Feasibility of Continuous Actigraphy in Patients in a Medical Intensive Care Unit.

BACKGROUND: Poor sleep and immobility are common in patients in the medical intensive care unit (MICU) and are associated with adverse outcomes. Interventions to promote sleep and mobilization in the MICU are gaining popularity, but feasible instruments to measure their effectiveness are lacking. Actigraphy may be useful for large-scale, continuous measurement of sleep and activity, but its feasibility in MICU patients has not been rigorously evaluated. OBJECTIVE: To evaluate the feasibility of continuous actigraphy measurement in consecutive MICU patients. METHODS: Wrist and ankle actigraph

Actigraphy-measured sleep characteristics and risk of falls in older women.

BACKGROUND: Prior studies have suggested that insomnia and self-reported poor sleep are associated with increased risk of falls. However, no previous study, to our knowledge, has tested the independent associations of objectively estimated characteristics of sleep and risk of falls, accounting for the use of commonly prescribed treatments for insomnia. METHODS: Study subjects were participants in the Study of Osteoporotic Fractures. In 2978 primarily community-dwelling women 70 years and older (mean age, 84 years), sleep and daytime inactivity were estimated using wrist actigraphy data collec

Perfect Enough to Sleep? Perfectionism and Actigraphy-Determined Markers of Insomnia.

OBJECTIVES: Perfectionism is an important factor in insomnia development and maintenance. Previous studies exploring the relationship between perfectionism and insomnia have predominantly relied on self-reported sleep measures. Therefore, this study sought to assess whether actigraphy-measured sleep parameters were associated with perfectionism. METHODS: Sixty adults (85% females, mean age 30.18 ± 11.01 years) were sampled from the Australian general population. Actigraphy-derived objective sleep measures, subjective sleep diary measures, the Frost Multidimensional Perfectionism Scale (FMPS),

Exploring the Congruence of actigraphy and the Pediatric Autism Insomnia rating Scale.

OBJECTIVE/BACKGROUND: Insomnia is common in children with autism spectrum disorder (ASD). We recently developed and validated the 21-item Pediatric Autism Insomnia Rating Scale (PAIRS). This report explores the associations and agreements between actigraphy and PAIRS. PARTICIPANTS METHODS: Children with ASD, with and without sleep problems, were assessed with a battery of parent-rated and clinician measures (N = 134). In a subset (n = 70), a wrist-worn actigraph measured sleep for five consecutive nights. Parents completed logs for scoring sleep intervals. Spearman correlations evaluated asso

Jointly modeling of sleep variables that are objectively measured by wrist actigraphy.

Recently developed actigraphy devices have made it possible for continuous and objective monitoring of sleep over multiple nights. Sleep variables captured by wrist actigraphy devices include sleep onset, sleep end, total sleep time, wake time after sleep onset, number of awakenings, etc. Currently available statistical methods to analyze such actigraphy data have limitations. First, averages over multiple nights are used to summarize sleep activities, ignoring variability over multiple nights from the same subject. Second, sleep variables are often analyzed independently. However, sleep varia

Durability of benefit after transcatheter tricuspid valve intervention: insights from actigraphy.

AIMS: Tricuspid regurgitation (TR) is associated with high mortality, morbidity and reduced physical capacity. This study was designed to examine the long-term impact of transcatheter tricuspid valve intervention (TTVI) on physical activity by using the method of actigraphy. METHODS AND RESULTS: Overall, we prospectively included 128 heart failure patients with severe TR (median age 79 years, 48% female) who were scheduled for TTVI. Patients were equipped with activity tracking devices for 1 week before TTVI, and again at 1-6 months and 1 year after TTVI. We compared continuous physical activ

Actigraphy and subjective sleep predictors of nine-year generalized anxiety disorder.

BACKGROUND: Sleep disturbances have been linked to generalized anxiety disorder (GAD) symptoms. However, cross-sectional studies, linearity assumptions, and limited predictor sets preclude identifying which unique sleep disturbance markers precede GAD symptoms. We thus harnessed machine learning (ML) to determine objective and subjective sleep disturbance predictors of nine-year GAD symptoms. METHODS: Community adults (N = 1054) underwent baseline surveys, clinical interviews, and seven-day sleep actigraphy protocols. GAD symptoms were reassessed nine years later. Seven ML models were examine

Apnea detection using wrist actigraphy in patients with heterogeneous sleep disorders.

Obstructive sleep apnea (OSA) and related hypoxia are well-established cardiovascular and neurocognitive risk factors. Current multi-sensor diagnostic approaches are intrusive and prone to misdiagnosis when simplified. This study introduces an enhanced single-sensor-based OSA screening method, leveraging novel signal processing and machine learning to ensure accurate detection across diverse populations. Wrist actigraphy is a widely-used and energy-efficient tool for respiratory rate estimation. The main challenge in OSA pattern recognition is handling various disturbances in real-world applic

Actigraphy against 32-hour polysomnography in patients with suspected idiopathic hypersomnia.

