Investment in HIV/AIDS programs: does it help strengthen health systems in developing countries?

BACKGROUND: There is increasing debate about whether the scaled-up investment in HIV/AIDS programs is strengthening or weakening the fragile health systems of many developing countries. This article examines and assesses the evidence and proposes ways forward. DISCUSSION: Considerably increased resources have been brought into countries for HIV/AIDS programs by major Global Health Initiatives. Among the positive impacts are the increased awareness of and priority given to public health by governments. In addition, services to people living with HIV/AIDS have rapidly expanded. In many countrie

The BMP antagonist Gremlin regulates outgrowth, chondrogenesis and programmed cell death in the developing limb.

In this study, we have analyzed the expression and function of Gremlin in the developing avian limb. Gremlin is a member of the DAN family of BMP antagonists highly conserved through evolution able to bind and block BMP2, BMP4 and BMP7. At early stages of development, gremlin is expressed in the dorsal and ventral mesoderm in a pattern complementary to that of bmp2, bmp4 and bmp7. The maintenance of gremlin expression at these stages is under the control of the AER, ZPA, and BMPs. Exogenous administration of recombinant Gremlin indicates that this protein is involved in the control of limb out

Proposal to develop a detailed safety program for general/laser surgical patients infected with AIDS.

Medical health personnel have acquired serious and rarely fatal infections in the operating room from AIDS patients. Yet, there are no officially approved AIDS safety programs designed to protect medical health care personnel in the operating room. A sequential safety program is proposed beginning with the following steps: 1) evaluation and staging of the patient and associated complications; 2) protection of the major surgical and laser instruments; 3) use of an efficient evacuator system to avoid plumes of laser fragments from polluting the operating room environment; 4) protection of the op

[Congenital diaphragmatic hernia: an analysis of the results and prognostic factors prior to the development of an ECMO program].

BACKGROUND: Some neonates with congenital diaphragmatic hernia (CDH) and persistent pulmonary hypertension are not adequately oxygenated with conventional treatment. The extracorporeal membrane oxygenation (ECMO) has been successful in some of them as an alternative in their management. PATIENTS AND METHODS: We studied the charts of 47 neonates with CDH, symptomatic within 24 hours of birth, treated in our institution during the last seven years (1987-1994). In all of them, conventional ventilation and hemodynamic support was used. In 12 patients high frequency ventilation (HFV) was used and

[Heart transplant: development and maturity of the program at the University of South Florida and the General Hospital of Tampa].

Cardiac transplantation has evolved from an experimental procedure to an accepted mode of therapy that prolongs life in patients with severe heart failure. The University of South Florida-Tampa General Hospital began performing cardiac transplantation for the treatment of end-stage cardiac disease in June 1985. Since then, 50 heart transplantations have been performed and 37 patients are alive and well. The one-year actuarial survival is 78% and the two-year actuarial survival is 72%. Multiple complications have been encountered, most notably rejection and infection. The recent approval of the

NuViva: Development of postpartum nutrition and exercise program implementing the FIGO pregnancy passport to reduce cardiometabolic risk.

Pregnancy provides a unique opportunity to identify women at increased risk of future chronic disease, as adverse pregnancy outcomes (APOs) such as hypertensive disorders of pregnancy, gestational diabetes mellitus, preterm birth, placental complications, and fetal growth restriction are associated with later cardiometabolic morbidity. However, postpartum care remains fragmented and limited, representing a missed opportunity for preventive intervention. Currently in pilot phase, NuViva is a novel, multidisciplinary, community-based postpartum program, providing nutrition, physical exercise, an

Charge Your Brainzzz: the systematic development of a whole systems action program promoting sleep health in adolescents.

BACKGROUND: Inadequate sleep among Dutch adolescents is a complex public health issue with detrimental effects on physical and mental well-being. Previous interventions have shown limited or no lasting effects. While systems approaches help to understand and address such challenges, effective prevention efforts require theory- and evidence-based intervention design using behavior change techniques. This study outlines the systematic development of a ‘whole systems action program’, named Charge Your Brainzzz (CYB), to promote healthy sleep in Dutch adolescents aged 12–15, by integrating theory-

Development of a health literacy-based hypertension self-management education program using sign language for Deaf individuals.

BACKGROUND: While there is some research on hypertension self-management programs for individuals with profound hearing loss who use sign language as their primary mode of communication (commonly referred to as Deaf), there is no systematic program that considers their unique needs and health literacy. Given the high prevalence of hypertension among Deaf individuals and their vulnerability to limited health literacy, we developed the Hypertension Self-Management Education for the Hearing-Impaired using Sign Language (H-SMILE) program, focusing on health literacy. METHODS: Following the Analys

Development of an automated foot contact area measurement program for podoscopes using ChatGPT-4: a case report.

Accurate measurement of the foot contact area is crucial for diagnosing pes planus (flatfoot) and pes cavus (high arch), which significantly affect pressure distribution across the plantar surface. This study aimed to develop a program using ChatGPT-4 to automate foot contact area measurements using a podoscope, thereby enhancing diagnostic precision. A 53-year-old female volunteer stood on a podoscope to capture images of her feet, which were processed to isolate the foot contours and measure the contact areas. A program developed utilizing ChatCPT-4 was designed to outline the feet, detect c
