The role of multimodality imaging in selection, response assessment, and follow-up of patients receiving 177Lutetium-PSMA-therapy.

Prostate cancer is the most commonly diagnosed cancer among men in 112 countries, accounting for approximately 15% of all cancer cases. Whilst the 5-year survival rate for localised disease exceeds 90%, there is a significant drop to 50% if metastases are present. Following the VISION and TheraP trials, 177Lu-PSMA-therapy was approved for treatment of metastatic castrate resistant prostate cancer by the FDA and EMA 2022. Patient selection for 177Lu-PSMA-therapy is now relatively well defined, guided by PSMA-PET/CT criteria established in pivotal trials. Nevertheless, clinical consensus on appr

The value of non-enhanced magnetic resonance imaging based on T2-weighted imaging and apparent diffusion coefficient in the study of three-tiered HER2 status in patients with breast cancer.

BACKGROUND: Preoperative assessment of human epidermal growth factor receptor 2 (HER2) status relies on invasive biopsy, which is subject to sampling limitations. Meanwhile, conventional contrast-enhanced magnetic resonance imaging (MRI) and radiomics face barriers to widespread adoption due to high costs, complexity, and poor reproducibility. In contrast, non-contrast MRI based on T2-weighted imaging (T2WI) and diffusion-weighted imaging (DWI)/apparent diffusion coefficient (ADC) offers a non-invasive, cost-effective, and safe alternative. This study aimed to evaluate whether non-contrast T2W

Correlation-Induced Accessibility Bridges in Biomedical Networks: A Proof-of-Concept Relational Graph Model.

Complex diseases often involve distributed interactions among biological regions, physiological systems, imaging phenotypes, and clinical variables that are not fully captured by anatomical proximity, isolated biomarkers, or conventional feature-based representations. In oncology, neuroimaging, critical care, and systems medicine, distant or apparently separate biomedical sectors may show strong statistical or functional coupling associated with multimodal imaging signatures, inflammatory responses, metabolic constraints, treatment-induced changes, or shared disease-state organization. In this

A conceptual blueprint for "turning cold to hot" in Osteosarcoma: from TME stratification hypotheses to adaptive therapeutic prospects.

Osteosarcoma (OS) is a quintessential “cold tumor,” and outcomes for patients with metastatic or recurrent disease have remained poor for decades. The failure of immune checkpoint inhibitors (ICIs) in OS reflects a multilayered immunosuppressive architecture rather than a single dominant lesion. This review deconstructs three principal barriers within that architecture: (1) physical T-cell exclusion driven by a dense fibrotic stroma and aberrant vasculature; (2) a myeloid-dominant suppressive network enriched for Tumor-Associated Macrophages (TAMs) and Myeloid-Derived Suppressor Cells (MDSCs);

Performance of machine learning algorithms in diffusion tensor imaging of movement disorders: an exploratory meta-analysis.

BACKGROUND: Machine learning (ML) applied to diffusion tensor imaging (DTI) has emerged as a promising tool for detecting microstructural brain alterations in movement disorders. However, existing studies vary widely in design, sample size, imaging pipelines, and analytic rigor, resulting in high methodological heterogeneity that limits quantitative comparability. OBJECTIVES: This exploratory meta-analysis and narrative synthesis aimed to characterize performance trends, methodological diversity, and sources of variability among ML models trained on DTI data for classifying movement disorders

3D Segmentation of Whole Lung and Metastatic Lung Nodules Using Adaptive Region Growing and Shape-based Morphology.

OBJECTIVE: Early diagnosis of primary and metastatic lung nodules is critical for effective therapeutic planning. Manual delineation of lung nodules is not time-efficient and is prone to human error as well as interobserver and intraobserver variability. This study aimed to address the unmet need for an open-source computer-aided detection (CAD) system for 3D segmentation of lung and metastatic lung nodules along with radiomic feature extraction. METHODS: The proposed adaptive region-growing-based lung nodule segmentation (RGLNS) tool was developed in-house, requiring only manual input to sel

Anal squamous cell carcinoma: Impact of radiochemotherapy evolution over years and an explorative analysis of MRI prediction of tumor response in a mono-institutional series of 131 patients.

INTRODUCTION: Radiochemotherapy (RCHT) for the treatment of anal squamous cell carcinoma (ASCC) has evolved dramatically, also thanks to intensity-modulated RT (IMRT) and 3D image guidance (3D IGRT). Despite most patients presenting fair outcomes, unmet needs still exist. Predictors of poor tumor response are lacking; acute toxicity remains challenging; and local relapse remains the main pattern of failure. PATIENTS AND METHODS: Between 2010 and 2020, ASCC stages I-III treated with 3D conformal radiotherapy or IMRT and CDDP-5FU or Mytomicine-5FU CHT were identified. Image guidance accepted in

Artificial intelligence advances in cystoscopy and imaging for bladder cancer: a narrative review.

Bladder cancer (BCa) diagnosis relies heavily on cystoscopy and imaging. Both have limited sensitivity and accuracy, particularly for muscle-invasive disease. Artificial intelligence (AI) has emerged as a promising tool for improving detection, grading, and staging by extracting imaging features that exceed human perception. We conducted a narrative review of peer-reviewed, English-language studies published between 2015 and 2025. We identified 75 articles and synthesized data from 35 key studies retrieved via PubMed, Google Scholar, Scopus, and Embase. Data were synthesized narratively, empha

Narrative Review: Predictive Biomarkers of Tumor Response to Neoadjuvant Radiotherapy or Total Neoadjuvant Therapy of Locally Advanced Rectal Cancer Patients.

Background/Objectives: Treatment of locally advanced rectal cancer (LARC) very often requires a neoadjuvant multimodal approach. Neoadjuvant treatment (NAT) encompasses treatments like chemoradiotherapy (CRT), short-course radiotherapy (SCRT), radiotherapy (RT) or a combination of either of these two with additional induction or consolidation chemotherapy, namely total neoadjuvant treatment (TNT). In case of complete radiological and clinical response, the non-operative watch-and-wait strategy can be adopted in selected patients. This strategy is impacted by a regrowth rate of approximately 30

Clinical Utility of Dual-Energy CT for Detection, Characterization, and Staging of Lung Tumors: A Rapid Review.

Background/Objectives: Lung cancer remains one of the leading causes of cancer-related mortality worldwide. Conventional computed tomography (CT) is the preferred imaging modality for evaluating pulmonary nodules because of its high spatial resolution; however, it primarily provides morphological information, including lesion size, shape, and density. Dual-energy CT (DECT), a more recent imaging technique, uses two different energy levels to enable material decomposition and quantitative parameter assessment. These parameters may provide additional information regarding tumor perfusion, vascul