Actigraphy, a tool known for investigating sleep-wake patterns at home, lacks scientific validation in hypersomnolent subjects. We aim to validate an actigraphy-based sleep-wake prediction algorithm against 32-h continuous polysomnography in patients with suspected idiopathic hypersomnia, and to compare its performance to predict sleep-wake parameters assessed by polysomnography with those of a commercially available algorithm. Two hundred and six hypersomnolent subjects were included prospectively in a Reference Centre for Hypersomnias, and underwent a 32-h bedrest protocol, wearing wrist-act

Automated sleep stage classification based on tracheal body sound and actigraphy.

The current gold standard for assessment of most sleep disorders is the in-laboratory polysomnography (PSG). This approach produces high costs and inconveniences for the patients. An accessible and simple preliminary screening method to diagnose the most common sleep disorders and to decide whether a PSG is necessary or not is therefore desirable. A minimalistic type-4 monitoring system which utilized tracheal body sound and actigraphy to accurately diagnose the obstructive sleep apnea syndrome was previously developed. To further improve the diagnostic ability of said system, this study aims

Reliability of Actigraphy for the Assessment of Sleep and Circadian Rhythms in Rett and Related Syndromes.

PURPOSE: Actigraphy is being increasingly used to assess sleep and circadian rhythms among populations with intellectual and developmental disabilities and genetic syndromes, including Rett syndrome and related disorders, but the reliability of these measures in these populations is unclear. The primary purpose of the current study was to evaluate the impact of recording duration on the reliability of various measures of sleep and circadian rhythm in Rett and related syndromes. METHOD: Two 14-day recordings were collected between 4 and 12 weeks apart in a sample of 30 individuals (aged 2-36 y

Using a Non-Wearable Actigraphy in Nursing Care for Dementia With Lewy Bodies.

People who have dementia with Lewy bodies often have sleep disorders. We used non-wearable devices to record and categorize the sleep patterns of patients with Lewy body dementia. Individual sleep data at a dementia-care unit in Japan were recorded using non-wearables. One week's worth of data from 18 patients was analyzed. Median metrics for all participants were the following: sleep efficiency, 68% (23-89); sleep duration at night, 6.8 hours (1.6-11.1); times getting out of bed at night, 3.5 (0-13). We identified three types of abnormal sleep: extremely short sleep duration, excessive sleep

The Use of Actigraphy Differentiates Sleep Disturbances in Active and Inactive Crohn's Disease.

BACKGROUND: Sleep disturbances (SDs) are commonly reported in patients with Crohn's disease (CD). Several survey instruments assessing subjective measures of insufficient sleep have identified SDs in subjects with CD. However, there are limited data on objective measures of SDs in these patients as they relate to disease activity. In this prospective cross-sectional study, we compared objective estimates of sleep obtained using multiday wrist actigraphy in individuals with CD with varying disease activity. METHODS: Eighty patients with a diagnosis of CD were recruited to take part in the stud

Actigraphy-derived sleep fragmentation index: convergent validity and associations with clinical outcomes.

STUDY OBJECTIVES: Wake after sleep onset and sleep efficiency derived from actigraphy are common assessments of sleep fragmentation (or continuity). The sleep fragmentation index (SFI), measuring the frequency of sleep-wake transitions, is less understood. This study examined (1) the convergent validity between SFI and other sleep metrics obtained by actigraphy and polysomnography; and (2) associations of SFI with sleep symptoms, obstructive sleep apnea, periodic limb movement index, and cognition (Digit Symbol Coding test). METHODS: Cross-sectional analysis using logistic and multiple regres

Near-Death Experiences and Sleep Disturbance: An Exploratory Study Using Wrist Actigraphy.

Near-death experiences (NDEs) are nonordinary states of consciousness typically occurring on the brink of death. Sleep changes after NDEs have been described, including shorter sleep duration, longer sleep latency, and more sleep disturbances; however, objective verification is lacking. In this exploratory research, 57 participants took part in a 14-day actigraphy study and were assigned to three groups: those who have had an NDE ( n = 26); those who experienced a near-death event but without NDE ( n = 12); and those who had never come close to death ( n = 19). No significant differences were

Correlates of poor sleep based upon wrist actigraphy data in bipolar disorder.

BACKGROUND: Wrist-worn actigraphy can objectively measure sleep, and has advantages over self-report, particularly for people with Bipolar Disorder (BD) for whom self-reports might be influenced by affect. Clinically useful data reduction approaches are needed to explore these complex data. METHODS: We created a composite score of sleep metrics in BD based on 51 BD and 80 healthy comparison (HC) participants. Subjects wore an actigraph for up to 14 consecutive 24-h periods, and we assessed total sleep time (TST), wake after sleep onset (WASO), percent sleep (PS), and number of awakenings (NA)
