Management of cerebrospinal fluid leak associated with craniomaxillofacial trauma.

PURPOSE: The management of persistent, post-traumatic cerebrospinal fluid (CSF) rhinorrhea and otorrhea remains a surgical challenge. Repair of CSF leaks has evolved from that of an intracranial approach to one that is primarily extracranial and endoscopic. The purpose of this retrospective analysis is to determine the incidence of persistent CSF rhinorrhea and otorrhea and assess the clinical outcomes of patients presenting to a level 1 trauma center with posttraumatic CSF leaks who were managed by both surgical and nonsurgical means. PATIENTS AND METHODS: The records of all patients with ba

Reducing the risk for distal forearm fracture: preserve bone mass, slow down, and don't fall!

A case-control study of 1,150 female and male distal forearm cases and 2,331 controls of age 45 years and older was undertaken from 1996-2001 in five Northern California Kaiser Permanente Medical Centers. Most information on possible risk factors was obtained by an interviewer-administered questionnaire, supplemented by a few tests of lower extremity neurological function. Previous fractures since 45 years of age, a rough marker of osteoporosis, were associated with an increased risk (adjusted odds ratio [OR] [95% confidence interval] = 1.48 [1.20-1.84 ] per previous fracture). Several factors

The aetiology of dento-alveolar injuries and factors influencing attendance for emergency care of adolescents in the north west of England.

The aims of the study were to determine the main aetiological factors involved in injuries to anterior teeth and to identify factors influencing the attendance for emergency care. This information is essential, first, for planning dental health education programmes aimed at reducing the incidence of dental trauma and, second, to devise strategies for its effective treatment. Of 2022 schoolchildren examined for evidence of dental trauma, 696 (34%) had experienced injury and were interviewed regarding this injury. Of these, 403 knew about their damaged front tooth, but only 330 recollected the i

Erect radiographs to assess clinical instability in patients with blunt cervical spine trauma.

BACKGROUND: Computed tomography (CT) and magnetic resonance imaging (MRI) are sensitive modalities for the assessment of the spine, but certain injuries remain poorly assessed with supine radiographs. We describe four cases in which cervical spine injuries were proven as unstable with erect radiographs after being previously evaluated with supine radiographs and CT scans. METHODS: A retrospective review of medical records and images was used to identify four patients who presented to a teaching hospital from April to December 2010 with unstable cervical spine injuries that were only demonstra

Inaccurate trauma history due to fear of health care personnel involving law enforcement in children of noncitizen immigrants.

Two cases illustrating intentional withholding of information by noncitizen parents of children with major trauma are described. The difficulties and the results of not obtaining an accurate history in the initial assessment of accidental trauma victims are discussed. The core problem in these cases seems to be a fear of health care providers and a false perception that emergency medical services and emergency department personnel are extensions of law enforcement. These cases illustrate potential issues in managing the children of noncitizen immigrants.
