Anatomic basis for disk displacement in temporomandibular joint (TMJ) dysfunction.

This study was designed to investigate the site of lateral pterygoid muscle insertion into the temporomandibular joint (TMJ) disk, and the relationship between that attachment and the disk displacement. One hundred and ten TM joints from 78 cadavers were collected and sectioned in the sagittal plane. Sixty-two unilateral specimens were available and 16 additional specimens were selected randomly from the remaining cadavers in which both condyles were available. Thus statistical comparisons were not confounded by autocorrelation and were based on 78 independent specimens. The sites of muscle at

Traumatic root resorption in dentine-immunized mice.

Traumatic root resorption in a mouse model has been shown to coincide with a decline in naturally occurring serum antibody levels to dentin. It has been proposed that root resorption may be dentin antibody mediated. The purpose of this study was to examine the traumatic root resorption response in mice after hyperimmunization with a crude tooth extract (dentin). The hypothesis of this study was that elevated dentin antibody titers would positively correlate with root resorption. Mice were immunized with mouse dentin and controls were sham immunized. All mice were boosted 4 weeks later with or

Serum and alveolar bone phosphatase changes reflect bone turnover during orthodontic tooth movement.

Acid and alkaline phosphatase changes have been demonstrated histochemically and biochemically during the early stages of an orthodontic tooth movement cycle. However, quantitative data on neither alveolar bone nor serum phosphatase changes over an entire tooth movement cycle have been reported. This study examined acid, tartrate-resistant acid (TRAP), and alkaline phosphatase changes in serum and alveolar bone during an orthodontic tooth movement cycle in 288 adult male Sprague-Dawley strain rats. The effect of differing initial force magnitudes on phosphatase changes was also examined. Data

Resorbed lateral incisors adjacent to impacted canines have normal crown size.

The importance of detecting maxillary canine impaction is that it may compromise dental health, particularly since a small but significant proportion of impacted canines is associated with the resorption of the roots of neighboring teeth. It has been shown that the existence of small and peg-shaped lateral incisors is highly correlated with maxillary canine palatal impaction. To date, however, it has not been established whether the root resorption that occurs has a similar correlation or a predilection for one or other types of lateral incisor crown structure. Accordingly, a group of patients

An American Board of Orthodontics case report: treatment of a crowded Class II malocclusion in an adult.

A case report is presented of an adult with a crowded Class II, Division I subdivision left malocclusion. There was a moderate arch length deficiency in both arches. All four canines were prominent and had gingival recession. The patient's profile necessitated nonextraction treatment. Free gingival grafts were placed to prevent further recession and the Class II relationship and crowding were resolved with a positive profile change. [This case was presented to the American Board of Orthodontics in partial fulfillment of the requirements for the certification process conducted by the board.]

A mesially impacted mandibular second molar. Treatment considerations and outcome: a case report.

The uprighting of an impacted mandibular second molar presents special problems that may require auxiliary appliances and the implementation of "therapeutic diagnosis." In this case, the presence of an ectopically positioned third molar required modification of the original treatment plan.

Submentovertex cephalometric norms in male Chinese subjects.

Submentovertex (SMV) cephalograms of a random sample of 32 male Chinese subjects who were 20.1 +/- 1.9 years old and were free of temporomandibular joint symptoms were studied. A submentovertex cephalometric analysis comprised of 5 angular measurements and 10 linear parameters was developed for the purposes of establishing SMV cephalometric norms among our Chinese adult population for clinical usage. With the exception of condylar angulation, the lack of SMV cephalometric norms published in the literature for other parameters precluded comparison with the results of our study. This new SMV cep

Normalization of incisor position after adenoidectomy.

Changes in incisor inclination and position in both jaws of children during the first 5 years after adenoidectomy were studied. The main change is a significant increased labial inclination of the incisors for the adenoidectomy groups. All variables that measure the labiolingual position of the mandibular incisors confirm a significant labial incisor positioning for both sexes. Stepwise regression analysis shows that 41% to 44% of the incisor proclination after adenoidectomy is accounted for by two regressors--sex (female) and increase in the sagittal size of the nasopharynx. The study support

An example of regional variation in the tempos of tooth mineralization and hand-wrist ossification.

