Infective Endocarditis-Induced Lung Injury Mimicking Acute Vanishing Lung Syndrome.

Infective endocarditis is the infection of the endocardial surface of the heart valve. The right-sided endocarditis can be complicated by pulmonary injury. The pulmonary complications of infective endocarditis include pulmonary embolism, empyema, pleural effusion, lung abscess, and, in rare cases, pneumothorax. We present a case of bilateral pneumatoceles mimicking vanishing lung syndrome, a very rare pulmonary complication of right-sided infective endocarditis.

Lung epithelial cell apoptosis during acute lung injury in infancy.

CONTEXT: Apoptosis of lung epithelial cells is implicated in the pathogenesis of acute lung injury. Most research on this subject has focused on adults. Very little is known about a potential interaction of this process with lung development in children. OBJECTIVE: To summarize the current literature on lung epithelial cell apoptosis and common causes of acute lung injury in infants and young children and to identify new areas of research. DESIGN: A Medline-based literature search. RESULTS AND CONCLUSIONS: Few studies have focused on lung epithelial cell apoptosis during common causes of ac

Chlorine gas induced acute lung injury in isolated rabbit lung.

This study was designed to investigate the pathogenesis of chlorine gas (Cl2) induced acute lung injury and oedema. Isolated blood-perfused rabbit lungs were ventilated either with air (n=7) or air plus 500 parts per million (ppm) of Cl2 (n=7) for 10 min. Capillary pressure, measured by analysing the pressure/time transients of pulmonary arterial, venous and double (both arterial and venous) occlusions, was unchanged in both groups. In Cl2-exposed lungs, the fluid filtration rate increased from -0.228+/-0.25 to 1.823+/-1.23 mL min(-1) x 100 g(-1) (p<0.001) and the filtration coefficient increa

Acute Lung Injury and the NLRP3 Inflammasome.

Acute lung injury (ALI) manifests through harm to the capillary endothelium and alveolar epithelial cells, arising from a multitude of factors, leading to scattered interstitial alterations, pulmonary edema, and subsequent acute hypoxic respiratory insufficiency. Acute lung injury (ALI), along with its more serious counterpart, acute respiratory distress syndrome (ARDS), carry a fatality rate that hovers around 30-40%. Its principal pathological characteristic lies in the unchecked inflammatory reaction. Currently, the main strategies for treating ALI are alleviation of inflammation and preven

Molybdenum Nanodots for Acute Lung Injury Therapy.

Acute respiratory disease syndrome (ARDS) is a common critical disease with high morbidity and mortality rates, yet specific and effective treatments for it are currently lacking. ARDS was especially apparent and rampant during the COVID-19 pandemic. Excess reactive oxygen species (ROS) production and an uncontrolled inflammatory response play a critical role in the disease progression of ARDS. Herein, we developed molybdenum nanodots (MNDs) as a functional nanomaterial with ultrasmall size, good biocompatibility, and excellent ROS scavenging ability for the treatment of acute lung injury (ALI

Reversible Acute Lung Injury due to Bleomycin.

Bleomycin is a unique antibiotic agent with cytotoxic activity and is used successfully in various malignant diseases, such as Hodgkin lymphoma and germ cell tumors. Drug-induced lung injury (DILI) is one of the major limitations of bleomycin administration in particular clinical settings. The incidence varies among patients and depends on a variety of risk factors, such as cumulative drug dose, underlying malignant disease, and concurrent radiation. The clinical presentations are non-specific for bleomycin-induced lung injury (BILI), depending on the onset and severity of symptoms. There is n

Acute lung injury-from cannabis to COVID.

Recent world events have refocused attention on the pathology associated with clinical acute respiratory distress syndrome (ARDS). The vast majority of cases of clinical ARDS will have diffuse alveolar damage (DAD) histologically, but other histologies may occur less frequently. The aim of this paper is to provide a review of the pathology of DAD and acute fibrinous and organizing pneumonia and provide insights into the pathologic features associated with the E-cigarette/vaping-associated lung-injury outbreak and the ongoing SARS-CoV-2 pandemic.

Acute lung injury following refrigeration coil deicing.

We report a case of a worker who developed ALI requiring mechanical ventilatory support after attempting to melt ice condensate by applying the flame of an oxy-acetylene torch to refrigeration coils charged with a halocarbon refrigerant in a closed environment. A discussion of possible etiologies are discussed, including phosgene, carbonyl fluoride, and nitrogen oxides. Primary prevention with adequate respiratory protection is recommended whenever deicing is performed in a closed space environment.

[Electrical impedance tomography in acute lung injury].

