Recurrence of major depressive disorder in hospitalized children and adolescents.

OBJECTIVE: To evaluate the outcome of a sample of children and adolescents hospitalized with major depressive disorder (MDD) and to assess different duration and severity criteria to define recovery and recurrence. METHOD: Fifty-nine of 70 children and adolescents were reevaluated 1 to 5 years later, and the intervening course of depression and other disorders was assessed using the Kiddie-Longitudinal interval Follow-up Evaluation (K-LIFE). RESULTS: Ninety-eight percent of subjects had recovered from their index MDD episode within 1 year of their initial evaluation, but 61% had at least one

Interaction sequences in families of psychiatrically hospitalized and nonpatient adolescents.

A KEY theme running through competing views of family influences upon adolescent development is that of directionality, on two levels: influences within the flow of family interaction; and influences from family processes to individual adolescent development. In this paper our focus is upon the first level, intrafamilial sequences within families. More specifically, we study links between psychiatric impairment in adolescence and developmentally relevant parent-child and parent-parent sequences. This report extends a previous investigation (Hauser et al. 1984), which described our new family c

Trends in adolescent pregnancies at Tygerberg Hospital, 1976-1980.

This study was undertaken to highlight trends relating to pregnancies among females aged less than 21 years seen at Tygerberg Hospital, Parowvallei, CP, during the 5-year period 1976-1980 inclusive. There was a fairly steady decline in the pregnancy rate for Coloured adolescents over this period. The rate for Black adolescents increased after a 3-year decline, and that for Whites also increased after a 2-year drop. The caesarean section rate increased after a marked drop in 1978, both in White adolescents and in the White group as a whole, possibly due to an increase in the instrumental delive

Hospitalization of adolescents: collecting the data base. Variations and implications.

A survey of 2208 adolescents (aged 12 through 18 years) hospitalized during 1977 identified variations in data-base development and unrecognized health problems. Forty-eight percent were admitted to obstetrics and gynecology, 29% to surgery, 13% to pediatrics/internal medicine combined, and 10% to psychiatry. Historical, physical, and laboratory data bases and diagnoses were examined from 325 records. All records included a history of present illness and blood pressure, and most included a past medical and family history. Excluding pediatric records, a minority of records included social, immu

An evaluation of hospital in-patient treatment in adolescent school phobia.

Thirty-two school-phobic youngsters of mean age 13 years were randomly allocated to two treatment groups after stratifying for sex. Length of in-patient management in a psychiatric hospital unit for young adolescents was evaluated. One group was admitted for three months and the other for six. Care was taken to ensure that cases had been fairly distributed between the groups by comparing them on a variety of clinical features. Follow-up assessments were carried out in a reliable fashion at six months, one year and two years after discharge. Outcome, overall, was similar to that found in a prev

Determinants of severe bradycardia in adolescents hospitalized for anorexia nervosa.

BACKGROUND: Severe bradycardia is an indication supporting hospitalization in adolescents with eating disorders. Some adolescents with anorexia nervosa (AN) and significant weight loss present with a normal pulse rate at admission, whereas others have severe bradycardia, suggesting that total weight loss is not the most important determinant of bradycardia. The aims of this study were to define the prevalence of severe bradycardia as the cause for hospital admission in adolescents with AN, to evaluate correlations between known determinants of severe bradycardia and pulse rate at admission, an

Addressing Reproductive Health in Hospitalized Adolescents-A Missed Opportunity.

PURPOSE: Adolescents are at high risk for sexually transmitted infections (STIs) and pregnancy. Since many adolescents have poor access to preventive care, hospitalizations present a critical opportunity to address adolescents' reproductive health. The purpose of this study was to assess provision of reproductive health services within a hospital setting. METHODS: Retrospective study of a consecutive sample of adolescent patients aged 13 years and older hospitalized on the hospitalist service at a large academic pediatric tertiary care center. Measures included sexual history documentation, p

Prediction of suicidality and violence in hospitalized adolescents: comparisons by sex.

OBJECTIVE: To examine psychological correlates of suicidality and violent behaviour in hospitalized adolescents and the extent to which these associations may be affected by their sex. METHOD: A sample of 487 psychiatric inpatients (207 male, 280 female), aged 12 to 19 years, completed a battery of psychometrically sound self-report measures of psychological functioning, substance abuse, suicidality, and violent behaviour. We conducted multiple regression analyses to determine the joint and independent predictors of suicide risk and violence risk. In subsequent analyses, we examined these ass

A prospective study of adolescent suicidal behavior following hospitalization.

Identification of factors associated with adolescent suicidal behavior following psychiatric hospitalization would facilitate development of effective aftercare interventions. This prospective study identified specific predictors of suicidal behavior in 100 adolescents during a 6-month follow-up period. Standardized baseline assessments and structured follow-up telephone interviews were conducted with adolescents and their guardians. Eighteen percent of adolescents reported suicidal behavior during the follow-up period, primarily of low medical lethality and minimal or ambivalent suicidal inte

Psychosis, aggression, and self-destructive behavior in hospitalized adolescents.

The authors studied the history of aggressive and self-destructive behaviors in psychotic and nonpsychotic hospitalized adolescents (N = 137). A multidimensional measure of self- and other-directed aggression was retrospectively applied to each patient's social and developmental history. Nonsignificant gender and diagnostic differences were obtained on ratings of violence and suicide. Broader definitions of internal and external aggression yielded nonsignificant diagnostic differences, but gender differences were observed on both internal and external aggression measures. Females displayed gre

Adolescent elopement from a psychiatric hospital. Multiple dimensions.

The authors report an unusual epidemic of elopement from an adolescent ward of a psychiatric hospital and demonstrate the utility of the single case study method with the hospital ward as the unit of study. The authors found that elopement was associated with patient factors (such as the absence of the diagnosis of affective disorder and a previous history of running away from psychiatric treatment) and patient-therapist interaction factors (separations from treatment team members), both of which seemed to interact with the milieu. The authors discuss how the single case study permits examinat

Psychopathology and management of hospitalized Ethiopian immigrant adolescents in Israel.

Over a period of about 12 months a large number of Ethiopian Jews emigrated to Israel under very stressful conditions. Many of these immigrants were adolescents who came on their own, leaving their families behind. Despite the culture shock involved, most have done well. Nine of these youngsters, who developed psychiatric disabilities severe enough to warrant hospitalization, were referred to us. In this paper we describe the clinical and management problems posed by these patients.

Social skills training with hospitalized adolescents: an evaluative experiment.

The effectiveness of social skills training on adolescent males admitted to a psychiatric hospital was evaluated through the use of empirically derived anger scenarios, blind raters, matched experimental and control treatment groups, and a repeated measures analysis. Results indicated that the social skills training effectively improved the dimension of verbal response and eye contact. Facial expression did not show measurable improvement. The use of specific skills components and scoring criteria in the social skills training was emphasized. Overall, support was found for the efficacy of soci

Parent-child relationships of adopted adolescents in a psychiatric hospital.

This study compared the parent-child relationships of 140 adopted and non-adopted adolescents treated in a psychiatric hospital through examination of information contained in the adolescents' medical records. Specifically, comparisons were made of the mention of parental contribution to the problems precipitating hospitalization, psychiatric restriction of parental visits to hospitalized offspring, and referral of parents to an adjunct parents' group. Data were gathered by uninformed research assistants, from the hospital charts of adolescents who had already been discharged from the hospital
