Amplitude modulation of DPOAEs by acoustic stimulation of the contralateral ear.

CONCLUSION: Otoacoustic emissions generated by outer hair cells (OHCs) are influenced by stimulation of the contralateral ear via a neural pathway involving the olivo-cochlear efferent system. This is often referred to as a contralateral 'suppression reflex', but we suggest that such a term is inappropriate since distortion product otoacoustic emissions (DPOAEs) can be both enhanced and suppressed, and there is continuous modulation with no threshold effects. OBJECTIVE: To characterize the continuous amplitude modulation of DPOAEs by contralateral sound stimulation. MATERIALS AND METHODS: In

The Codacs™ direct acoustic cochlear implant actuator: exploring alternative stimulation sites and their stimulation efficiency.

This work assesses the efficiency of the Codacs system actuator (Cochlear Ltd., Sydney Australia) in different inner ear stimulation modalities. Originally the actuator was intended for direct perilymph stimulation after stapedotomy using a piston prosthesis. A possible alternative application is the stimulation of middle ear structures or the round window (RW). Here the perilymph stimulation with a K-piston through a stapes footplate (SFP) fenestration (N = 10) as well as stimulation of the stapes head (SH) with a Bell prosthesis (N = 9), SFP stimulation with an Omega/Aerial prosthesis (N = 8

Intermediate latency evoked potentials of cortical multimodal vestibular areas: acoustic stimulation.

OBJECTIVE: Loud acoustic stimuli at 500Hz activate the vestibular system. Intermediate-latency vestibular cortical potentials of multimodal cortex regions were investigated, beyond the 20ms time range. METHODS: Eighteen healthy subjects with 32-channel EEG and one epilepsy patient with right-sided intracortical electrodes received three types of stimuli: tone bursts capable of evoking vestibular evoked myogenic potentials (VEMP) in neck muscles and sham stimuli matched for either frequency or amplitude, which cannot evoke myogenic responses. RESULTS: VEMP-capable stimuli activated anterior i

Transcranial magnetic stimulation and acoustic trauma or hearing loss in children.

Transcranial magnetic stimulation is a non-invasive method used to assess motor function in humans; however, some reports suggest it may cause internal ear damage (cochlear). Eighteen patients with normal auditory function (ages 2 months to 16 years, mean 6.8 years), two medical doctors and two technicians who performed the studies were tested with brain stem auditory evoked potentials, otoacoustic emissions, acoustic reflex and a pure tone audiometric and logoaudiometric test when possible, before and after transcranial magnetic stimulation for central motor conduction studies in different ne

No evidence of hearing loss due to fetal acoustic stimulation test.

The fetal acoustic stimulation test is used increasingly for fetal assessment. To evaluate the possibility of acoustic trauma, 465 children who had been exposed to vibroacoustic stimulation in utero were screened at 4 years of age for high-frequency hearing loss. A hand-held audiometer with test tones at 25 dB for 1000 and 4000 Hz was used because responses at this level indicate hearing within normal limits for middle and high frequencies. Thirty-one children failed the test. Failure to respond was followed by inspection of the ear canal for wax, tympanometry, and reflex measurement to assess

The significance of amniotic fluid volume during intrapartum fetal acoustic stimulation.

The relationship of intrapartum fluid volume to the fetal response to acoustic stimulation was investigated in 112 patients who were in the latent phase of labor. Amniotic fluid volume appears to play an insignificant role in the genesis of fetal heart rate decelerations that occur after fetal acoustic stimulation, regardless of the amount of amniotic fluid present.

Acoustic stimulation on the round window for active middle ear implants.

Many clinical reports have discussed the effectiveness of stimulating the ear's round window (RW) with a tool to mitigate conductive and mixed hearing loss. The RW is one of the two openings from the middle ear into the inner ear. Various methods have been proposed to construct a highly efficient, easily implanted, and reliable RW transducer. Devices, however, such as floating mass transducers, have difficulty establishing proper contact without some degree of bone incision around the RW. Additionally, vibration energy may not be fully transmitted to the cochlea, but instead will be spread thr

Transcanal endoscopic ear surgery for perilymphatic fistula after electric acoustic stimulation.

Transcanal endoscopic ear surgery (TEES) will become a very useful therapeutic option. A perilymphatic fistula (PLF) is defined as sudden sensorineural hearing loss and/or vertigo caused by leakage of the perilymph through a fistula from the oval window and/or round window. We report a case of PLF after electric acoustic stimulation (EAS), a kind of cochlear implant, successfully treated by TEES. A 38-year-old man presented to our hospital with vertigo and hearing loss (HL). His vertigo was induced by Valsalva maneuvers. Eight months ago, he underwent EAS for his right ear for congenital senso

Transient and steady state auditory responses with direct acoustic cochlear stimulation.