Electrical impedance tomography has been described as a new method of monitoring critically ill patients on mechanical ventilation. It has recently gained special interest because of its applicability for monitoring ventilation and pulmonary perfusion. Its bedside and continuous implementation, and the fact that it is a non-ionizing and non-invasive technique, makes it an extremely attractive measurement tool. Likewise, given its ability to assess the regional characteristics of lung structure, it could be considered an ideal monitoring tool in the heterogeneous lung with acute lung injury. Th

The pathologist's approach to acute lung injury.

CONTEXT: Acute lung injury and acute respiratory distress syndrome are significant causes of pulmonary morbidity and are frequently fatal. These 2 entities have precise definitions from a clinical standpoint. Histologically, cases from patients with clinical acute lung injury typically exhibit diffuse alveolar damage, but other histologic patterns may occasionally be encountered such as acute fibrinous and organizing pneumonia, acute eosinophilic pneumonia, and diffuse hemorrhage with capillaritis. OBJECTIVE: To review the diagnostic criteria for various histologic patterns associated with a

Therapeutic strategies for severe acute lung injury.

OBJECTIVE: In the management of patients with severe acute lung injury and acute respiratory distress syndrome, clinicians are sometimes challenged to maintain acceptable gas exchange while avoiding harmful mechanical ventilation practices. In some of these patients, physicians may consider the use of "rescue therapies" to sustain life. Our goal is to provide a practical, evidence-based review to assist critical care physicians' care for patients with severe acute lung injury and acute respiratory distress syndrome. DATA SOURCES: We searched the PubMed database for clinical trials that examin

[Case of pemetrexed-induced acute lung injury].

A 65-year-old man was admitted with dyspnea. Chest radiograph showed left pleural effusion and chest CT scan revealed multiple left pleural masses. The diagnosis of malignant pleural mesothelioma was made by CT-guided needle biopsy. He received the combination chemotherapy with pemetrexed and carboplatin. The day after initiating chemotherapy, he had fever and dyspnea. Chest CT scan showed diffuse ground glass opacities mainly in the right lung, suggestive of drug-induced interstitial pneumonia. Corticosteroid pulse therapy was started, but he died of respiratory failure 10 days after beginnin

Cytokine-mediated inflammation in acute lung injury.

Clinical acute lung injury (ALI) is a major cause of acute respiratory failure in critically ill patients. There is considerable experimental and clinical evidence that pro- and anti-inflammatory cytokines play a major role in the pathogenesis of inflammatory-induced lung injury from sepsis, pneumonia, aspiration, and shock. A recent multi-center clinical trial found that a lung-protective ventilatory strategy reduces mortality by 22% in patients with ALI. Interestingly, this protective ventilatory strategy was associated with a marked reduction in the number of neutrophils and the concentrati

Monokine-induced acute lung injury in rabbits.

Interleukin-1 (IL-1) mediates components of the acute phase response, stimulates granulocyte metabolism, and induces endothelial cell surface changes. We studied in unanesthetized rabbits the effects of intravenous divided dose infusions of a murine monokine preparation containing IL-1 activity, on circulating granulocytes, their sequestration within the pulmonary microvasculature, pulmonary edema formation, and changes in pulmonary vascular permeability. Monokine administration induced significant (P less than 0.01) granulocytopenia as well as a significant (P less than 0.001) increase in mea

Acute lung injury. Assessment of tissue oxygenation.

The physiologic factors governing oxygen delivery to the peripheral tissues are briefly reviewed to show how tissue PO2 is defended when acute lung injury causes arterial hypoxemia. A loss of recruitable capillary reserve in the tissues may be another serious consequence of the same events that cause lung injury in the adult respiratory distress syndrome, and may cause a defect in tissue oxygen extraction.

Acute Lung Injury Caused by Mugwort Steaming.

Mugwort steaming is a traditional health practice with reported biological benefits, but its potential adverse effects on lung health remain unexplored. We report a case of a 48-year-old Japanese female who developed recurrent respiratory symptoms and abnormal lung shadows following occupational exposure to mugwort steaming. Initial diagnosis suggested nonfibrotic hypersensitivity pneumonitis. However, transbronchial lung cryobiopsy revealed findings consistent with acute lung injury (ALI). Multi-disciplinary discussion led to a final diagnosis of ALI caused by mugwort steaming. The patient's

Cutaneous lewisite exposure causes acute lung injury.

Lewisite is a strong vesicating and chemical warfare agent. Because of the rapid transdermal absorption, cutaneous exposure to lewisite can also elicit severe systemic injury. Lewisite (2.5, 5.0, and 7.5 mg/kg) was applied to the skin of Ptch1+/- /SKH-1 mice and acute lung injury (ALI) was assessed after 24 hours. Arterial blood gas measurements showed hypercapnia and hypoxemia in the lewisite-exposed group. Histological evaluation of lung tissue revealed increased levels of proinflammatory neutrophils and a dose-dependent increase in structural changes indicative of injury. Increased inflamma
