Acute type A intramural hematoma: The less-deadly acute aortic syndrome?

OBJECTIVE: To evaluate the short- and midterm outcomes of surgically managed acute type A intramural hematoma (IMH) versus classic acute type A aortic dissection (ATAAD). METHODS: From 1996 to February 2023, a total of 106 patients with acute type A IMH and 795 patients with classic ATAAD presented for open aortic repair at our institution. Data were obtained from the local Society of Thoracic Surgeons' Data Warehouse and medical chart review. RESULTS: Compared with the classic ATAAD group, the IMH group was older (65 vs 59 years, P < .001) and more likely to be female (45% vs 32%, P = .005)

Acute aortic syndromes: An internist's guide to the galaxy.

Acute aortic syndromes (AASs) are severe conditions defined by dissection, hemorrhage, ulceration or rupture of the thoracic aorta. AASs share etiological and pathophysiological features, including long-term aortic tissue degeneration and mechanisms of acute aortic damage. The clinical signs and symptoms of AASs are unspecific and heterogeneous, requiring large differential diagnosis. When evaluating a patient with AAS-compatible symptoms, physicians need to integrate clinical probability assessment, bedside imaging techniques such as point-of-care ultrasound, and blood test results such as d-

[Acute aortic syndrome. Review and update of the literature].

Acute aortic syndrome (AAS) is a term that describes interrelated aortic emergencies with similar clinical characteristics and challenges. These are aortic dissection (AD), intramural hematoma (IH), and penetrating atherosclerotic ulcer (PAU). The incidence of AAS is three cases per 100.000 persons per year. Diverse genetic disorders and acquired conditions have been related to the pathogenesis of this disease. Clinical features of patients with any of the three conditions comprising AAS are very similar. A high degree of clinical suspicion and imaging studies are necessary for an accurate dia

Acute aortic dissection in pregnancy with the marfan syndrome.

Acute aortic dissection (AAD) during pregnancy can be fatal to both the pregnant mother and the baby, particularly in patients with the Marfan syndrome. We report a case of the modified Bentall procedure in surgery for AAD in a 31-year-old pregnant woman at 24 weeks of gestation with the Marfan syndrome. The patient recovered well after the operation, but unfortunately, the fetus could not be saved.

Computer tomography imaging for suspected acute aortic syndrome.

BACKGROUND: Acute Aortic Syndrome (AAS) is a life-threatening condition that presents diagnostic challenges due to variable clinical presentations. Limited data exists regarding Computed Tomography Angiography (CTA) utilisation patterns and outcomes for patients with suspected AAS in Irish emergency departments (ED). This study evaluates trends in CTA utilisation and positivity rate for patients with suspected AAS presenting to a level 4 hospital ED. METHODS: A retrospective single-centre chart review was conducted of all adult patients (≥ 16 years) who underwent CTA aorta imaging requested b

[Surgical treatment of acute aortic syndrome: an update].

The term "acute aortic syndrome" describes a variety of acute and emerging aortic pathologies that include intramural hematoma, penetrating aortic ulcer and acute aortic dissection. However, the acute pathology of the thoracic aorta also includes the contained ruptures of aortic aneurysms, traumatic aortic ruptures and iatrogenic aortic dissections. In all these acute situations, in which emerging surgical treatment is often required, decision-making represents a crucial and extremely important phase, which often affects the patient's prognosis, in the short and long term. This review aims to

Acute Coronary Syndrome Following Transcatheter Aortic Valve Replacement.

BACKGROUND: Scarce data exist on coronary events following transcatheter aortic valve replacement (TAVR), and no study has determined the factors associated with poorer outcomes in this setting. This study sought to determine the clinical characteristics, outcomes, and prognostic factors of acute coronary syndrome (ACS) events following TAVR. METHODS: Multicenter cohort study including a total of 270 patients presenting an ACS after a median time of 12 (interquartile range, 5-17) months post-TAVR. Post-ACS death, myocardial infarction, stroke, and overall major adverse cardiovascular or cereb

Acute Aortic Thrombus Presenting as Cauda Equina Syndrome.

BACKGROUND: Occlusive abdominal aortic thrombus is a rare but critical clinical emergency with life-threatening consequences. Clinical presentation may mimic other diagnoses, resulting in a delay in the appropriate investigations for this condition. Spinal arterial involvement is a recognized complication of aortic thrombus and can result in pain, lower limb weakness, and loss of continence. These symptoms are usually associated with local spinal compression or stenosis manifesting as cauda equina syndrome (CES): a clinical finding of disrupted motor and sensory function to the lower extremiti

Thoracic aortic dissection presenting as acute coronary syndrome.

