Grouped beating in eisenmenger: successful localization and ablation of an accelerated idioventricular rhythm from within the purkinje system.

A 33-year old female with a background of Eisenmenger syndrome secondary to multiple congenital muscular ventricular septal defects (VSD) was admitted with a recent history of frequent intermittent palpitations. It was noted that she had an independent accelerated idioventricular rhythm (AIVR), with rates varying between 85-110bpm, which exhibited a repetitive grouped beating pattern. Although generally perceived as benign, in this case this rhythm was drug refractory, was associated with significant compromise to cardiac filling and output and progressed to haemodynamically intolerable sustai

Third degree atrioventricular block and accelerated idioventricular rhythm associated with a heart base chemodectoma in a syncopal Rottweiler.

A 7-year-old male intact Rottweiler was presented with a 1-week history of lethargy, anorexia, vomiting and multiple syncopal events. The results of the clinical examination and electrocardiography were consistent with a third degree atrioventricular block and an intermittent accelerated idioventricular rhythm. Haematology, serum biochemistry, serology for Borrelia burgdorferi, blood culture, total T4, thoracic radiography and echocardiography did not reveal the cause of the arrhythmia. Response to medical treatment with isoproterenol was poor. Pacemaker placement was declined by the owners an

Accelerated idioventricular rhythm resulting in torsades de pointes and cardiac arrest in a child: successfully cryoablated in left'coronary cusp.

Known as a benign arrhythmia and normally requiring no specific treatment, accelerated idioventricular rhythm can rarely degenerate to a life-threatening arrhythmia. Here, we present a child with left coronary cusp-originating accelerated idioventricular rhythm, degenerating into torsades de pointes and resulting in cardiac arrest, which was ablated with a cryocatheter. An 11-year-old boy, followed due to asymptomatic accelerated idioventricular rhythm before, was referred to our department because he had experienced an aborted cardiac arrest during sleep. He had been resuscitated for 5 minute

Prevalence and significance of accelerated idioventricular rhythm in patients with ST-elevation myocardial infarction treated with primary percutaneous coronary intervention.

Accelerated idioventricular rhythm (AIVR) has been considered a marker of successful reperfusion in fibrinolytic-treated patients. Evidence is limited regarding its significance in patients with ST-elevation myocardial infarction treated with primary percutaneous coronary intervention (PPCI). The purpose of the present study was to determine the prevalence and associated outcomes of arrhythmias and conduction disturbances occurring during PPCI. In 503 patients with ST-elevation myocardial infarction, the arrhythmias and conduction disturbances occurring from arrival at the catheterization labo

Pilsicainide Intoxication Presenting With Alternating Accelerated Idioventricular and Junctional Escape Rhythms During Acute Kidney Injury: A Case Report.

Pilsicainide is a class Ic sodium channel blocker that is eliminated predominantly by the kidneys and may accumulate when renal function deteriorates. An 85-year-old man receiving pilsicainide 150 mg/day developed acute kidney injury (AKI) during hospitalization for hemorrhagic colitis. He subsequently developed impaired consciousness, dyspnea, pulmonary congestion, and alternating episodes of junctional escape rhythm at 30-37 beats/min and a regular, wide QRS rhythm at 79 beats/min. The QRS duration was approximately 150 ms, and the serum pilsicainide concentration was 3.19 µg/mL. The wide QR

Delayed occurrence of an accelerated idioventricular rhythm with alternating bundle branch block after myocardial infarction as predictor of sudden cardiac arrest: a case report.

BACKGROUND: Accelerated idioventricular rhythm (AIVR) is known as reperfusion arrhythmia in the setting of acute myocardial infarction (AMI). In healthy individuals, it is usually considered to be benign. Alternating bundle branch block (ABBB) often progresses to complete atrioventricular block requiring permanent pacemaker implantation. We report a case of delayed appearance of AIVR following myocardial infarction (MI) in combination with ABBB as precursor of sudden cardiac arrest due to ventricular fibrillation (VF). CASE SUMMARY: A 62-year-old male with pre-existing left bundle branch bloc

Double ectopic accelerated ventricular and nonaccelerated ventricular or supraventricular rhythms.

Three patients with acute inferior wall myocardial infarction has accelerated idioventricular rhythms alternating and (or) coexisting with: (a) AV junctional (or low atrial) rhythm (Case 2); (b) paroxysmal ventricular tachycardia and another accelerated idioventricular rhythm (Case 3); and (c) AV junctional rhythm, paroxysmal ventricular tachycardia and accelerated idioventricular rhythm from another center (Case 4). The tracings from a fourth patient (Case 1) served to focus attention on one of the most pertinent features of accelerated idioventricular rhythms, namely the gradual decrease in

Accelerated ventricular rhythm in the neonatal period: a review and two new cases in asymptomatic infants with an apparently normal heart.

UNLABELLED: Accelerated ventricular rhythm (AVR) was observed in two newborn infants. In the first case, arrhythmia was noted during the foetal period. Both neonates were asymptomatic and had no evidence of cardiac disease. The arrhythmia eventually disappeared when the infants were 4 mo and 24 d old, respectively. AVR in the neonatal period is reviewed in this report and recent information regarding appropriate diagnostic evaluation, differentiation from ventricular tachycardia and treatment is outlined. CONCLUSION: Accelerated ventricular rhythm is a benign and self-limited arrhythmia in th

Premature beats originating from the atrioventricular accessory pathway.

