Morphology of the Undersurface of the Anterolateral Acromion and Its Relationship to Surrounding Structures.

BACKGROUND: A better understanding of the morphology underneath the acromion is needed to prevent complications after arthroscopic subacromial decompression. The precise correlations between the morphologic features underneath the acromion and the surrounding structures including the attachment of the coracoacromial ligament (CAL) and the origin of the deltoid middle head have not yet been determined in the absence of artifacts on the bony surface caused by dissection techniques. Moreover, anatomic findings in previous studies using only older-aged cadavers or dried bones may not reflect the m

Aneurysmal bone cyst of the acromion treated by selective arterial embolization.

Aneurysmal bone cysts are rarely located in the scapula and are especially rare in the acromion. We present an 8-year-old boy with a large aneurysmal bone cyst of the acromion that achieved curative treatment with one selective transcatheter arterial embolization. Five years after the procedure, healing of the lesion and remodeling of the acromion was observed without any evidence of recurrence.

Association of a large lateral extension of the acromion with rotator cuff tears.

BACKGROUND: Factors predisposing to tearing of the rotator cuff are poorly understood. We have observed that the acromion of patients with a rotator cuff tear very often appears large on anteroposterior radiographs or during surgery. The purpose of this study was to quantify the lateral extension of the acromion in patients with a full-thickness rotator cuff tear and in patients with an intact rotator cuff. METHODS: The lateral extension of the acromion was assessed on true anteroposterior radiographs made with the arm in neutral rotation. The distance from the glenoid plane to the lateral bo

The anterior tilt of the acromion: radiographic evaluation and correlation with shoulder diseases.

The aim of this study was to test whether the anterior tilt of the acromion can be objectively evaluated on lateral radiographs, and whether there is a relation between this anatomical feature and the most common shoulder diseases. Lateral radiographs of 15 dried scapulas were performed in neutral position and with 5 degrees of caudal, cranial, anterior and posterior angulations. Two hundred and forty-three shoulders, both asymptomatic and affected by chronic and post-traumatic impingement, calcific tendinitis and instability, were examined by conventional radiography. The presence of rotator-

Concomitant fracture of the coracoid and acromion after direct shoulder trauma.

Fractures of the acromial process or coracoid process of the scapula are rare. We present a combined fracture of the coracoid and acromion after direct trauma to the shoulder. An anteroposterior radiography with the central x-ray beam angled 25 degrees cephalad or an axillary lateral radiograph may be needed to detect these fractures.

Intraarticular lesions in calcifying tendinitis: incidence and association with the acromion index.

INTRODUCTION: Intraarticular pathologies are a common finding during arthroscopy for rotator cuff lesions. Both rotator cuff tears as well as cartilage lesions have been described as correlating with the acromion index. METHODS: In the current study, we aimed to determine the incidence of intraarticular lesions in calcifying tendinitis, to compare the incidence in patients with partial- and full-thickness tears of the rotator cuff, and to determine the acromion index in these groups. The incidence and type of accompanying intraarticular lesions were compared between three consecutive, age-mat

Resection of the acromion in the treatment of persistent rotator cuff syndrome of the shoulder.

Partial or complete excision of the acromion was performed in 25 patients (29 shoulders) with long-standing pain in the shoulder typical of rotator cuff syndrome. There was no verified history of trauma in nine cases (13 shoulders), while injury was the cause of pain in 16 patients (16 shoulders). In nine shoulders there was a small and in two a large rupture of the rotator cuff. Twelve of the 13 non-traumatic shoulders became painless postoperatively and the pain was relieved in one. In the 16 traumatic shoulders the relief of pain was complete in six, partial in nine, and one remained unchan

Bizarre parosteal osteocondromatous proliferation (BPOP) of the acromion with soft tissue recurrence.

Bizarre parosteal osteochondromatous proliferation (BPOP) is a benign but rare periosteal-originating chondrogenic tumor. It commonly arises from the hands and feet. It is slow-growing and often presents as a painless lump. On imaging, the mass is well-marginated and almost always remains contiguous with the cortical bone. Histologically, the lesion is composed of a disorganized admixture of fibrous tissue, bone, and cartilage with bizarre features. Treatment is surgical and local recurrence is common contiguous with bone. This case report demonstrates an uncommon acromial BPOP with the first

Evaluation of the cross-sectional area of acromion process for shoulder impingement syndrome.

BACKGROUND: Anatomic changes in the acromion have been considered a main cause of shoulder impingement syndrome (SIS). To evaluate the relationship between SIS and the acromion process, we devised a new morphological parameter called the acromion process cross-sectional area (APA). We hypothesized that the APA could be an important morphologic diagnostic parameter in SIS. METHODS: We collected APA data from 95 patients with SIS and 126 control subjects who underwent shoulder magnetic resonance imaging (MRI). Then we measured the maximal cross-sectional area of the bone margin of the acromion

ANATOMICAL RELATIONSHIP OF THE SUPRASCAPULAR NERVE TO THE CORACOID PROCESS, ACROMIOCLAVICULAR JOINT AND ACROMION.

