Human error causes or contributes to considerably over half of all aviation mishaps. This report describes a 7-year study of aircraft incident data conducted in an attempt to further our understanding of the phenomenon of human error. The study of incidents as a surrogate for aircraft accidents is relevant only if incidents constitute a population or universe of which accidents are a subset. This assumption has been examined in a study of over 35,000 reports of aviation incidents collected from 1976-83 by the NASA Aviation Safety Reporting System. One-third of the reports involve conflicts amo

IntroductionAlthough often harmless, unexpected turbulence can occasionally cause injuries and even mortality for passengers on commercial airline flights. Studies of the mechanisms of injury associated with turbulence-related accidents can help airlines and regulatory agencies develop strategies to improve safety during encounters with turbulence.MethodsWe examined accident reports from the National Transportation Safety Board (NTSB), a federal agency tasked with investigating aviation accidents. Reports for accidents with at least one serious injury between January 1, 2008, and December 31,

INTRODUCTION: Until recently, glaucoma requiring treatment was disqualifying for U.S. pilots and required an aeromedical special issuance waiver. Since 2013 Aerospace Medical Examiners (AMEs) have been authorized to evaluate third-class pilots with mild glaucoma using similar protocols and issue medical certificates without a waiver if these criteria specified in the AME Guide are met.
METHODS: The FAA's medical database was searched for pilots with glaucoma between 2005 and 2014. The National Transportation Safety Board (NTSB) accident database was then searched to determine which pilots wit

The U.S. Department of Transportation is responsible for implementing new safety improvements and regulations with the goal of ensuring limited funds are distributed to where they can have the greatest impact on safety. In this work, we conduct a study of new regulations and other reactions (such as recalls) to fatal accidents in several different modes of transportation implemented from 2002 to 2009. We find that in the safest modes of commercial aviation and bus transport, the amount of spending on new regulations is high in relation to the number of fatalities compared to the regulatory att

Approximately 2-3% of all fatal air accidents may be attributed to suicide, and in many other accidents in aviation there are grounds for inferring that self-destructive or suicidal behavior was involved. Narcissistic personality traits are of paramount importance for the choice of this suicide method. Precursors include undetected depressions, alcoholism, and family problems of pilots.

During the period 1969-1975, 12 disqualified U.S. Army helicopter aviators with various finger amputations were evaluated by the U.S. Army Aeromedical Consultation Service for return to flying duties. The evaluations included both medical examinations and in-flight performance observed by aviation medicine specialists and standardization instructor pilots. The criteria for aeromedical disposition are presented with a discussion of the hand and finger requirements to fly U.S. Army UH-1 rotary wing aircraft. All evaluated aviators were presented to a board of senior U.S. Army flight surgeons, wh

Keratoconus causes progressive blurring and distortion of vision, which threatens the career of a military aviator. To assess the impact of keratoconus on flying careers, we reviewed the records of all aviators with keratoconus who have been examined at the United States Air Force School of Aerospace Medicine over the past 23 years. Of the 22 aviators observed for more than three years, 18 were still qualified to fly at their most recent examination. The remaining four aviators were permanently removed from flying duties because of visual dysfunction caused by keratoconus. Of the 22 aviators e

Aviation medicine has researched many important facts on pilots, but little on direct relationships between controllers, aviation medicine and air safety. The unsuspecting flying public accepts a 'blind faith' in aircraft and pilots, unaware that aircraft are controlled within 'suspect' ATC systems. The deceptive simplicity of controlling air traffic in apparently limitless skies belies the complexity of man-machine ATC systems operated in ever-crowded airspace, sometimes with antiquated equipment and indifferent communications. The indivisible operational controller/pilot team strives to meet

BACKGROUND: The routine occupational hazards of flying and parachute jumping place U.S. Army aviators at risk for sustaining high-energy traumatic injuries, such as thoracolumbar fractures.
METHODS: A longitudinal, prospective, epidemiologic database was used to determine the incidence, injury history, and aeromedical disposition of U.S. Army aviators who sustained thoracolumbar fractures for calendar years 1987 to 1997.
RESULTS: The overall incidence rate of thoracolumbar fracture was 12.8 per 100,000 aviators per year. Thirty aviators with thoracolumbar fractures were identified, and the a