This article reports a case of a 34-year-old male patient with ventricular pre-excitation (WPW). Under sinus rhythm, the patient presented with alternating fully manifest WPW complexes and normal cardiac beats. In addition, premature beats unrelated to P waves were observed, with morphology consistent with the fully manifest WPW complexes. Such premature beats are highly susceptible to misdiagnosis as accelerated idioventricular rhythm or accelerated junctional rhythm with left bundle branch block (LBBB).

Flecainide Toxicity From Clinical Pharmacology Perspectives.

We report a case comparing the measured half-life of flecainide with the half-life stated on the label. An 84-year-old woman presented with symptoms of anorexia and exertional dyspnea. She had undergone mitral and aortic valve replacements and excision of the membranous septum in the atrium for mitral and aortic stenosis and cor triatriatum. She was regularly administered 100 mg/day flecainide for paroxysmal atrial fibrillation. A previous electrocardiogram (ECG) showed a regular sinus rhythm. However, upon admission, the ECG revealed a heart rate of 94 bpm and an accelerated idioventricular r

Presumed primary cardiac mast cell tumor, pericardial effusion, and arrhythmia in a dog.

A 13-year-old female spayed border collie cross presented for pericardial effusion, arrhythmia, and a suspected cardiac mass. Echocardiogram revealed severe thickening and hypokinesis of the interventricular septum with a heterogenous, cavitated myocardium, concerning for neoplasia. Electrocardiogram revealed predominantly accelerated idioventricular rhythm with frequent periods of nonsustained ventricular tachycardia. Occasional prolonged PR intervals terminating in an aberrantly conducted QRS complex were present. These beats were postulated to represent either first-degree atrioventricular

Ventricular arrhythmias in children with attention deficit disorder--a symptom of autonomic imbalance?

OBJECTIVES: Potential side effects of stimulants for attention deficit disorder are in the focus of scientific discussions, intensified by the higher number of prescriptions. Children with known arrhythmias or other severe cardiac problems should not receive stimulants because of their sympathomimetic effects. METHODS: This is a retrospective analysis of 24-hour Holter electrocardiograms from 100 consecutive children with attention deficit disorder from January, 2006 to April, 2012. RESULTS: In all, nine children had significant ventricular arrhythmia (mean age 11.4 ± 3.1 years, 77% male, 77

The clinical impact of untreated slow ventricular tachycardia in patients carrying implantable cardiac defibrillators.

INTRODUCTION: The clinical impact of slow ventricular tachycardia (VT), occurring in patients carrying implantable cardiac defibrillators (ICD), is still under debate. METHODS AND RESULTS: From the UMBRELLA registry (multicenter, observational, and prospective study on patients with ICD), 659 episodes of slow VT were observed in 97 patients. Untreated slow VT (n = 93) had longer duration (23.7 min, CI95%: 10-39), compared with episodes treated effectively by anti-tachycardia pacing (ATP; n = 527; 0.32 min, IC95%: 0.22-0, 48) or shock (n = 39; 1 min, CI95%: 0.8-1.2). Despite of longer duration

Monomorphic repetitive rhythms originating from the outflow tract in patients with minor forms of right ventricular cardiomyopathy.

We studied in detail 17 patients presenting with monomorphic repetitive ventricular rhythms having left bundle branch block morphology and right axis deviation. All had an apparently normal heart at physical examination. At chest radiography, three patients had mild cardiomegaly, and at electrocardiography, five patients had inverted T waves beyond V2. Five patients had syncope or near syncope. In seven patients the tachycardia occurred on effort. One patient died suddenly. The patients were extensively investigated, using cross-sectional echocardiography, complete haemodynamic and angiographi

Study of pathogenesis of ventricular arrhythmia in experimental rats by separation of sinus and ventricular substitutional rhythms.

Pathogenesis of cardiac arrhythmia was determined by EEC changes after ATP-induced complete atrioventricular block. The re-entry mechanism underlays extrasystoles with equal coupling intervals with complexes of ventricular substitution rhythms, which transformed into paroxysmal tachycardia with equal R-R intervals, ventricular flutter, and ventricular fibrillation. Ectopic automaticity was characterized by extrasystole unrelated to the complexes of substitutional rhythms, which was transformed into accelerated idioventricular rhythm and asystole. During trigger activity, the extrasystoles were

[Ventricular tachycardia. A possible complication of intravenous atropine in the coronary patient].

Ventricular tachycardia occurred with chest pain in a 64 year old man with coronary artery disease after an intravenous injection of atropine. The particular feature of this case as compared to the other 8 reported cases is the restoration of sinus rhythm after a passage of accelerated idioventricular rhythm by the administration of oxygen and nitroglycerin. The increased oxygen consumption and myocardial ischaemia due to the tachycardia seem to be the factors responsible for these ventricular arrhythmias. Such cases, though rare, incite caution in the administration of atropine to patients wi
