Impact of the "July effect" in head and neck microvascular reconstruction: A retrospective review.

OBJECTIVE: The "July effect," a theory that the beginning of the academic year has worse operative outcomes and complication rates, remains controversial. We evaluated the "July effect" as a risk factor for negative operative outcomes in head and neck microvascular reconstruction. METHODS: Multi-institutional retrospective review at three academic tertiary care centers from January 2010 to August 2021. Free flaps were stratified by the academic quarter. Patient factors, operative variables, length of stay (LOS), flap failures, and postoperative complications and adverse events were compared b

Gender, Self-Perception, and Academic Problems in High School.

Given the increasing importance of education to socioeconomic attainment and other life course trajectories, early academic struggles can have long-term consequences if not addressed. Analysis of a nationally representative sample with official school transcripts and extensive data on adolescent functioning identified a social psychological pathway in this linkage between external feedback about early struggles and truncated educational trajectories. For girls, class failures absent of diagnosed learning disabilities engendered increasingly negative self-perceptions that, in turn, disrupted ma

Academic Procrastination in University Students: Conscientiousness and Domain-Specific Executive Function Associations.

Academic procrastination is a common and persistent difficulty in higher education and is widely conceptualized as a self-regulatory failure. Although conscientiousness is a robust predictor of procrastination, less is known about whether specific executive function (EF) components contribute to procrastinatory behavior beyond personality traits. The present study examined relationships among domain-specific EF, conscientiousness, academic procrastination, and academic performance indicators in 117 university students. Participants completed performance-based measures of EF (inhibition, cognit

Commentary: "Who was caring for Mary?" revisited: a call for all academic physicians caring for patients to focus on systems and quality improvement.

Over 15 years have passed since Mary's near death (Annals of Internal Medicine. 1993;118:146-148). Disappointment in the care by fellow academic physicians persists; however, a reanalysis of her case through the lens of complex systems design and performance yields a more accurate and actionable perspective. Mary's suffering was not due to human failure alone. Human failure was provoked and exacerbated by broken processes including ambiguous assignments of responsibility; inadequate transfers of information and authority; unreliable or unavailable protocols for providing safe, effective treatm

Acuity patterns of heart failure among emergency departments in a large health system.

BACKGROUND: The prognostic importance of Emergency Heart Failure Mortality Risk Grade (EHMRG) score in assessing short term mortality in Congestive Heart Failure (CHF) patients has been validated in the past, however, few studies have examined acuity patterns in the CHF population across healthcare settings. We aim to understand acuity patterns of CHF patients across a large health system for better resource utilization. METHODS: Retrospective chart review of adult patients with acute CHF in a large Metropolitan health system was performed in 3 community and 3 academic hospitals between Janua

EHR-Based Quality Interventions Associated With Differential GDMT Prescribing in Community Versus Academic Hospitals.

BACKGROUND: Institutional barriers are associated with suboptimal guideline-directed medical therapy (GDMT) prescribing at heart failure discharge. PROJECT RATIONALE: Practice variations between academic medical centers (defined as "teaching" hospitals with residents) and community hospitals are poorly understood. Evaluating how diverse settings respond to quality improvement strategies is vital. PROJECT SUMMARY: A multicenter study across 10 electronic health record-unified hospitals evaluated 3,535 heart failure encounters. Phase 1 implemented educational work groups and financial incentiv

Locus of control and learning disabilities: a review and discussion.

It has been widely reported that an external locus of control is associated with children who experience failure. A review of the relevant literature indicates that learning disabled children, like other groups of children who have experienced failure, are more likely to exhibit an external locus of control than their normally achieving peers. In particular, learning disabled children have been found to be more likely than normally achieving students to attribute their successes, but not their failures, to external factors. The relationship of the locus of control construct to the field of lea

Malaysian adolescent students' needs for enhancing thinking skills, counteracting risk factors and demonstrating academic resilience.

The adolescence period of life comes along with changes and challenges in terms of physical and cognitive development. In this hectic period, many adolescents may suffer more from various risk factors such as low socioeconomic status, substance abuse, sexual abuse and teenage pregnancy. Findings indicate that such disadvantaged backgrounds of Malaysian adolescent students lead to failure or underachievement in their academic performance. This narrative review scrutinises how some of these students are able to demonstrate academic resilience, which is satisfactory performance in cognitive or ac

When can we identify the students at risk of failure in the national medical licensure examination in Japan using the predictive pass rate?

CONTEXT: Failure of students to pass the National Medical Licensure Examination (NMLE) is a major problem for universities and the health system in Japan. To assist students at risk for NMLE failure as early as possible after admission, this study investigated the time points (from the time of admission to graduation) at which predictive pass rate (PPR) can be used to identify students at risk of failing the NMLE. METHODS: Seven consecutive cohorts of medical students between 2012 and 2018 (n = 637) at the Gifu University Graduate School of Medicine were investigated. Using 7 variables before

Addressing Academic Concerns: A Survey of Primary Care Providers.

Given new clinical recommendations, we sought to better understand the perspectives and practices of primary care providers (PCPs) around pediatric academic concerns. Practicing PCPs were recruited to complete an 8-item self-report survey; 163 PCPs were included in the final analysis. Results indicated that 84% of PCPs perceived their role in addressing patients' academic concerns as highly important; yet there was variability in the frequency with which PCPs reported engaging in activities around these educational needs. There was a significant positive relationship between perception of role

Deficits in Pre- and Post-transplant Functional, Motor, and Cognitive Deficits Associated With Graft Failure and Mortality in Pediatric Heart Transplant Recipients.

BACKGROUND: Pretransplant functional, motor, cognitive, and academic deficits are common in pediatric patients requiring heart transplantation (HT); some persist post-HT. We assessed the association between these quality of life (QoL) deficits and post-HT outcomes. METHODS: Using SRTR data 2008-2023, we evaluated the functional, motor, cognitive, and academic status of pediatric HT recipients from listing to 15 years post-HT. We compared all-cause graft survival among patients with vs. without pre-HT deficits using Cox regressions. Among patients with a functioning graft at 1 year, we assesse

Positivity bias in academic future expectations: Evidence from production and recall tasks in first-year university students.

Individuals tend to process positive information more preferentially than negative information, a phenomenon known as positivity bias. This study examined the presence of this bias in academic expectations among first-year university students. Participants generated academic expectations in response to positive (e.g., achieve, collaborate) and negative (e.g., quit, lose) cues and rated the emotional intensity of these expectations. After completing several distractor tasks, they were asked to recall the generated expectations freely, and finally completed measures of academic satisfaction, sel