OBJECTIVE: To establish the anatomical relationship of the suprascapular nerve (SSN) located in the suprascapular notch, to the medial border of the base of the coracoid process, the acromial joint surface of the acromioclavicular joint and the anterolateral border of the acromion. METHODS: We dissected 16 shoulders of 16 cadavers (9 males and 7 females). The distances from the suprascapular nerve (at its passage beneath the transverse ligament) to certain fixed points on the medial border of the base of the coracoid process, the acromial joint surface of the acromioclavicular joint, and the

The effect of arm position on the ultrasonographic measurements of the acromion-greater tuberosity distance.

Ultrasonographic measurements of acromion-greater tuberosity (AGT) distance have shown to be reliable and valid in the assessment of glenohumeral subluxation (GHS) in patients with stroke. The primary aim of this study was to investigate the effect of arm positions on ultrasonographic measurements of AGT distance. The secondary aim was to assess the intra-rater reliability of AGT distance in different arm positions. Sixteen healthy individuals with a mean age 28 standard deviation 11 years who gave informed written consent were recruited. Four clinically relevant arm positions for patients wit

The influence of the acromioclavicular joint degeneration on supraspinatus outlet impingement and the acromion shape.

PURPOSE: To assess the anatomic association of acromioclavicular joint degeneration to supraspinatus outlet impingement and the acromion shape. METHODS: Sagittal oblique magnetic resonance images of 49 shoulders in 49 patients were reviewed. 29 of them (mean age, 59 years) underwent surgery for impingement with or without rotator cuff tear (group 1), whereas the 20 controls (mean age, 27 years) were treated for shoulder instability without rotator cuff disease or acromioclavicular joint derangement (group 2). The supraspinatus outlet and the acromion shape of the 2 groups were compared. RESU

Biomechanical evaluation of acromion and scapular spine fracture repair techniques systematic review.

BACKGROUND: The incidence of reverse total shoulder arthroplasty (rTSA) is rising. Acromion and scapular spine fractures (ASFs) are a known complication following rTSA with an incidence rate of around 5%. The purpose of this study was to review biomechanical studies that have explored fixation techniques for ASFs following rTSA. METHODS: PubMed, CENTRAL, Embase, and MEDLINE were searched from January 1, 2000, to August1, 2025, to identify biomechanical studies evaluating fixation techniques of acromion or scapular spine fractures following rTSA. Study characteristics, specimen type, sample si

Osteosynthesis of an acromion fracture using a proximal clavicle plate: a case report.

Displaced acromion fractures are commonly treated surgically; however, no implants have been specifically designed for this fracture type. Although various plates have been used, fixation failure and implant-related irritation remain concerns. We report a case of acromion fracture fixation using a proximal clavicle plate. A 77-year-old man presented with right shoulder pain after a fall. Computed tomography revealed an Ogawa type III acromion fracture extending to the spinoglenoid notch. Through a posterior approach along the scapular spine, the fracture was anatomically reduced and fixed with

Changes in the Lateral Acromion Angle in Rotator Cuff Tear Patients with Acromioplasty.

BACKGROUND: Arthroscopic surgery has been established as an efficacious intervention for the treatment of rotator cuff tears. The primary aim of this study was to analyze the modifications in the lateral acromial angle (LAA) subsequent to rotator cuff repair surgery using single-row rivet fixation and double-row rivet fixation techniques. Furthermore, we sought to investigate the influence of LAA on the prognosis of rotator cuff repair surgery. METHOD: This observational study retrospectively enrolled 105 patients diagnosed with degenerative rotator cuff tears who underwent arthroscopic rotat

Morphological analysis of acromion and hook plate for the fixation of acromioclavicular joint dislocation.

PURPOSE: Acromioclavicular (AC) joint dislocation is a common sports injury. Hook plate fixation is currently widely used to treat this injury, as it can promote the natural healing of the ligament with good clinical outcomes. However, subacromial erosion and impingement are frequently observed post-operatively. It was hypothesized that the morphology and the contact characteristics between the hook portion and the acromion are the main causes of complications after hook plate fixation with the currently available commercial designs. METHODS: Three-dimensional reconstructed models of the AC j

Familial congenital bilateral agenesis of the acromion: a radiologically illustrated case report.

Familial congenital bilateral acromion absence was found in four members of one family. Only one of them presented with gradually increasing pain in his left shoulder, resembling a shoulder impingement syndrome. The other members did not have any symptoms. This is the first report of familial occurrence of this extremely rare congenital anomaly.

Neglected Acromion Fracture Causing Chronic Post-traumatic Shoulder Pain - A Case Report.

INTRODUCTION: Osteomyelitis pubis is a rare disease that presents with fever, pubic or hip pain, pain during hip movement, and claudication to avoid pain. It most frequently affects young athletes involved in sports that require twisting and cutting, but its exact cause is unknown and no definite treatment has been established. We report a case of pubic osteomyelitis in a young football player who was treated using the Masquelet technique and antibiotics with a good clinical course. Arthroscopic lateral acromion resection (ALAR) optimizes rotator cuff tear relevant scapula parameters.

BACKGROUND: The acromion index (AI), critical shoulder angle (CSA) and lateral acromion angle (LAA) are predictive for degenerative rotatory cuff tears. Their unfavorable values are associated with a suboptimal deltoid force vector. The aim of this study was to evaluate whether an optimization of the radiological parameters could be achieved through a specific arthroscopic lateral acromion resection (ALAR). MATERIALS AND METHODS: The procedure was performed in eight fresh frozen cadaver shoulders. True a.p. and axial radiographs were taken before and after the intervention for radiological ev
