an enlarged TEP.

Pharyngeal angiosarcoma following multimodal treatment for oropharyngeal squamous cell carcinoma.

It is well established that angiosarcoma can develop following radiotherapy. We present an unusual case of angiosarcoma of the pharynx that developed three years after treatment with surgery and adjuvant chemoradiotherapy for a T2N2bM0 squamous cell carcinoma of the oropharynx. The patient was tumour free until developing dysphagia, which was found to be caused by an angiosarcoma. The patient underwent surgery of the pharyngeal angiosarcoma by laryngopharyngectomy, tongue base resection, selective neck dissection and radial forearm microvascular free flap reconstruction. Angiosarcoma following

Advanced carcinoma of the hypopharynx: functional results after circumferential pharyngolaryngectomy with flap reconstruction.

Surgical treatment of advanced cancers of the hypopharynx inevitably impairs swallowing, respiration and phonation. The purpose of this study was to analyze the functional results after circumferential pharyngolaryngectomy (CPL) and flap reconstruction, in order to offer decisional guidelines for the choice of the most effective reconstructive method. We performed a retrospective analysis on the medical records of patients submitted to reconstructive surgery after CPL from July 1991 to November 2011. 75% of the 94 patients underwent reconstruction with a free flap (group A), while 25% underwen

External monitoring of buried radial forearm free flaps in hypopharyngeal reconstruction.

CONCLUSION: The radial forearm free flap (RFFF) is one of the optimal choices for hypopharyngeal reconstruction. Our series demonstrates that the technique of an indicator flap for the monitoring of the buried flap is simple, reliable, and inexpensive. The condition of the indicator flap can be easily interpreted by the physicians and the nursing staff. Therefore, the success rate of this microsurgical reconstruction may be improved. OBJECTIVES: The RFFF is increasingly applied in reconstruction of the hypopharynx after radical resection for advanced hypopharyngeal cancer. However, postoperat
