Detecting Parkinson's Disease from Wrist-Worn Accelerometry in the U.K. Biobank.

Parkinson's disease (PD) is a chronic movement disorder that produces a variety of characteristic movement abnormalities. The ubiquity of wrist-worn accelerometry suggests a possible sensor modality for early detection of PD symptoms and subsequent tracking of PD symptom severity. As an initial proof of concept for this technological approach, we analyzed the U.K. Biobank data set, consisting of one week of wrist-worn accelerometry from a population with a PD primary diagnosis and an age-matched healthy control population. Measures of movement dispersion were extracted from automatically segme

The Neurological Examination Improves Cranial Accelerometry Large Vessel Occlusion Prediction Accuracy.

BACKGROUND/OBJECTIVE: We combined cranial accelerometry, a device-based approach to large vessel occlusion (LVO) prediction, with neurological examination findings to determine if this improves diagnostic accuracy compared to either alone. METHODS: Cranial accelerometry recordings and NIHSS scores were obtained during stroke codes and thrombectomy transfers at an academic medical center using convenience sampling. The reference standard was discharge diagnosis of LVO stroke. We compared accuracy statistics between machine learning models trained using cranial accelerometry alone, with asymmet

Integrating GPS and Accelerometry to Capture Life-Space Mobility in Parkinson's Disease.

Mobility is key to independent living, and life-space mobility (LSM) describes the extent to which a person moves within and beyond their home. Restricted LSM may limit participation in life. Parkinson's disease (PD) can influence LSM through changes in physical function and social engagement. This study investigated LSM in people with mild-to-moderate PD using self-reported and objective measures, examining agreement between measures and relationships with physical activity and quality of life. The participants completed the Life-Space Assessment (LSA) and wore an accelerometer and GPS monito

Heart Rate Supports Accelerometry as a Reliable Predictor of Energy Expenditure in Bats.

Measuring the energetic cost of behavior is essential for understanding decision-making in wild animals. This is particularly true for bats where life near their energetic ceiling has resulted in a diversity of behaviors to compensate for social and environmental unpredictability. While GPS and tri-axial accelerometry has improved movement-based estimates of energy expenditure in the field, complex flight kinematics in bats may violate key assumptions accelerometry-based estimates rely on. To test whether energy expenditure is predicted by acceleration-based movement in bats, we deployed heart

Description of physical activity in children with juvenile idiopathic arthritis using accelerometry.

OBJECTIVES: Juvenile idiopathic arthritis (JIA) can lead to functional impotence and reduced physical activity (PA) in patients. The objective of this study was to monitor the daily PA of children aged 6-18 years with JIA using accelerometry based on disease activity and individual abilities, and to compare this with age recommendations. METHODS: Patients volunteering for the study wore an accelerometer for 7 consecutive days; data were evaluated using a wGT3X ActiGraph™. The 2010 World Health Organization (WHO) recommendations were used for comparison. RESULTS: We collected accelerometry da

Evidence of Authorship on Messages in Facilitated Communication: A Case Report Using Accelerometry.

Facilitated communication (FC) belongs to augmentative and alternative methods of communication. Currently, FC is very rarely and unofficially used with people suffering from verbal/communicative disorders or neurodevelopmental disorders such as intellectual deficiency or autism spectrum disorder (ASD). FC consists of physical support exerted by a facilitator at the hand/wrist/forearm/elbow of a patient/participant, aimed at helping him/her to point at pictures/words, and sometimes to type letters/words on a keyboard. Given most of (but not all) validation studies using control procedures fail

Habitual Physical Activity in Patients with Myasthenia Gravis Assessed by Accelerometry and Questionnaire.

BACKGROUND: Physical activity (PA) in patients with myasthenia gravis (MG) is considered safe and beneficial, and an active lifestyle is required to obtain the health benefits of exercise. However, as the disease leads to physical impairments an insight into the overall PA habits in this patient population is relevant but lacking. OBJECTIVE: To measure habitual physical activity in a Danish cohort of patients with MG measured by accelerometer and questionnaire, and to determine relevant predictors for PA intensities. METHODS: Habitual physical activity was assessed by; 1) the accelerometer A

Use of Accelerometry as an Educational Tool for Spinal Cord Stimulation: A Pilot Study.

