[Confocal microscopy in the diagnosis of acanthamoeba keratitis].

UNLABELLED: Acanthamoeba keratitis (AK) is a dangerous disease of the cornea, its prevalence has increased significantly due to widespread usage of contact lenses. The similarity of the clinical manifestations of AK to other infectious keratitis (especially herpetic keratitis) requires introduction of a rapid diagnosis method into clinical practice. PURPOSE: To evaluate the capabilities of corneal confocal microscopy (CCM) in the diagnosis of acanthamoeba keratitis. MATERIAL AND METHODS: We examined 33 patients (35 eyes) with suspected acanthamoeba keratitis using light and laser confocal mi…

Acanthamoeba keratitis--importance of the early diagnosis.

This is a case report of Acanthamoeba as a causative agent of keratitis at the Department of Ophthalmology, University Hospital Rijeka. Delay in treatment led to an advanced stage of the disease with multiple complications. Initially, presented symptoms were similar to those of herpetic keratitis. In the mean time progression of the disease led to a cloudy cornea with a stromal ring infiltrate, poor vision, elevated intraocular pressure, a mature cataract and finally corneal melt. Corneal scrapings were obtained from a 28 years old female patient, wearing soft contact lenses and with poor lens

An expert's guide to Acanthamoeba keratitis pharmacology.

INTRODUCTION: Acanthamoeba keratitis (AK) is a rare but sight-threatening corneal infection, increasingly associated with contact lens wear, whose treatment remains constrained by the lack of agents that are both cysticidal and well tolerated by the cornea. This review appraises the current pharmacological landscape and the obstacles impeding therapeutic progress. AREAS COVERED: PubMed, Scopus, Google Scholar, and Web of Science were searched for articles published between January 2000 and June 2026 using combinations of the terms 'Acanthamoeba keratitis,' 'treatment,' 'pharmacology,' 'amoebi…

A report of 10 patients with Acanthamoeba keratitis.

CLINICAL CASES: Cases are presented of 10 patients with Acanthamoeba keratitis treated between 2008 and 2017. All were contact lens wearers. All of them received treatment with a biguanide combined with a diamidine. In 3 cases the infestation did not exceed the superficial stroma, responding to topical treatment. In 7, the infection reached the deep stroma, with 6 of these cases requiring penetrating keratoplasty (PKP), 3 of them therapeutic PKP because of perforation risk or ocular spreading. The visual acuity improved in all the cases. CONCLUSION: The infestation depth at the time of diagno…

Acanthamoeba as a cause of peripheral ulcerative keratitis.

PURPOSE: To report a case of peripheral ulcerative keratitis caused by Acanthamoeba. METHODS: Case report and review of the literature. A 37-year-old woman with a history of pain and redness of the right eye with no apparent predisposing factors, on examination, revealed a peripheral ulcerative keratitis. RESULT: Microbiological investigations of the corneal infiltrate revealed Acanthamoeba cysts. CONCLUSION: All cases of peripheral ulcerative keratitis should be subjected to routine microbiological evaluation including those for Acanthamoeba.

Clinical features and outcome of Acanthamoeba keratitis.

We retrospectively reviewed 11 patients with culture-proven Acanthamoeba keratitis who presented at the National Taiwan University Hospital between 1989 and 1996. We assessed predisposing factors, initial diagnosis, clinical presentation, treatment, and outcome. A history of contact lens-wear, poor contact lens hygiene, intractable eye pain, and ring infiltrates in the cornea were the most prominent characteristics and clinical manifestations. Acanthamoeba keratitis was often misdiagnosed, with herpetic keratitis (7/11) being the most common initial diagnosis from referring hospitals. These pa…

[Acanthamoeba keratitis in Iceland from 1996-2021].

INTRODUCTION: Acanthamoeba keratitis is a rare but serious eye infection primarily associated with contact lens use. Its diagnosis is often delayed due to treatment challenges, frequent co-infections, and the amoeba's high resistance, leading to significant visual impairment. This study assessed the incidence of Acanthamoeba keratitis in Iceland over 25 years, identified risk factors, diagnostic difficulties, and explored potential improvements. MATERIALS AND METHODS: A retrospective descriptive study including all individuals diagnosed with amoebic corneal infection in Iceland from 1996-2021…

Acanthamoeba Keratitis: Are Recent Cases More Severe?

