A chrome cobalt mandibular advancement appliance for snoring and obstructive sleep apnoea used during active aligner orthodontic treatment.

Adult orthodontics is now mainstream, made all the more acceptable by the advent of removable aligners. These patients may also coincidentally suffer from snoring and obstructive sleep apnoea (OSA), for which the indefinite wearing of a mandibular advancement device (MAD) during sleep may be required. Indefinite removable nocturnal orthodontic retention is now established practice. This case report describes a cast chrome cobalt MAD and its application in the management of a patient who, having very successfully managed his snoring and mild OSA with a removable cast chrome cobalt MAD, wished t

Time-Driven, Activity-Based Costing of Presurgical Infant Orthopedics: A Critical Component of Establishing Value of Latham Appliance and Nasoalveolar Molding.

BACKGROUND: Value-based health-care reform requires assessment of outcomes and costs of medical interventions. In cleft care, presurgical infant orthopedics is still being evaluated for clinical benefits and risks; however, the cost of these procedures has been largely ignored. This study uses robust accounting methods to quantify the cost of providing two types of presurgical infant orthopedics: Latham appliance treatment and nasoalveolar molding. METHODS: This is a prospective study of patients with nonsyndromic cleft lip and/or palate who underwent treatment with presurgical infant orthope

[The time-course change of brain activity during learning new movement patterns of pronunciation with orthodontic appliances and its correlation with self-evaluation of pronunciation difficulty: an fMRI study].

Pronunciation requires speed and accuracy, like many other fundamental activities such as breathing, eating, and walking. People can pronounce almost automatically without paying much attention to correctness, also like other activities. However, people may have to focus carefully on pronunciation when learning a foreign language, wearing dental appliances or rehabilitating after an injury or disease that affects speech. In the present study, we investigated neural changes underlying the process of learning new movement patterns of pronunciation in such cases. Nine healthy adults participated

A Comparative Assessment of Acceptance of Different Types of Functional Appliances.

Background Modern clinical orthodontics' functional appliances, a well-established modality of treatment, exhibit an amazing diversity of design. Clinical findings show that people have difficulty adjusting to these devices due to their size and unfixed positioning inside the mouth and that patient adaptation may vary based on the type of orthodontic functional appliance employed. Despite the fact that they appear to inflict more pain and soreness than, for example, removable plates, the effects of various orthodontic functional appliances on patients' acclimation have not yet been researched.

[Activator or Herbst, what's best?].

In this article, the short- and long-term results of two types of functional appliances are discussed regarding their ability to stimulate the mandibular growth at mandibular retrognathia and reduce an increased overjet. Removable functional appliances, or activators, are compared to a fixed functional appliance, the Herbst appliance. The activator, often consisting of an acrylic base, is advised to be worn for 12 to 20 hours a day. The Herbst appliance consists of interconnected bands around the molar- and premolar bands, keeping the mandibula continuously positioned forward by means of hinge

Cost-effectiveness of interceptive orthodontics: a long-term evaluation of early treatment strategies.

OBJECTIVES: Malocclusions occur at high frequencies in children and adolescents. While early orthodontic (interceptive) interventions may reduce the need for later comprehensive treatment, their cost-effectiveness in publicly funded health systems is unclear. This study compares the cost-effectiveness of interceptive orthodontic care and fixed-appliance therapy, focusing on total costs, treatment outcomes, and resource use. METHODS: A decision tree model was developed using observational data on treatment success rates, duration, and appointments to estimate resource use and costs for interce