OBJECTIVES: Direct acoustic cochlear implants directly stimulate the cochlear fluid of the inner ear by means of a stapes piston driven by an actuator and show encouraging speech understanding in noise results for patients with severe to profound mixed hearing loss. Auditory evoked potentials recorded in such patients would allow for the objective evaluation of the aided auditory pathway. The aim of this study was (1) to develop a stimulation setup for EEG recordings in subjects with direct acoustic cochlear implants, (2) to show the feasibility of recording auditory brainstem responses (ABRs)

Effect of prolonged contralateral acoustic stimulation on transient evoked otoacoustic emissions.

Although the suppressive effect of the medial olivocochlear system (MOCS) on peripheral auditory active mechanisms is well documented in humans, the effect of efferent inhibition over prolonged periods of acoustic stimulation is less well documented, especially as observed by transient evoked otoacoustic emission (TEOAE) suppression. The present study evaluated the relationship between the duration of contralateral acoustic stimulation and the suppression of TEOAE in 10 normal-hearing adults. TEOAE recordings with linear clicks (60 dB SPL) were measured at four intervals during 15 min of conti

Effects of contralateral acoustic stimulation on otoacoustic emissions following vestibular neurectomy.

This study demonstrates that, following unilateral vestibular neurectomy, the inhibitory effect of contralateral acoustic stimulation on evoked otoacoustic emissions is absent. The patient acts as her own control in that the unoperated side shows normal suppression of otoacoustic emission amplitude with contralateral acoustic stimulation. The lack of interaural suppression of otoacoustic emissions on the sectioned side, in the presence of normal acoustic reflex threshold levels, provides evidence that observed phenomena are not merely a function of middle ear reflex activity. It is concluded t

[Electrode model for multiaxonal stimulation of the acoustic nerve (author's transl)].

An electrode arrangement is described, which allows an isolated stimulation of 25--50 singular axons: By laser beams a small ceramic plate got 25--50 perforations through which glass isolated platin wires were so fixed that they end like a brush. Only the tips of the "hairs" are not isolated. The diameter of the electrodes is approximately 10 micrometer, the length ranges between 0,5 and 1,5 mm. This electrode system is enough stabile to be pressed into a hard boiled egg without being damaged.

Residual Hair Cell Responses in Electric-Acoustic Stimulation Cochlear Implant Users with Complete Loss of Acoustic Hearing After Implantation.

Changes in cochlear implant (CI) design and surgical techniques have enabled the preservation of residual acoustic hearing in the implanted ear. While most Nucleus Hybrid L24 CI users retain significant acoustic hearing years after surgery, 6-17 % experience a complete loss of acoustic hearing (Roland et al. Laryngoscope. 126(1):175-81. (2016), Laryngoscope. 128(8):1939-1945 (2018); Scheperle et al. Hear Res. 350:45-57 (2017)). Electrocochleography (ECoG) enables non-invasive monitoring of peripheral auditory function and may provide insight into the pathophysiology of hearing loss. The ECoG r

Access and Polarization Electrode Impedance Changes in Electric-Acoustic Stimulation Cochlear Implant Users with Delayed Loss of Acoustic Hearing.

Acoustic hearing can be preserved after cochlear implant (CI) surgery, allowing for combined electric-acoustic stimulation (EAS) and superior speech understanding compared to electric-only hearing. Among patients who initially retain useful acoustic hearing, 30-40 % experience a delayed hearing loss that occurs 3 or more months after CI activation. Increases in electrode impedances have been associated with delayed loss of residual acoustic hearing, suggesting a possible role of intracochlear inflammation/fibrosis as reported by Scheperle et al. (Hear Res 350:45-57, 2017) and Shaul et al. (Oto

Facilitation of electric forepaw stimulation-induced somatosensory activation in rats by additional acoustic stimulation: an fMRI investigation.

The influence of scanner acoustic noise on somatosensory activation pattern in rat cortex was investigated by functional magnetic resonance imaging (fMRI) using the blood oxygenation level-dependent (BOLD) contrast. This was achieved by two approaches. The first approach was to compare a conventional, loud fMRI sequence with a new sequence, in which the noise level was reduced by about 30 dB. In the second approach, the inner ear of the animal was destroyed, resulting in deafness. We compared the activation patterns obtained with both sequences before and after cochleotomy. The activated area

Activate & fire: a feasibility study in combining acoustic stimulation and continuous theta burst stimulation in chronic tinnitus.

BACKGROUND: Low frequency repetitive transcranial magnetic stimulation (rTMS) is commonly used to inhibit pathological hyperactivity of the auditory cortex in tinnitus. Novel and supposedly superior and faster inhibitory protocols such as continuous theta burst stimulation (cTBS) were examined as well, but so far there is not sufficient evidence for a treatment application in chronic tinnitus. rTMS effects in general are dependent on the brain state immediate before stimulation. This feasibility study was designed based on the concept to shift the pathological intrinsic brain state of tinnitus
