The prehospital management of avalanche victims.

Avalanche accidents are frequently lethal events with an overall mortality of 23%. Mortality increases dramatically to 50% in instances of complete burial. With modern day dense networks of ambulance services and rescue helicopters, health workers often become involved during the early stages of avalanche rescue. Historically, some of the most devastating avalanche accidents have involved military personnel. Armed forces are frequently deployed to mountain regions in order to train for mountain warfare or as part of ongoing conflicts. Furthermore, military units are frequently called to assist

Emergency Department Management of Maxillofacial Trauma.

Patients with maxillofacial trauma require careful evaluation due to the anatomical proximity of the maxillofacial region to the head and neck. Facial trauma can lead to life-threatening airway compromise or hemorrhage, or permanent facial deformity. Although the Advanced Trauma Life Support guidelines provide a framework for the management of trauma patients, they do not provide a detailed reference for many subtle or complex facial injuries. In addition to an overview of maxillofacial trauma pathophysiology, associated injuries, and physical examination, this review will also discuss relevan

A survey of undergraduate teaching in anaesthesia.

Currently, no well accepted and clearly defined 'core' curriculum for undergraduate anaesthesia, teaching exists. To address this deficiency, we surveyed 73 university departments of anaesthesia and intensive care. Sixty-five replied from South-east Asia (12), Australasia (13), the UK and Ireland (28) and Canada (12). A questionnaire containing 37 items ranging from departmental structure to curriculum content was used. We found significant regional differences. Overall, most departments taught pharmacology of anaesthetic drugs (83%), pre-operative assessment (92%) and care of the unconscious

Meaning-centered psychotherapy for existential distress in advanced cancer: A culturally informed case report from Vietnam.

OBJECTIVES: Existential distress is common among patients with advanced cancer receiving palliative care and is associated with hopelessness, loss of meaning, spiritual distress, and desire for death. Meaning-centered psychotherapy (MCP) has shown potential in reducing existential suffering and improving psychological well-being in patients with advanced cancer. This article presents a case report describing the application of MCP in a young woman with advanced cancer experiencing depressive symptoms, trauma-related distress, hopelessness, and loss of meaning near the end of life. METHODS: We

Clinical Utility of Routine Chest X-Rays During the Initial Stabilization of Trauma Patients.

OBJECTIVES: The Advanced Trauma Life Support (ATLS) course encourages the use of chest x-ray (CXR) to identify injuries that may change clinical management during the initial stage of trauma resuscitations. Several studies have failed to show benefit for the routine use of CXR without a clinical indication, however. We sought to validate these findings by determining the incidence of clinically significant findings discovered on a portable single-view CXR during the initial stabilization of trauma patients at a Level 1 trauma center. METHODS: Using our electronic medical record system, we sea

Virtual reality simulation to enhance advanced trauma life support trainings - a randomized controlled trial.

BACKGROUND: Advanced Trauma Life Support (ATLS) is the gold standard of initial assessment of trauma patients and therefore a widely used training program for medical professionals. Practical application of the knowledge taught can be challenging for medical students and inexperienced clinicians. Simulation-based training, including virtual reality (VR), has proven to be a valuable adjunct to real-world experiences in trauma education. Previous studies have demonstrated the effectiveness of VR simulations for surgical and technical skills training. However, there is limited evidence on VR simu

A protocol of trauma care in the emergency service including MDCT imaging.

Trauma is the principal cause of young peo ple's death. Care should be appropriate to the severity of the injuries and involves a multidisciplinary team around the clock. In this article, we examine a protocol of care based on ATLS (Advanced Trauma Life Support) recom mendations, including advances in imaging technology, which have greatly increased quality care in our institu tion. The protocol is based on patient stability and on injury mechanisms. The challenge is to avoid wasting time in the definitive treatment of lesions. Multi-detector computed tomogra phy (MDCT) has brought complete an

Evaluating age-based vital sign cutoffs for pediatric trauma: a multicenter evaluation of the Japanese trauma data bank.

PURPOSE: To evaluate differing vital signs criteria for the prediction of major trauma in children. METHODS: We conducted a multicenter retrospective cohort study including children (< 18 years) from the 2019-2022 Japan Trauma Data Bank. We compared Pediatric Advanced Life Support (PALS), Advanced Trauma Life Support (ATLS), and empirically-derived criteria for first-measured emergency department vital signs. The primary outcome was major trauma, defined as having an elevated Injury Severity Score and meeting Need for Trauma Intervention criteria (mortality, transfusion, intensive care unit a

Indications and outcomes of extracorporeal life support in trauma patients.