PURPOSE: To identify a recent change in disease severity and visual outcomes of patients with Acanthamoeba keratitis (AK) at Wills Eye Hospital (WEH). METHODS: A retrospective chart review was performed of all cases diagnosed with AK at WEH between January 1, 2009, and December 31, 2015. Failure of treatment was defined as having a final best-corrected visual acuity worse than 20/100 and/or requiring keratoplasty. The patients were grouped into 2 outcome categories: successfully treated and those who failed treatment; and prognostic factors associated with failure status were evaluated. Addit…

[Acanthamoeba keratitis--a too seldom diagnosed disease?].

Acanthamoeba keratitis has become a difficult diagnostic and management problem in contact lens patients over the last few years. A young female contact lens wearer presented with a long history of painful central corneal ulcer. The suspected diagnosis of Acanthamoeba keratitis could only be confirmed by histology and electron microscopy following penetrating keratoplasty. Special stains of scrapings and culture of material on special media failed to establish the diagnosis. This rare (or underdiagnosed?) infection must be considered in the differential diagnosis of keratitis following contact…

Experimental modeling of Acanthamoeba keratitis in mice: Isolation and cultivation of Acanthamoeba castellanii.

Free-living Acanthamoeba can lead to granulomatous amoebic encephalitis or Acanthamoeba keratitis (AK), an eye infection. AK is most common among contact lens wearers, and causes include long-term lens use, contamination of the contact lens, and corneal trauma. In vitro studies of AK, especially the development of therapeutic drugs, need to be confirmed by in vivo experiments. We developed our AK mouse model using a variety of methods. A. castellanii (AK/AJ1; 1 × 106 cells) were loaded onto 2 mm contact lens pieces for insertion into the scratched eyes of mice under anesthesia, and the eyelids…

Effect of immunization with the mannose-induced Acanthamoeba protein and Acanthamoeba plasminogen activator in mitigating Acanthamoeba keratitis.

PURPOSE: The mannose-induced cytopathic protein (MIP-133) and Acanthamoeba plasminogen activator (aPA) play key roles in the pathogenesis of Acanthamoeba keratitis by inducing a cytopathic effect on the corneal epithelial and stromal cells and by production of proteolytic enzymes that facilitate the invasion of trophozoites through the basement membrane. The goal of the present study was to gain insight into the pathogenicity of Acanthamoeba infection as well as to determine whether oral immunization with aPA and MIP-133 produce an additive protection against Acanthamoeba keratitis. METHODS: …

Total Anterior Staphyloma Secondary to Acanthamoeba Keratitis.

There are very few published cases of total anterior staphyloma, all of which have been reported as secondary to fungal keratitis. This study reports the clinical and histopathological findings and subsequent management of a 27-year-old healthy female patient who developed total anterior staphyloma after poor compliance with treatment for clinically diagnosed acanthamoeba keratitis. She underwent a successful evisceration with good long-term results. This case highlights that total anterior staphyloma may also result from untreated keratitis which is not fungal in origin. In cases of fungal an…

Acanthamoeba keratitis: diagnosis and treatment update 2009.

PURPOSE: To describe the current management of Acanthamoeba keratitis (AK). DESIGN: A perspective based on the literature and author experience. RESULTS: Early diagnosis and appropriate therapy are key to a good prognosis. A provisional diagnosis of AK can be made using the clinical features and confocal microscopy, although a definitive diagnosis requires culture, histology, or identification of Acanthamoeba deoxyribonucleic acid by polymerase chain reaction. Routine use of tissue diagnosis is recommended, particularly for patients unresponsive to treatment for AK. Topical biguanides are th…

Acanthamoeba keratitis related to cosmetic contact lenses.

We report a rare case of Acanthamoeba keratitis related to cosmetic contact lenses in both eyes. A 17-year-old girl with a history of wearing cosmetic contact lenses presented with keratitis. She purchased cosmetic contact lenses via the Internet, and followed a contact lens care system irregularly, occasionally using tap water. Cell culture was performed on samples collected from a corneal scraping, the contact lenses and the storage cases. The isolated organism was Acanthamoeba. The patient was treated with polyhexamethylene biguanide and chlorhexidine for 3 months, and recovered with normal…

Acanthamoeba keratitis in non-contact lens wearers.

Nine cases of Acanthamoeba keratitis not associated with contact lens wear were diagnosed between July 1987 and August 1989. Patients were treated with topical neomycin-polymyxin B-bacitracin (Neosporin) drops alone or in combination with either miconazole nitrate or ketoconazole drops. At the time of data collection four patients were available for follow-up for an average of 4 months; however, four patients were unavailable for follow-up and one is still undergoing treatment. In four patients corneal infiltrates cleared completely with topical medication (Neosporin, two patients; Neosporin p…