INTRODUCTION: In 1997, in an effort to reduce the crash rate of scheduled commuter flights, the FAA required aircraft with 10-30 passenger seats to operate under stricter rules. Training and other requirements of 14 CFR Part 121 rules were applied to these midsize commuters, which previously had operated under the less strict Part 135 rules. Published crash rates obscured changes related to aircraft size. This research was undertaken to determine whether the rule change affected crash rates of aircraft with 10-30 passenger seats.
METHOD: We determined the number of passenger seats on each Par

ObjectiveWe sought to identify types of events pilots find most unexpected, strategies they employ to manage them, and recommendations for improving training. In doing so, we aimed to inform the development of more effective training programs, enhance pilot preparedness for unexpected events, and improve aviation safety.BackgroundThe increasing complexity of contemporary flight creates more opportunities for unexpected events. This poses a threat to aviation safety, as these events can cause cognitive overload, stress, and diminished performance. While current training focuses on known anomali

INTRODUCTION: Recently, an analysis of Canadian seaplane accidents terminating in water (1995-2019) was conducted, but ultralight water accidents were excluded due to differences from general aviation operations. This is the first literature that reports a series of ultralight accidents that occurred in water. The purpose of this paper is to identify the circumstances surrounding ultralight water accidents in Canada and to identify actions with the potential to improve survival.METHODS: Ultralight water accidents that were reported to the Transportation Safety Board of Canada between 1990 and

Injuries in civil aviation can occur as a consequence of work-related accidents happening in airport. The ground crew can sustain slips, trips, falls, and machinery accidents. Most such accidents are observed when aircraft is departing. This clinical report describes a case of an airport ground assistant severely injured by a helicopter after the strike with a main rotor blade that was slowing after that the craft was landed and the engine was stopped, and reports surgical emergency treatment of life-threatening facial lesions.

The aeromedical literature typically uses fatal accident rates (fatal accidents/100,000 aircraft flight hours) as a measure of aviation-related mortality. Calculation of population-based mortality rates (deaths/1,000,000 population) would permit comparison of aviation-related rates with rates due to other causes of death such as heart disease or motor vehicle accidents. Population-based mortality rates for air and space transport accidents were generated for the years 1979-89 using National Center for Health Statistics (NCHS) data sources. The NCHS reported 15,017 air and space transport death

The U.S. Federal Aviation Administration (FAA) recently provided detailed instructions on how Aviation Medical Examiners (AMEs) should assess and evaluate pilots for post-traumatic stress disorder (PTSD). The European, Australian and International Civil Aviation Organization guidelines for the assessment of PTSD in aviation are general guidelines and do not address the unique and specific circumstances of a flight crew per se. The starting point of the U.S. FAA's guidance is an already-established clinical PTSD diagnosis since it is known that PTSD compromises aviation safety and has been rela

Fatal civil aviation crashes in Ontario from 1985 to 1989 were studied. Data regarding accident circumstances, injury patterns and medical factors (disease, alcohol/drugs) which could have contributed to accident causation was obtained from a review of the files of the Chief Coroner for Ontario in Toronto and the aviation occurrence reports of the Transportation Safety Board of Canada. Forty-seven crashes involving mainly general aviation type aircraft but also 2 gyroplanes, 2 ultralights and a glider were reviewed. About half occurred during the cruise phase of the flight. Seventy (40 pilots;

BACKGROUND: In the past several decades, multiple studies have examined factors influencing occupant survival in aviation crashes, but only a few have addressed this question in Helicopter Emergency Medical Systems (HEMS) accidents. The four-point FIA Score is a valid tool to measure fatality risk in aviation crashes.
OBJECTIVE: The aim of the present study was to analyze the performance of the FIA Score when applied to German HEMS accidents, and to determine the prognostic value for fatalities and for survival.
METHODS: The FIA Score uses three parameters (F=fire; I=Instrument meteorologica