BACKGROUND: The use of extracorporeal life support (ECLS) in the trauma population remains controversial and has been reported only in small cohort studies. Recent ECLS technical advances have increased its use as an advanced critical care option in trauma. Given the degree of resource utilization, costs, and expertise required to provide ECLS support in trauma patients, we sought to perform a multi-institutional study to assess the indications and outcomes of ECLS in trauma. METHODS: A retrospective review of adult (≥16 years) trauma patients receiving ECLS support in the Extracorporeal Life

Evaluation of just-in-time trauma training for international surgical military preparedness at a US Civilian Levell Trauma Center: A proof of concept.

BACKGROUND: Increasing global conflicts continue to heighten the need for increased focus on preparedness for military physicians and surgeons. Simulation has recently been adopted by civilian surgical trainees to offset the problem of increased work hour restrictions and shift the current focus toward minimally invasive techniques. We hypothesized that just-in-time trauma training, incorporating both focused clinical and simulated experience at our civilian Level I Trauma Center, would increase the competence and confidence of international military physicians in trauma care. METHODS: We per

Triage and management of cranial injuries.

This article discusses basic head and intracranial CNS anatomy, cerebral physiology, and classifications of head injury. Management principles and the practice of head-injury evaluation are reviewed and supplemented by the presentation of selected head-injury scenarios. The optimal evaluation and treatment of the head-injured patient is predicated on initially following established ATLS principles and preventing secondary injury. Once specific injuries have been identified, maintenance of cerebral perfusion and oxygenation is the key to maximizing patient outcomes, and this often requires meas

Hypertrophic cardiomyopathy in the trauma patient: A case report and review of the literature.

INTRODUCTION: Hypertrophic cardiomyopathy (HCM), a prevalent genetic cardiac condition characterized by myocardial thickening, poses unique challenges in trauma care. Sparse evidence seems to agree that HCM patients have worse outcomes following non-cardiac surgeries, particularly emergent procedures. However, despite a prevalence of 1 in 500 in the US population, the intersection of HCM and Advanced Trauma Life Support (ATLS) remains undiscussed in current literature. CASE PRESENTATION: A 54-year-old female with unknown past medical history presented as a level 2 trauma alert following a mot

Electrical shock and lightning strike.

Cardiac arrest from electrical shock or lightning strike is associated with significant mortality and requires modification and extension of standard advanced life support measures to achieve successful resuscitation. Patients who experience electrical shock or lightning strike may sustain cardiac and respiratory arrest secondary to the direct effects of current. However, the majority of victims have associated multisystem involvement, including neurologic complications, cutaneous burns, and associated blunt trauma. As a result, a combination of advanced cardiac life support measures and advan

Update on the management of dentoalveolar trauma.

PURPOSE OF REVIEW: Dentoalveolar injuries are the most common type of facial injury and are often associated with concomitant facial fractures. These injuries may initially be seen by surgeons who do not have formal dental training. This article reviews the assessment and management of dentoalveolar fractures. RECENT FINDINGS: Semi-flexible splinting of traumatized teeth for shorter periods of time than previously suggested is associated with better long-term outcomes. The use of dental implants to replace teeth lost to trauma is a predictable restorative option. SUMMARY: Surgeons need to be

Non-randomized comparative study on the efficacy of a trauma protocol in the emergency department.

PURPOSE: Trauma is a major health problem in developing countries and worldwide which requires many resources and much time in an emergency department (ED). Although Advanced Trauma Life Support (ATLS) is the gold standard, operator dependence can affect the quality of care. The objective was to identify differences in numbers and time to lifesaving interventions, investigation, ED length of stay, and mortality between an in-house protocol and conventional practice. METHODS: This was a single-center prospective non-randomized study for adult trauma patients in the ED. The trauma protocol was

Terrestrial trauma systems in support of human spaceflight.

As spaceflight expands to the private sector, the number of space travelers potentially at risk for traumatic injury during a spaceflight emergency will increase. Intersection with terrestrial health care systems will most likely occur during launch and landing phase emergencies for the majority of providers. The terrestrial trauma system should be prepared to provide prehospital and hospital care for this unique patient population. Emergency medical providers should be aware of potentially altered physiology from long-duration microgravity exposure as well as chemical and thermal exposures. F

Prehospital intraosseous access in combat trauma: a retrospective registry analysis.

INTRODUCTION: Intraosseous (IO) devices have become a vital tool in trauma care for establishing rapid circulatory access, particularly in emergency and prehospital environments where peripheral vascular access may be delayed or unsuccessful. In modern combat settings, detailed data on prehospital IO use, indications and outcomes remain limited. We aimed to describe the indications and success rates of prehospital IO access in combat trauma casualties treated by military advanced life support providers during the 2023-2024 Israel-Gaza conflict, and to report any acute complications documented