Two groups of adolescent orthodontic patients, one from the Midwest and one from the Midsouth, were compared to test the clinical impression that the permanent teeth of southern children form and erupt at significantly later ages. Indeed, a marked difference is documented, with the Midsouth series achieving mineralization stages at least 1 1/2 years later on average. This suggests that regional differences are appreciably greater than previously suspected. Clinical consequences revolve on the use of conventional (generally Northeast-based) norms for tooth formation and eruption and predictive

Long-term occlusal guidance of a severely intoxicated patient with yusho (PCB poisoning): a case report.

The peculiar dentoorofacial characteristics and 12 years of interdisciplinary management of a patient who was severely intoxicated with the man-made chemical polychlorinated biphenyls (PCBs) are described in this case report. Cephalometric measurements showed that the SNA and SNB angles were reduced but that the developments in height and skeletal maturity were in normal ranges. Gingival hyperpigmentation that was caused by high blood PCB concentration was extremely slow to fade. A cystic mass, diffused trabeculae, and irregular calcification, which were shown on the radiograph and which were

The effects of infraocclusion: Part 1. Tilting of the adjacent teeth and local space loss.

The normal arrangement of the transseptal fibers in the dentition is one of a completely horizontal system that links the mesial and the distal aspects of each tooth within the dental arch, from the most distal tooth of one side to the most distal tooth of the other, across the midline. This continuous fiber band is parallel to the occlusal plane, being attached to each tooth coronal to the level of the interradicular alveolar bone crest and apical to the cementoenamel junction. Alteration of the occlusal plane will be mirrored by a similar change in the plane of the transseptal fiber system.

The effects of infraocclusion: Part 2. The type of movement of the adjacent teeth and their vertical development.

This second article discusses how the inclusion of a vertical component (the infraocclusion of the deciduous molar) into the otherwise horizontal band of transseptal fibers affects the vertical development of the two adjacent teeth. From the hypothesis proposed in the first article of this series, it seemed logical to assume that the occlusal heights of the teeth adjacent to the affected tooth would themselves be lowered, when measured to the inferior border of the mandible and compared with the height of the occlusal table of the control side. Secondly, since the vertical component affects on

A long-term comparison of nonextraction and premolar extraction edgewise therapy in "borderline" Class II patients.

The long-term effects of extraction and nonextraction edgewise treatments were compared in 63 patients with Class II, Division 1 malocclusions who were identified by discriminant analysis as being equally susceptible to the two strategies. A lateral cephalogram, study models, and a self-evaluation of the esthetic impact of treatment were obtained from each of the 33 extraction and 30 nonextraction subjects. The average posttreatment interval was 14.5 years. Although the two strategies produced significant, long-lived differences in the convexity of the profile and the protrusion of the dentiti

The role of the condylar cartilage in mandibular growth. A study in thanatophoric dysplasia.

A new approach to evaluate the role of the condyle in mandibular growth could be its study in chondrodysplasias. The growth of the condylar cartilage and the mandible has not previously been reported in thanatophoric dysplasia (TD), a lethal osteochondrodysplasia. We have studied the light microscopic, histomorphometric, and radiologic findings in four infants affected by TD and in four control infants. The diagnosis was made on the basis of clinical, radiographic, and pathologic criteria. All the measured radiographic parameters of the patients' mandibles showed a normal longitudinal growth i

Prevalence of temporomandibular joint internal derangement in patients with craniomandibular disorders.

To determine the prevalence of temporomandibular joint internal derangement in patients with signs and symptoms of craniomandibular disorders, bilateral imaging was performed in a consecutive series of 115 patients with signs and symptoms of craniomandibular disorders. Ninety patients (78%) had different stages of unilateral or bilateral internal derangement, and 25 patients (22%) had normal temporomandibular joints bilaterally. Out of 230 joints, 60 showed disk displacement with reduction, 8 showed disk displacement without reduction, and 29 showed disk displacement without reduction associat

Temporomandibular joint sounds: correlation to joint structure in fresh autopsy specimens.

In an attempt to better understand the cause of different types of temporomandibular joint (TMJ) sounds, we recorded joint sounds from 27 fresh autopsy specimens, displayed the time frequency distribution of the sound as a three-dimensional graph, and correlated the sound character to morphologic observations at subsequent dissection. Eleven joints elicited sounds, and 16 joints were silent. All joints with sounds had different degrees of intraarticular changes. These ranged from disk displacement with reduction to displacement without reduction and arthrosis of the articular surfaces. Recipro
