OBJECTIVES: Although it has been well established that the prevalence of and severity of hearing loss increase with age, the contribution of familial factors to age-related hearing loss cannot be quantified. This is largely because hearing loss in older people has both genetic and environmental contributions. As environmental factors play an increasing role with age, it is difficult to delineate the separate contribution of genetic factors to age-related hearing loss. In a population-based survey of hearing loss in a representative older Australian community, we attempted to overcome this usin

Between 1991 and 1996, 35 patients had an intracanalicular tumor: 26 patients were operated via middle cranial fossa approach (MFA: 23 cases) or translabyrinthine approach (TL: 3 cases), 9 cases were supervised by audiovestibular testing and MRI. For the operated patients through MFA the functional hearing (pure tone loss < 50 dB and 100% vocal discrimination) was preserved in 47.8% of cases, facial function was grade I and II in 83% of cases and grade III in 17% of cases. One recurrence was noted and reoperated by TL. For the TL operated patients, the postoperative facial function was normal

Since its introduction in 1999 by Friedrich, the titanium vocal fold medialization implant (TVFMI) is widely used for medialization thyroplasty in glottal closure insufficiency. The purpose of this study was to investigate the long-term functional outcome after medialization thyroplasty using TVFMI. Between 1999 and 2009 123 patients (mean age 55.3 years, 76 male and 47 female) underwent medialization thyroplasty with the TVFMI (96 left, 27 right). For purpose of long-term follow-up, 33 patients could be examined. Prior to surgery, about 8 weeks and at least 1 year after surgery perceptual, ac

Although cleft palate anomaly is frequent, the criterion standards in surgical treatment have not been determined yet. There are a few techniques described for cleft palate repair owing to the limited tissue in the palatal mucosa, the rigid structure of the palatal mucosa, and the limited vascularity of the hard palate. In this study, a novel cleft palate repair technique based on separating the soft palate from the hard palate as a musculomucosal flap and using it as a rotation flap has been described. The operation is evaluated individually for each anomaly because variations occur in the su

CONCLUSION: S These results support previous ones with regard to FN risk factors in VS surgery. However, they also provide new preoperative factors that influence postoperative FN function, such as clinical symptoms, the nature of the surgical procedure (use of laser) and ABR results.
OBJECTIVE: To determine pre- and perioperative factors influencing facial nerve (FN) outcome in vestibular schwannoma (VS) surgery.
MATERIAL AND METHODS: A total of 424 patients undergoing VS surgery were included in this retrospective study. Patients were divided into two groups according to the existence or a

Several surgical approaches to the cerebellopontine angle and internal auditory canal have been developed for the removal of acoustic neuromas. The choice of an approach may be influenced by hearing levels and tumor size. We reviewed the records of the primary translabyrinthine removal of 167 large (> or = 4 cm) acoustic neuromas performed between 1982 and 1990. Patients ranged in age from 15 to 83 years, with a mean of 43 years (male, 49%; female, 51%). Total removal was achieved in 95%. The facial nerve was preserved anatomically intact in 91%. At follow-up (mean, 2.1 yr), facial nerve funct

OBJECTIVES: The digits-in-noise (DIN) test is a validated tool for assessing speech reception thresholds in noise and is widely used in hearing screening. The diotic DIN condition presents identical digits and noise to both ears, whereas the antiphasic condition uses phase-inverted digits, which may enhance sensitivity to specific types of hearing loss. This study aimed to evaluate whether the antiphasic DIN condition better predicts various types of hearing loss than the diotic condition, with a particular focus on its value in identifying patterns suggestive of noise-induced hearing loss in

Speech perception difficulties in noise are common among older adults and individuals with hearing impairment, but also occur among younger adults whose audiometric thresholds are within the clinically-normal range. We examined how aging, cochlear synaptopathy (CS), and outer hair cell (OHC) damage affect speech encoding and phoneme discrimination. Envelope-following responses (EFRs) to rectangular amplitude-modulated (RAM) tones and speech-like phoneme pairs were recorded in quiet using EEG, and behavioral discrimination was assessed in quiet, and with ipsilateral and contralateral noise. Musicians are at risk of hearing loss and tinnitus due to regular exposure to high levels of noise. This level of risk may have been underestimated previously since damage to the auditory system, such as cochlear synaptopathy, may not be easily detectable using standard clinical measures. Most previous research investigating hearing loss in musicians has involved cross-sectional study designs that may capture only a snapshot of hearing health in relation to noise exposure. The aim of this study was to investigate the effects of cumulative noise exposure on behavioural, electrophysiological, an

BACKGROUND: Prospective research in the field of cochlear implants is hampered by methodological issues and small sample sizes. The ELEPHANT study presents an alternative clinical trial design with a daily randomized approach evaluating individualized tonotopical fitting of a cochlear implant (CI).
METHODS: A single-blinded, daily-randomized clinical trial will be implemented to evaluate a new imaging-based CI mapping strategy. A minimum of 20 participants will be included from the start of the rehabilitation process with a 1-year follow-up period. Based on a post-operative cone beam CT scan

The sight and sound of a person speaking or a ball bouncing may seem simultaneous, but their corresponding neural signals are spread out over time as they arrive at different multisensory brain sites. How subjective timing relates to such neural timing remains a fundamental neuroscientific and philosophical puzzle. A dominant assumption is that temporal coherence is achieved by sensory resynchronisation or recalibration across asynchronous brain events. This assumption is easily confirmed by estimating subjective audiovisual timing for groups of subjects, which is on average similar across dif

Electrically evoked compound action potentials (ECAPs) have been used to examine temporal response patterns of the auditory nerve in cochlear implant (CI) recipients. ECAP responses to individual pulses in a pulse train vary across stimulation rates for individual CI users. For very slow rates, auditory neurons have ample time to discharge, recover, and respond to each pulse in the train. As the pulse rate increases, an alternating ECAP-amplitude pattern occurs. As the stimulation rate increases further, the alternating pattern eventually ceases and the overall ECAP amplitudes are diminished,
