[Diagnosis of acute coronary syndrome].

Acute coronary syndrome is one of the most important differential diagnostic considerations in emergency medicine. It describes the constellation of newly occurring clinical symptoms, often accompanied by typical 12-lead ECG changes and the release of cardiac troponins. The spectrum includes unstable angina pectoris, non-ST-segment elevation myocardial infarction (NSTEMI), and ST-segment elevation myocardial infarction (STEMI). It is important to consistently carry out the diagnostic steps that are crucial for further therapeutic procedures to avoid delaying life-saving invasive coronary diagn

Acute Coronary Syndrome in Women.

Acute coronary syndrome is pathologically distinct in women and requires an appreciation of the specific risk factors, presenting symptoms, laboratory findings, and imaging results to treat correctly. Persistent disparities in mortality between men and women may be the result of failure to recognize and intervene, especially in the case of women aged less than 55 years. Protocols which establish criteria for activating the cardiac catheterization laboratory and which empower emergency department physicians to do so without delay show signs of eliminating disparities, as does guideline-directed

Acute Coronary Syndrome: Postdischarge Care.

Patients who have experienced an acute coronary syndrome should be evaluated in the office setting soon after hospital discharge. First follow-up appointments more than 6 weeks after discharge reduce adherence to therapy. Care coordinator involvement is helpful and associated with lower readmission and mortality rates. Postdischarge care should focus on blood pressure and lipid management with diet and drugs (all patients should be taking high-intensity statins if tolerated), along with antiplatelet therapy, including dual antiplatelet therapy if a stent was placed. Dual antiplatelet therapy s

Acute Coronary Syndrome: Diagnostic Evaluation.

Myocardial infarction (MI), a subset of acute coronary syndrome, is damage to the cardiac muscle as evidenced by elevated cardiac troponin levels in the setting of acute ischemia. Coronary artery disease is the leading cause of mortality in the United States. Chest pain is a common presentation in patients with MI; however, there are multiple noncardiac causes of chest pain, and the diagnosis cannot always be made based on initial presentation. The assessment of a possible MI includes evaluation of risk factors and presenting signs and symptoms, rapid electrocardiography, and serum cardiac tro

Pathophysiology of acute coronary syndrome.

Despite improvements in interventional and pharmacological therapy for atherosclerotic disease, it is still the leading cause of death in the developed world. Hence, there is a need for further development of more effective therapeutic approaches. This requires better understanding of the molecular mechanisms and pathophysiology of the disease. Recent research in the last decade has changed our view of acute coronary syndrome (ACS): from a mere lipid deposition to an inflammatory disease; from ACS exclusively due to plaque rupture to the novel definitions of plaque erosion or calcified nodule;

[Prevention after acute coronary syndrome].

Mortality is increased in acute coronary syndrome (ACS) related to the extent of myocardial injury even if percutaneous coronary intervention (PCI) was successful. The development of congestive heart failure (CHF) after PCI in ACS is of prognostic interest. Non-invasive imaging plays a major role for determination of structural myocardial damage and loss of function. Secondary prevention regarding pharmacologic and non-pharmacologic therapy is dependent on myocardial function and the presence or absence of CHF. Exercise training as part of the non-pharmacological therapy plays an important rol

Statins for acute coronary syndrome.

BACKGROUND: The early period following the onset of acute coronary syndrome (ACS) represents a critical stage of coronary heart disease, with a high risk of recurrent events and deaths. The short-term effects of early treatment with statins on patient-relevant outcomes in patients suffering from ACS are unclear. This is an update of a review previously published in 2011. OBJECTIVES: To assess the effects, both harms and benefits, of early administered statins in patients with ACS, in terms of mortality and cardiovascular events. SEARCH METHODS: We updated the searches of CENTRAL (2013, Issue

[Acute coronary syndrome after chemotherapy].

Systemic therapy for malignancy may be accompanied by an acute coronary syndrome (ACS), regardless of cardiovascular risk factors. We present three patients with few cardiovascular risk factors and no history of cardiovascular disease, who suffered an ACS within a week of starting systemic treatment of colorectal cancer, non-Hodgkin's lymphoma and breast cancer, respectively. In all three patients, systemic anti-cancer therapy was continued after making individualised adjustments to the treatment regimen. It is important to recognize the possible relationship between chemotherapy - including t

[Acute coronary syndrome-initiating factors].

Acute coronary syndrome(ACS), including acute myocardial infarction and unstable angina, is a clinical state induced by the thrombus formation following the disruption of unstable atherosclerotic plaque. Recent studies have reported that activated macrophages in the vulnerable plaques destabilize the fibrous cap by the induction of protease such as matrix metalloproteinases. Also, macrophages produce tissue factor and plasminogen-activator inhibitor type 1 those activate thrombus formation following plaque rupture. On the other hand, coronary spasm plays an important role in the pathogenesis o

[Acute coronary syndrome related factors].