BACKGROUND: Spinal cord stimulation (SCS) is an important option for patients with chronic neuropathic pain. In the United States, a successful SCS trial determines eligibility for SCS implant. Metrics to determine success are often self-reported and subjective, which may limit achievement of patient goals. This study aimed to assess whether patients undergoing SCS implant after successful trial felt that use of external accelerometry prior to implant was a useful educational tool to objectively appraise function and achievement of treatment goals. METHODS: This was a single center, prospecti

Accelerometry-enabled measurement of walking performance with a robotic exoskeleton: a pilot study.

BACKGROUND: Clinical scores for evaluating walking skills with lower limb exoskeletons are often based on a single variable, such as distance walked or speed, even in cases where a host of features are measured. We investigated how to combine multiple features such that the resulting score has high discriminatory power, in particular with few patients. A new score is introduced that allows quantifying the walking ability of patients with spinal cord injury when using a powered exoskeleton. METHODS: Four spinal cord injury patients were trained to walk over ground with the ReWalk™ exoskeleton.

Correlation of accelerometry with clinical balance tests in older fallers and non-fallers.

BACKGROUND: falls are a common cause of injury and decreased functional independence in the older adult. Diagnosis and treatment of fallers require tools that accurately assess physiological parameters associated with balance. Validated clinical tools include the Berg Balance Scale (BBS) and the Timed Up and Go test (TUG); however, the BBS tends to be subjective in nature, while the TUG quantifies an individuals functional impairment but requires further subjective evaluation for balance assessment. Other quantitative alternatives to date require expensive, sophisticated equipment. Measurement

Quantitative assessment of parkinsonian and essential tremor: clinical application of triaxial accelerometry.

Triaxial accelerometry was used to study various motion characteristics in 10 patients with Parkinson disease and 11 patients with essential tremor. The smoothness of motion factor (SPMEF) and the acceleration waveform factor (TIPAR) were frequently and consistently abnormal. When both factors were analyzed together and compared with a control population, 100% of the left arm and 88% of the right arm values were abnormal in the Parkinson disease group, and 82% of the left arm and 91% of the right arm values were abnormal in the essential tremor patients. Repeat evaluation revealed consistent i

Pelvic floor tissue damping during running using an intra-vaginal accelerometry approach.

BACKGROUND: While cumulative loading of the pelvic floor during exercise appears to increase the risk of developing pelvic floor disorders, the pathophysiologic role of pelvic floor loading is poorly understood. The aim of this exploratory study was to present a method for evaluating vibrational frequency damping of the female pelvic floor and to investigate the potential utility of this approach in a preliminary evaluation. METHODS: Female participants were instrumented with an intravaginal accelerometer and a hip-mounted accelerometer, then ran on a treadmill at 7 km/h and 10 km/h both befo

Combined accelerometry and writing analysis for quantifying tremor during focused ultrasound thalamotomy.

OBJECTIVE: MRI-guided focused ultrasound (MRgFUS) thalamotomy is used to treat tremors. Identifying the correct area to ablate relies on subjective physician assessment of tremor improvement during treatment. To address this limitation, objective quantification of tremor severity during MRgFUS is needed. The aim of this study was to develop and evaluate an objective method for quantifying tremor progression during MRgFUS using an MRI-compatible accelerometer and tablet. METHODS: Forty patients (30 male, mean age 74.5 years) who underwent MRgFUS thalamotomy to treat tremor were included in thi

Loneliness and physical activity: Findings from an ecological momentary assessment and accelerometry study.

This study examined how loneliness relates to physical activity at between-person and within-person (daily and momentary) levels. Adults in committed relationships (n = 291; ages 40-70) completed a dispositional loneliness measure and participated in an 8-day ecological momentary assessment paired with accelerometry. Loneliness was assessed three times daily via self-report, with partners providing end-of-day ratings of participants' loneliness. Physical activity indices (e.g. active time) were derived for the full 8-day period, for each day, and for 20-60-minute windows before and after each
