INTRODUCTION: To support advance care planning (ACP), a conversation game, Anticip'action, and a connected 3-step ACP intervention using the game were developed. This study assessed the acceptability, feasibility, and effect of the intervention with patients undergoing dialysis at a tertiary hospital.
METHODS: Dialysis nurses received a 10-hour training in ACP and use of the card game Anticip'Action. All patients on dialysis in the nephrology division were eligible. Acceptability and feasibility were assessed using questionnaires and note taking. Effect was assessed using pre-post number of a

BACKGROUND: Advance care planning (ACP) is a widely advocated strategy to improve outcomes at end-of-life care for patients suffering from heart failure (HF). However, finding the right time to start ACP is challenging for healthcare providers because it is often a sensitive issue for patients with HF and their families. We interviewed patients with cardiovascular diseases regarding ACP readiness and investigated the relationship between the ACP desire and multiple clinical prognostic parameters.
METHOD: Eighty-one patients (average age 81.8 ± 10.3 years old, 42 men, 62 cases of HF) who intro

BACKGROUND: There is increasing awareness of the need for an integrated palliative care approach in chronic progressive neurological diseases. Advance care planning (ACP) is an integral part of this approach. As a systematically organized and ongoing communication process about patients' values, goals and preferences regarding medical care during serious and chronic illness, ACP aims to involve patients in decision-making before they become cognitively and communicatively incapable. However, it remains underutilized in daily neurological practice except for speciality clinics such as ALS cente

Patients with chronic kidney disease typically suffer a cascade of comorbid conditions, the magnitude of which have formidable impact on advance care planning (ACP). Complex health care decisions are complicated further by contextual issues that may change over time. A dynamic and evolving process, ACP ideally begins early in the continuum of chronic kidney disease, long before end-stage kidney disease is reached. Planning ahead for care is preparatory to making decisions about kidney replacement therapy and can make for a smooth transition in addition to preventing the start of dialysis by de

OBJECTIVES: To determine whether depression and anxiety are associated with advance care planning (ACP) engagement or values concerning future medical care.
DESIGN: Cross-sectional.
PARTICIPANTS: English- and Spanish-speaking patients, aged 55 years and older, from a San Francisco, CA, county hospital.
MEASURES: Depression was measured by the Patient Health Questionnaire 8-item scale, and anxiety was measured by the Generalized Anxiety Disorder 7-item scale, using standardized cutoffs of 10 or more for moderate-to-severe symptoms. ACP engagement was measured using validated surveys of ACP b

This article describes the role of nurses assisting people with degenerative illness in advance care planning (ACP) for a time when they may lose decision-making capacity. It looks at the concept of advance decisions to refuse treatment (ADRT), as defined in the Mental Capacity Act 2005 , exploring the legal, ethical and philosophical ramifications of carrying out, or overriding, formerly expressed wishes of someone who has subsequently lost decision-making capacity. It uses an illustrative composite case study of an individual with Huntington's disease whose prognosis includes future deterior

BACKGROUND: ACP documentation in oncology emergency care frequently fails to capture patient values, reasoning, and voice, limiting its clinical utility.
MEASURES: A five-domain ACP Documentation Quality Rubric (patient/family voice, goals/values, decision clarity, shared decision-making, plan integration) classified notes as Basic, Adequate, or Exemplary; the primary outcome was the proportion rated Exemplary.
INTERVENTION: Six components launched February 2026: provider education, exemplar dissemination, reviewer calibration, a point-of-care poster, unit champions, and formal kudos recogni

The percent of the American population that self-identifies as LGBTQ+ is currently doubling generation after generation. Advance Care Planning and Health Care Proxy completion have unique challenges in this growing, aging queer community due to nontraditional family structures, as well as risk factors from historical and current discrimination. LGBTQ+ older adults benefit from a systematized and provider-led approach to assistance with making difficult decisions and mitigating their vulnerability in health settings. This article highlights the implementation of a novel, community-emergent, and

IMPORTANCE: Advance care planning (ACP), in which patients or their surrogates discuss goals and preferences for care with physicians, attorneys, friends, and family, is an important approach to help align goals with actual treatment. ACP may be particularly valuable in patients with advanced serious illnesses such as Alzheimer's disease and related dementias (ADRDs) for whom surgery carries significant risks.
OBJECTIVE: To determine the frequency, timing, and factors associated with ACP billing in Medicare beneficiaries with ADRD undergoing nontrauma inpatient surgery.
DESIGN: This national

Given the barriers to advance care planning (ACP) and low utilization of outpatient advance directives in hospital settings, it is unclear if ACP in primary care for adults who have experienced homelessness is effective and a valuable use of clinical time. As part of our ACP Project, we examined the feasibility and outcomes of ACP in primary care for patients who have experienced homelessness. We found that such efforts had significant impacts on patients' hospital care and allowed their wishes to be honored when they lost capacity for medical decision-making, particularly at the end of life.

OBJECTIVE: Motor neuron disease (MND) is an incurable progressive illness, characterized by incessant deterioration of neuromuscular function. Timely commencement of advance care planning (ACP) may enable patients to participate in future care choices. The present systematic review aimed to summarize what is known about the prevalence, content, patient/caregiver benefits, healthcare professional (HCP) awareness/support, and healthcare outcomes associated with ACP in the MND setting.
METHOD: Quantitative and qualitative studies were identified through database searches and eligibility assessed

The prevalence of end-stage renal disease continues to increase, and dialysis is offered to older and more medically complex patients. Pain is problematic in up to one-half of patients receiving dialysis and may result from renal and nonrenal etiologies. Opioids can be prescribed safely, but the patient's renal function must be considered when selecting a drug and when determining the dosage. Fentanyl and methadone are considered the safest opioids for use in patients with end-stage renal disease. Nonpain symptoms are common and affect quality of life. Phosphate binders, ondansetron, and naltr

BACKGROUND: We aimed to evaluate the preferred and received end-of-life (EoL) care in glioblastoma patients who participated in a local nurse-led advance care planning (ACP) program, as well as the perceived quality of care.
METHODS: Twelve proxies of patients with glioblastoma who participated in an ACP program completed a study-specific questionnaire after the patient deceased, comprising the following topics: organization of EoL care, patient's preferences and wishes, received care during the last 3 months, and proxies' experiences with EoL care. The outcomes were compared with those from

BACKGROUND: Palliative care, including advance care planning (ACP), ensures a patient's medical care aligns with their values, goals, and priorities throughout serious illness or injury. The Alaska Native and American Indian (AN/AI) population is greatly increasing and less likely to have documentation on ACP conversations. AN/AI peoples are more likely to engage in ACP when it is culturally-tailored to their specific needs. Jumpstart AN/AI is a culturally-tailored tool to assist providers and customer-owners with starting ACP conversations. We conducted a qualitative study to gather feedback