Dysruption of vulnerable plaque is a major cause of acute coronary syndromes, including acute myocardial infarction and unstable angina. Therefore, plaque vulnerability has been important role in the pathogenesis of acute cardiovascular events. Recent studies have reported the mechanisms responsible for plaque rupture and erosion leading to acute thrombosis in the histologic, cellular, and molecular levels. A basic approach to atherosclerotic diseases is important and helpful for physicians in clinical situation. In this article, we review related factors leading to plaque rupture and erosion

[Pathophysiology of acute coronary syndrome].

This article reviews the current understanding of the pathophysiology of acute coronary syndrome and how these concepts have altered our clinical approach to the acute phase of coronary heart disease. Thrombosis due to erosion or, in most cases, rupture of a vulnerable atherosclerotic plaque underlies most acute coronary syndromes. The protective fibrous cap undergoes degradative processes controlled by inflammatory mediators that break down the interstitial collagen within the fibrous cap. Thrombus formation depends on factors in the solid-phase of the ruptured plaque as well as on fluid-phas

[Epidemiology of acute coronary syndrome].

As for myocardial infarction, the epidemiology of acute coronary syndromes is very different according to whether it concerns hospitalized patients only or acute coronary syndromes as a whole: many patients die before any medical intervention. The epidemiology of acute coronary syndromes is not well known since we have almost no specific data regarding unstable angina. The annual incidence of acute coronary syndromes in France is greater than 280 per 100,000 men and 60 per 100,000 women. The 1-month lethality rate is about 50%, increasing sharply with age.

Depression in acute coronary syndrome.

This study examined the prevalence of depression based on scores of 200 patients with acute coronary syndrome on the Emotion Profile Index of Plutchik and its relationship with the type of acute coronary syndrome and the severity of ischemic heart disease. Patients with acute coronary syndrome scored higher on depression than the control group. There was no difference in scores on Depression by type of acute coronary syndrome and no significant mean differences on Depression for patients with and without left ventricular failure. Patients with acute myocardial infarction and ventricular fibril

Clopidogrel and acute coronary syndrome.

Clopidogrel (Plavix-Sanofi-Synthelabo & Bristol-Myers Squibb) blocks platelet aggregation through an action distinct from that of aspirin. In the UK, it is licensed for the secondary prevention of atherosclerotic events and for this, we concluded 3 years ago that clopidogrel "appears to offer no worthwhile advantage over aspirin". After publication of the CURE (Clopidogrel in Unstable angina to prevent Recurrent Events) trial, which assessed adjunctive use of clopidogrel with aspirin in patients with acute coronary syndrome without ST-segment elevation (unstable angina or non-Q wave infarction

A Noonan Syndrome Mimicking Acute Coronary Syndrome.

Noonan syndrome is a genetic disorder that can present with a wide range of clinical manifestations, making diagnosis challenging. This article presents the case of a 29-year-old male who presented with chest pain and ST-segment elevation, initially raising suspicion for acute coronary syndrome. However, coronary angiography revealed only ectasia of the coronary arteries, with no other pathological findings. A detailed physical examination and echocardiography revealed a pulmonary murmur, pectus excavatum, and café-au-lait spots. Additionally, both echocardiography and cardiac magnetic resonan

[Bradyarrythmias in acute coronary syndromes].

In up to 30-40% of the cases acute coronary syndrome (ACS) is complicated by cardiac arrhythmias. The latter can be benign or malignant and mainly occur during the first 24 hours after myocardial infarction. Ischemia time being the key factor, arrythmias decreased dramatically since the implementation of accelerated reperfusion strategies. Bradyarrhythmias are often benign and self-limiting and are less frequent than tachyarrhythmias but can sometimes require specific treatment. The objective of this article is to provide an update on bradycardia and conduction system disorders occurring durin

The clinics of acute coronary syndrome.

Risk stratification and management of patients with chest pain continues to be challenging despite considerable efforts made in the last decades by many clinicians and researchers. The throutful evaluation necessitates that the physicians have a high index of suspicion for acute coronary syndrome (ACS) and always keep in mind the myriad of often subtle and atypical presentations of ischemic heart disease, especially in certain patient populations such as the elderly ones. In this article we aim to review and discuss the available evidence on the value of clinical presentation in patients with

Acute coronary syndrome in the elderly.

The spectrum of acute coronary syndrome (ACS) including unstable angina, non-ST-elevation myocardial infarction and ST-elevation myocardial infarction accounts for increasing numbers of deaths among persons age > or = 65 years in the US. This is important given demographic changes involving falling birth rates and increasing life expectancy. Elderly patients are likely to benefit the most from treatment of ACS, even though community practice still demonstrates less use of cardiac medications as an early-invasive approach among this population.