A patient presented with symptoms consistent with non-ST elevation myocardial infarction (NSTEMI) and was given appropriate antiplatelet and anticoagulant therapy. Despite treatment of the NSTEMI and fluid resuscitation the patient remained persistently hypotensive. Further imaging demonstrated an extensive Stanford type A dissection extending from the aortic root to the left common iliac artery. The patient was taken to theatre and had an emergency hemiarch replacement with repair of the aortic root. Post surgery the patient had an uneventful recovery. We present this case to highlight the im

Acute aortic syndrome: a 'last glance' before incision.

Acute aortic syndrome (AAS) (aortic dissection, intramural aortic haematoma, or penetrating atherosclerotic ulcer) is a surgical emergency. Computed tomography (CT) is the reference technique for the diagnosis of this critical situation. However, a few reports of false-positive images leading to unnecessary interventions have been published. It is important to recognize and understand the pitfalls in the CT diagnosis of AAS. We describe the case of a 76-year-old man with clinical signs of AAS and a CT-scan compatible with a diagnosis of aortic intra-mural haematoma, leading to emergency surger

Acute coronary syndrome following repair of aortic dissection.

Cardiac complications associated with type A aortic dissections are relatively common before and during the surgical repair. A cardiac event occurring a few days after routine recovery is rare though. We describe a case of acute occlusion of the left anterior descending artery by thrombus, 4 days after surgical repair, salvaged by emergency angioplasty and stenting. A possible explanation for the thrombus includes embolisation of a fragment of glue.

Predictors of Stable Aortic Dimensions in Medically Managed Acute Aortic Syndromes.

BACKGROUND: We aimed to identify predictors of stable aortic dimensions in medically managed type B aortic dissections (TBAD). METHODS: Medically managed TBAD patients from the International Registry of Acute Aortic Dissection with available aortic measurements at up to 24 months were included. Growth rate was calculated by dividing the largest descending diameter at the latest end point not influenced by intervention minus initial descending diameter, by the recorded time interval. Patients were split into 2 groups: without aortic growth (<0.0 mm/year, group I) and with aortic growth (>0.0 m

Acute Aortic Syndrome in a patient with COVID-19.

The global effects of the COVID-19 pandemic make it of the utmost importance to comprehend its mechanisms and define strategies for the most effective approach possible. The SARS-CoV-2 virus can be responsible for the induction of a hypercoagulable state, which can trigger vascular phenomena of venous etiology, specifically deep venous thrombosis or pulmonary embolism. Arterial thrombotic events associated with COVID-19 have also been described in the medical literature, although less frequently. In this paper the authors report the case of a 66-year-old man who was diagnosed with an Acute Aor

An evidence-based approach to acute aortic syndromes.

Aortic dissection, intramural hematoma, and penetrating atherosclerotic ulcer are parts of a spectrum of acute aortic syndromes that represent uncommon--but potentially deadly--diagnoses seen in the emergency department. The differential for acute aortic syndromes is large, as many conditions (including the much more common conditions of acute coronary syndromes and pulmonary embolism) present with many of the same chief complaints. This review looks at the features and classifications of acute aortic syndromes and presents evidence regarding the risk factors and chief complaints that can assi

[Acute aortic dissection: a challenging pitfall on the acute coronary syndrome pathway].

Acute aortic dissection is a life-threatening condition that is challenging and difficult to recognize since symptoms may mimic other time-dependent conditions like acute coronary syndrome, acute pulmonary embolism or abdominal conditions. We here describe the case of a middle-aged male with thoraco-abdominal pain, positive ECG and troponin tests that demonstrated a large type A aortic dissection at echocardiography done as part of the acute coronary syndrome work-up. This case report reminds clinicians that acute aortic dissection, although rare, should be considered as a differential diagnos

Acute aortic dissection-induced acute respiratory distress syndrome: pathogenesis and clinical implications.

Acute aortic dissection, a life-threatening cardiovascular emergency, is frequently complicated by acute respiratory distress syndrome. This complication exacerbates perioperative risks, poses challenges for clinical management, and impacts patient postoperative recovery. However, a comprehensive understanding of its pathogenesis remains elusive. This review synthesizes evidence to delineate aortic intimal tearing trigger the key initiating events comprising systemic inflammatory response, renin-angiotensin system dysregulation, high mobility group box 1 release, coagulation/fibrinolysis disor
