Atropine-Induced Anticholinergic Syndrome Following Off-Label Sublingual Administration for Refractory Sialorrhea: A Case Report.

A sublingual atropine ophthalmic solution is increasingly used as an off-label therapy for the symptomatic management of refractory sialorrhea because of its rapid onset of action, ease of administration, and generally favourable safety profile when used at recommended doses. Although generally well-tolerated at therapeutic doses, systemic anticholinergic toxicity may occur following medication administration errors or unintentional overdose. We report the case of a 76-year-old man with recurrent head and neck squamous cell carcinoma receiving palliative systemic therapy who presented with acu

[Relations between ventricle function, myocardial oxygen consumption and coronary circulation following sublingual administration of nitroglycerin].

The relationships between ventricular function, coronary blood flow, and myocardial oxygen consumption after sublingual administration of 1.6 mg nitroglycerin (TNG) were investigated in 12 patients. Left ventricular volumes, determined from cineventriculograms (enddiastolic volume index and endsystolic volume index), decreased (p less than 0.005) after TNG, as did left ventricular peak pressure (p less than 0.01), left ventricular enddiastolic pressure (p less than 0.005), mean aortic pressure (p less than 0.005), and mean pulmonary artery pressure (p less than 0.001). Crushed sublingual versus oral ticagrelor administration strategies in patients with unstable angina. A pharmacokinetic/pharmacodynamic study.

Oral administration of crushed ticagrelor tablets turned out to be an efficacious method that improves its pharmacokinetics and pharmacodynamics. This strategy, however, is unlikely to eliminate the drug-drug interaction in patients receiving intravenous morphine, as the impairment of the P2Y12 inhibitor absorption related to decreased propulsive motility of the gastro-intestinal tract is the most likely mechanism of interaction. Thus, we designed a pharmacokinetic and pharmacodynamic study setting the feasibility of platelet inhibition with a loading dose of ticagrelor given as crushed tablet

Plasma pharmacokinetics of desmopressin following sublingual administration: an exploratory dose-escalation study in healthy male volunteers.

OBJECTIVE: Desmopressin is usually administered intranasally in the treatment of central diabetes insipidus or nocturnal enuresis. The sublingual administration of desmopressin is expected to be an alternative to the intranasal route with advantages to children and to patients with allergic rhinitis or chronic rhinosinusitis. Therefore, the present study was carried out to explore the time-versus-concentration profile of desmopressin in plasma after sublingual administration to healthy volunteers. SUBJECTS AND METHODS: A total of 16 healthy male volunteers were enrolled in this open, explorat

Comparison of sublingual vs. intramuscular administration of vitamin B12 for the treatment of patients with vitamin B12 deficiency.

There are several methods to treat vitamin B12 deficiency (VB12d): intramuscular (IM), oral, sublingual (SL), and intranasal vitamin B12 (VB12) preparations. Large studies comparing the efficacy of SL vs. IM supplements are lacking. The aim of the present study was to compare the efficacy of SL versus the standard IM administration of VB12 in restoring B12 levels. This was a retrospective analysis of data from the computerized pharmacy records of Maccabi Health Service (MHS). Data were recorded for all patients older than 18 years of age who were prescribed VB12 during January 2014-December 20

Fast-Dissolving Pullulan Film as a Solid Dosage Form for Sublingual Administration of Nifedipine-Loaded Nanocapsules.

Fast-dissolving pullulan films were developed as a final dosage form for the sublingual administration of nanocapsules containing nifedipine (NIFE). Eudragit® RS100 nanocapsule suspensions presenting 1 mg/mL of NIFE, 150 nm in size, and positive zeta potential were converted into films using pullulan through the solvent-casting method. The films retained the nanometric size profile of the original suspension and NIFE homogeneity content. The conversion of the nanocapsule suspension into films improved the stability of NIFE. SEM analysis identified smooth, uniform surfaces, while AFM revealed i

Determining the conversion ratios for oral versus sublingual administration of tacrolimus in solid organ transplant recipients.

Tacrolimus is utilized as maintenance immunosuppression in solid organ transplant (SOT). Current literature has reported conflicting conversion ratios when transitioning between oral and sublingual tacrolimus, and the exact conversion ratio has not been fully established in SOT. The purpose of this study was to determine the conversion ratios between oral and sublingual tacrolimus needed to achieve equivalent whole blood concentrations in heart, kidney, liver, and lung transplant recipients. A retrospective, single-center analysis was conducted at Mayo Clinic in Florida. One hundred and eighte

Systemic immunological changes induced by administration of grass pollen allergens via the oral mucosa during sublingual immunotherapy.

Twenty-four patients suffering from grass pollen allergy underwent sublingual immunotherapy (SLIT) with standardized grass pollen extract for 1 year. In order to investigate immunological changes induced by the administration of allergens via the oral mucosa, the SLIT-spit method was applied. The cumulative dose of approximately 80 microg of major allergen (grass group 5 allergen), was relatively low. During the time of treatment, we could observe a significant increase in the levels of specific IgG and IgG4 antibodies. However, the titers of allergen-specific IgE antibodies showed a significa

Atropine-induced toxicity after off-label sublingual administration of eyedrop for sialorrhoea treatment in neurological disabled patients.

Sialorrhea is a troublesome and disabling symptom defined by the unintentional loss of saliva from the mouth, usually associated with swallowing disorders. Today there is no consensus about the management of sialorrhoea, but off-label use of ophthalmic atropine eyedrop administered sublingually may offer benefits, despite limited safety data. We report 2 cases of atropine overdose after sublingual administration illustrating that atropine can expose to severe adverse effects when administered sublingually. The noncompartmental pharmacokinetic study of atropine performed in 1 patient highlighte

Sublingual administration of Lactobacillus rhamnosus affects respiratory immune responses and facilitates protection against influenza virus infection in mice.

The extensive morbidity and mortality caused by influenza A viruses worldwide prompts the need for a deeper understanding of the host immune response and novel therapeutic and/or prophylactic interventions. In this study, we assessed the sublingual route as an effective means of delivering probiotics against influenza virus in mice. In addition, IgA levels, NK cell activity, T cell activation, and cytokine profiles in the lungs were examined to understand the mechanism underlying this protective effect. Sublingual administration of Lactobacillus rhamnosus provided enhanced protection against i
