The purpose of the present study was to analyze the effects of early orthopedic intervention in patients with hemifacial microsomia, clinically as well as radiologically, by computed tomographic examination to assess soft and hard tissue temporomandibular joint changes. Five patients, one with mandibular dysostosis, one with otomandibular dysostosis, two with Goldenhar syndrome, and one with a surgically revised fibroma of the right mandible that was reconstructed by a costochondral graft, were treated by means of an activator functional appliance. During functional appliance treatment, all pa

Although the effects of oral respiration on the growth and development of craniofacial structure have been studied previously, little is known about how altered respiration affects the activity of the jaw-closing muscles. Obstruction of the nasal airway in the cat significantly inhibited the masseteric stretch reflex and discharges of masseteric motor units but did not affect the electromyographic activity of the diaphragm. This inhibition was greater during inspiration than during expiration. In addition, the amplitude of the masseteric monosynaptic reflex elicited by electrical stimulation o

The long-term effects of the Herbst appliance on the temporomandibular joint were assessed in 20 patients (10 girls and 10 boys) who had completed treatment an average of 4 years previously. The TMJ analysis comprised of: (1) an anamnestic questionnaire (2) a clinical investigation (manual functional analysis) and (3) magnetic resonance imaging of the left and right joints. The results revealed that the incidence of anamnestic and clinical signs and symptoms of temporomandibular disorders was within the range of "normal" reported in the literature. The frequency of disk displacement was not hi

Anterior open bites may be localized dental manifestations, which are caused by habits or skeletal disharmony, with or without functional aberration. Previous studies suggest various associations between open bites and underlying etiologic factors. We hypothesize that respiratory efficiency may be associated with anterior open bites. Under the assumption that breathing efficiency of the oropharynx and hypopharynx may be related to pharyngeal airway length, a cephalometric variable, vertical airway length (VAL), was measured on lateral cephalograms obtained from a total of 58 subjects with, so

The purpose of this study was to acquire tooth movement, histomorphometric and biochemical data on oral tissues that had previously been loaded with calibrated orthodontic forces. One hundred and forty-four male Sprague-Dawley rats were randomly divided into two groups: Group I, orthodontic appliances placed for 16 days to mesially move maxillary first molars with an initial force of 40 gm, and group II, sham orthodontic treatment. Seven to twelve rats were killed at each of six times after removal of appliance. Tooth movement was measured cephalometrically, alveolar bone turnover by histomorp

In a retrospective study, 90 American Board of Orthodontic (ABO) cases were evaluated for treatment outcome. Changes in occlusion, cephalometric skeletal and dental variables, soft tissue variables, and root resorption were evaluated. The occlusions of completed ABO cases were compared with 147 naturally occurring good-to-excellent occlusions from the Andrews Foundation for Education and Research, using the Ideal Tooth Relationship Index (ITRI). Cephalometric variables were evaluated in relation to an "acceptable range" based on established standards. Photographs were evaluated for lip posture

The increased use of implants in orthodontics has stimulated interest in augmenting bone in patients who have deficient alveolar ridges that preclude ideal implant placement. A nonsurgical technique for increasing the amount of available bone for implant site development and fixture placement is orthodontic extrusion, or forced eruption. The concept of a tooth moving coronally by orthodontic means and the clinical alterations in the soft tissue architecture of the periodontium demonstrated during orthodontic extrusive movement of periodontally compromised teeth have demonstrated a direct relat

A case report is presented of a female adolescent with a Class II, division 1 malocclusion that shows a significant mandibular arch length deficiency. Although the facial balance suggested a nonextraction approach, the degree of mandibular anterior dental crowding and preexisting gingival recession indicated extraction in the mandibular arch might be preferable.

Norms available for the posteroanterior (PA) cephalometric analysis yield different information. The material used to generate these standards is not clearly defined. The purpose of this study was to use data from the Bolton-Brush Growth Study to generate new norms for the PA analysis. The Bolton material is recognized as a controlled longitudinal record of growing children. The assumption that growth in the transverse dimension is the first to be completed is challenged by finding that increments in maxillary and mandibular width vary, like sagittal and vertical growth, according to gender an

This article presents a new approach to cleft lip and cleft palate repair, where the retained bulk of alveolar cleft grafts is augmented by simultaneous cartilage augmentation of the deficient maxilla. Nine patients who provided 10 cleft sites underwent secondary bone grafting of the clefted alveolus by this technique. This study evaluates the following: (1) the postoperative esthetic results, (2) the quantity of bone in the grafted area, and (3) the provision of bone support for unerupted teeth, for teeth adjacent to the cleft, and for orthodontic tooth movement when the secondary bone grafti

The purpose of this study was to confirm that correction of functional posterior crossbite through maxillary expansion is associated with a change in condylar position and occlusal relationships, and to determine whether maxillary expansion is associated with autonomous increase in mandibular arch width. Pretreatment and posttreatment study models of 61 patients ages 4.1 to 12.0 years (mean 8.5 years, SD 1.5) were available after maxillary expansion with a Quad Helix or a Haas expander for correction of a functional posterior crossbite. Pretreatment and posttreatment tomograms were available f

A group of 25 untreated subjects with Class II malocclusion in the deciduous dentition (featuring the concomitant presence of distal step, Class II deciduous canine relationship, and excessive overjet) was compared with a control group of 22 untreated subjects with ideal occlusion (flush terminal plane, Class I deciduous canine relationship, minimal overbite, and overjet) at the same dentitional stage. The subjects were monitored during a 2 1/2-year period in the transition from the deciduous to the mixed dentition, during which time no orthodontic treatment was provided. Occlusal analysis of

Extraction sites may be needed to achieve specific orthodontic goals of positioning the dentition within the craniofacial complex. The fundamental reality that determines the final position of the dentition, however, is the control exercised by the clinician in closure of the extraction sites. A specific treatment objective may require the posterior teeth to remain in a constant position anteroposteriorly as well as vertically, while the anterior teeth occupy the entire extraction site. Another treatment objective may require the reverse, or any number of purposeful alternatives of extraction

This is a case report of a patient with a skeletal Class III malocclusion and maxillary arch length deficiency. The patient was treated without extraction or surgery by increasing the maxillary arch length. Protraction of the maxillary complex and A point was the result. Favorable growth of both the maxilla and the mandible resulted in a functional Class I occlusion and an improved skeletal relationship. [This case report was presented to the American Board of Orthodontics in partial fulfillment of the requirements for the certification process conducted by the Board.]

The mandible of a deceased 19-year-old young woman who had been treated with an edgewise appliance was removed during autopsy. The overall tooth movements during the 19 months of treatment were reconstructed (treatment documents) and compared with the macroscopic, radiologic, and micromorphologic findings of the incisor/alveolar bone/symphysis complex of the dry mandible. The initial lateral cephalogram revealed an extremely narrow and high symphysis, with an incisor position straight above the thin bone. During treatment, the incisors had been moved to lingual (lingual root torque) and derota

The purpose of the present study was to determine whether pretreatment measures of facial and dentoalveolar structure can be used to predict apical root shortening liability. Four hundred patients were randomly selected from the files of the Orthodontic Clinic at the Saint Louis University Health Sciences Center. Pretreatment and posttreatment periapical radiographs of maxillary central incisors and pretreatment lateral cephalometric radiographs were traced and measured. A large amount of apical root shortening was found in 2% of the sample. A relationship that was weak but highly significant
