Evolution of Movement Disorders in Patients With CLN2-Batten Disease Treated With Enzyme Replacement Therapy.

OBJECTIVES: Neuronal ceroid lipofuscinosis type 2 (CLN2-disease) is an inherited childhood-onset neurodegenerative condition, with classical … double-scored, video-recorded examination using the Unified Batten Disease Rating Scale (UBDRS) and Abnormal Involuntary Movement Scale. RESULTS: Noncanonical movement disorders are common: while ataxia (89%) and myoclonus (83%) were near-universal, spasticity and dystonia were experienced by over half (61% each), with children having a median of 4 distinct movement disorder phenot …

Importance: Tardive dyskinesia (TD) is a persistent, potentially disabling, medication-induced movement disorder that has been underrecogniz … and via item response theory. Observations: In KINECT trials, TDIS followed a similar trajectory to Abnormal Involuntary Movement Scale. An MCID of 4 points in TDIS was considered clinically meaningful. Psychogenic non-epileptic seizures treated with guided transcranial direct current stimulation: A case report.

INTRODUCTION: Psychogenic non-epileptic seizures (PNES) are paroxysms of either altered subjective or objective manifestations that may mimic epileptic seizures (ES), without abnormal neuronal epileptiform activity. In this report, we present the case of a 39-year-old woman with PNES and functional movement disorders, who was successfully treated with neuro-guided transcranial direct current stimulation (tDCS). METHODS: We used a PET-guided tDCS approach, as a hypometabolism of the frontal region was revealed by FDG TEP scan. TDCs was performed 5 days/week, 2 times/day, during 3 weeks. All cl

Treatment of Hemichoreoathetosis with Arrhythmic Proximal Tremor after Stroke: The Role of Zona Incerta as a Target for Deep Brain Stimulation.

Deep brain stimulation (DBS) of the zona incerta has shown promising results in the reduction of medically refractory movement disorders. However, evidence supporting its efficacy in movement disorders secondary to hemorrhagic stroke or hemichoreoathetosis is limited. We describe a 48-year-old man who developed progressive hemichoreoathetosis with an arrhythmic, proximal tremor in his right arm following a thalamic hemorrhagic stroke. Pharmacological treatment was carried out with no change in the Abnormal Involuntary Movement Scale (AIMS) score after 4 weeks (14). After six sessions of botuli…

Evaluation of the quality of life in patients with segmental dystonia.

BACKGROUND/AIM: Segmental dystonia is an abnormal movement, characterized by involuntary, sustained and repetitive muscular contractions, causing twisting and abnormal posturing of two or more adjacent body parts. It is not a life-reducing condition, but it deteriorates physical, mental and social functioning. The aim of the study was to define the basic demographic and clinical characteristics of patients with segmental dystonia and to estimate their quality of life. METHODS: The study included patients treated at the Clinic for Neurology--Clinical Center of Serbia (Department for Involuntar

Clinical review of clozapine treatment in a state hospital.

Medical records of the first 37 patients to begin clozapine treatment at a state hospital in Oregon were reviewed for six months before clozapine treatment and six months after. Patients had a long history of schizophrenia and had responded poorly to antipsychotic medication. Clozapine treatment was generally well tolerated, although the rate of seizures (8 percent) was slightly higher than expected. Psychotic symptoms decreased as measured by the Brief Psychiatric Rating Scale, as did symptoms of tardive dyskinesia as measured by the Abnormal Involuntary Movement Scale. Thirty-four patients r…

[Tardive dyskinesia induced by classical antipsychotic drugs: a Tunisian sample of schizophrenics].

INTRODUCTION: The term tardive dyskinesia (TD) is used to describe abnormal movement, primarily associated with typical antipsychotic drugs, which are used to treat psychotic states such as schizophrenia. TD is characterised by repetitive involuntary purposeless muscle contractions that force parts of the body into abnormal, and sometimes painful, movements or postures. These movements are involuntary and are difficult or impossible to control. TD usually begins with the face, mouth, lips and tongue, and includes grimacing, lip-smacking, tongue movements and rapid blinking. It may also involve

Movement disorders in patients treated with long-acting injectable antipsychotic drugs.

The frequencies of antipsychotic drug-induced movement disorders documented by physicians and identified by researchers using standardized clinical rating scales were compared, as were the frequencies associated with use of fluphenazine decanoate and haloperidol decanoate. All outpatients at a Veterans Affairs medical center who were being maintained with intramuscular injections of fluphenazine decanoate or haloperidol decanoate and were in otherwise good health were studied. The patients were assessed by a pharmacist using the Abnormal Involuntary Movement Scale to evaluate the presence and …

The prevalence of tardive dystonia, tardive dyskinesia, parkinsonism and akathisia The Curaçao Extrapyramidal Syndromes Study: I.

A prevalence study of extrapyramidal syndromes was conducted among all psychiatric inpatients of the Netherlands Antilles (n = 194; mean age 53.1). The Netherlands Antilles are very suitable for epidemiological research as it is a well-defined catchment area with only one psychiatric hospital and a health care system based on western principles. In this mainly chronic population, the prevalence was measured of tardive dystonia, tardive dyskinesia, parkinsonism and akathisia using respectively the Fahn-Marsden rating scale, the Abnormal Involuntary Movement Scale, the Unified Parkinson Disease …

Chorea induced by non-ketotic hyperglycaemia: a case report.

We describe an 81-year-old woman presenting with sudden onset of generalised chorea. She was unaware of suffering from diabetes. Laboratory screening revealed non-ketotic hyperglycaemia. Brain magnetic resonance imaging (MRI) failed to show basal ganglia abnormalities. Monotherapy with subcutaneous regular insulin induced a progressive normalisation of glycaemia as well as a parallel improvement of the abnormal involuntary movement scale on a nine-day sequential observation. This correlation strongly supports the hypothesis that non-ketotic hyperglycaemia itself might play a major pathogenetic…

Decreased serum brain-derived neurotrophic factor levels in schizophrenic patients with tardive dyskinesia.

The pathogenesis of tardive dyskinesia (TD) may involve neurodegeneration and associated dysfunction of brain-derived neurotrophic factor (BDNF) for the survival and maintenance of function in neurons. We therefore compared serum BDNF levels in schizophrenic patients with (n=129) and without TD (n=235), and normal controls (n=323). Assessments included the abnormal involuntary movement scale (AIMS) and the positive and negative syndrome scale (PANSS). Our results were that patients with TD had lower serum BDNF levels than those without TD and normals. Lower serum BDNF levels were correlated wi…
