Screening for Adverse Childhood Experiences: A Critical Appraisal.

Adverse childhood experiences (ACEs) are common and can impact health across the life course. Thus, it is essential for professionals in child- and family-serving roles, including pediatric and adult primary care clinicians, to understand the health implications of childhood adversity and trauma and respond appropriately. Screening for ACEs in health care settings has received attention as a potential approach to ACEs identification and response. Careful examination of the existing evidence on ACEs screening and consideration, from a clinical and ethical perspective, of the potential benefits,

Adverse Childhood Experiences and Adult Mental Health Outcomes.

IMPORTANCE: Exposure to adverse childhood experiences (ACEs) has consistently been associated with multiple negative mental health outcomes extending into adulthood. However, given that ACEs and psychiatric disorders cluster within families, it remains to be comprehensively assessed to what extent familial confounding contributes to associations between ACEs and clinically confirmed adult psychiatric disorders. OBJECTIVE: To investigate whether associations between ACEs and adult mental health outcomes remain after adjusting for familial (genetic and environmental) confounding. DESIGN, SETTI

Measuring adverse childhood experiences with latent class trajectories.

BACKGROUND: Adverse childhood experiences (ACEs) are associated with several negative health and behavioral outcomes during adolescence, but most of the extant research has employed ACEs scores at one or two time points. Studies have not assessed whether latent class ACEs trajectories affect adolescent problem behaviors and conditions. OBJECTIVES: We used longitudinal data from the Fragile Families and Child Wellbeing Study (FFCWS, n = 3444) to assess ACEs at several time points and empirically developed latent class trajectories. We then examined the sociodemographic characteristics of youth

Early adverse childhood experiences and adolescent sleep outcomes.

BACKGROUND: Sleep is critical for physical, mental, and emotional health. This may be particularly true for adolescents experiencing rapid physiological changes. Relatively little is known about how adverse childhood experiences (ACEs) are implicated in adolescent experiences with sleep. OBJECTIVE: We use data (from the Future of Families and Child Wellbeing Study (FFCWS, n = 3444) to assess the relationship between early ACE exposure (by age 5) and various adolescent sleep outcomes. We anticipate that early ACEs will be associated with poor adolescent sleep outcomes. METHODS: FFCWS data inc

Health and adverse childhood experiences among homeless youth.

BACKGROUND: Homelessness is associated with health problems and with adverse childhood experiences (ACEs). The risk of chronic health conditions for homeless compared to housed youth, and how this risk interacts with ACEs remains unclear. This study investigated the relationship between ACEs, housing, and child health, and whether: 1) ACEs and health vary by housing context; 2) ACEs and homelessness confer independent health risks; and 3) ACEs interact with housing with regard to adolescent health. METHODS: Using data from 119,254 8th-11th graders, we tested independent and joint effects of A

Adverse childhood experiences and health anxiety in adulthood.

Childhood experiences are thought to predispose a person to the development of health anxiety later in life. However, there is a lack of research investigating the influence of specific adverse experiences (e.g., childhood abuse, household dysfunction) on this condition. The current study examined the cumulative influence of multiple types of childhood adversities on health anxiety in adulthood. Adults 18-59 years of age (N=264) completed a battery of measures to assess adverse childhood experiences, health anxiety, and associated constructs (i.e., negative affect and trait anxiety). Significa

Adverse childhood experiences and frequent headaches in adults.

BACKGROUND: A variety of studies have linked childhood maltreatment to headaches, including migraines, and to headache severity. This study assesses the relationship of adverse childhood experiences (ACEs) to frequent headaches during adulthood. METHODS: We used data from the Adverse Childhood Experiences (ACE) study, which included 17,337 adult members of the Kaiser Health Plan in San Diego, CA who were undergoing a comprehensive preventive medical evaluation. The study assessed 8 ACEs including abuse (emotional, physical, sexual), witnessing domestic violence, growing up with mentally ill,

The association between adverse childhood experiences and childhood obesity: A systematic review.

Adverse childhood experiences (ACEs) are associated with numerous physical and mental health issues in children and adults. The effect of ACEs on development of childhood obesity is less understood. This systematic review was undertaken to synthesize the quantitative research examining the relationship between ACEs and childhood obesity. PubMed, PsycInfo, and Web of Science were searched in July 2020; Rayyan was used to screen studies, and the Newcastle-Ottawa Scale was used to assess risk of bias. The search resulted in 6,966 studies screened at title/abstract and 168 at full-text level. Twen

Associations of childhood hearing loss and adverse childhood experiences in deaf adults.

Childhood trauma and adverse childhood experiences have a strong relationship with health disparities across the lifespan. Despite experiencing approximately doubled rates of trauma, Adverse Childhood Experiences (ACEs) are poorly characterized in deaf populations. We sought to characterize deaf-specific demographic factors and their association with multiple experiences of ACEs before the age of 18 years old. An analytical cross-sectional approach was used to ascertain associations of deaf-specific demographic factors and experiences with ACEs. The complete dataset included 520 participants f

Effects of HIV and Adverse Childhood Experiences on Neurocognition.

BACKGROUND: Both HIV and adverse childhood experiences (ACEs) can affect neurocognitive performance. Among youth with perinatal HIV (PHIV), living with a chronic viral infection and experiencing ACEs simultaneously may result in an interaction between the 2 and downstream effects on neurocognition. The purpose of this study was to test the effects of PHIV, childhood adversity, and their interaction on neurocognition in a sample of South African adolescent boys. SETTING: Soweto, South Africa. METHODS: Adolescent boys aged 15-19 years enrolled in a longitudinal study between November 2020 and

The effects of adverse childhood experiences on deaf adults.

This study examined childhood adversity, exploitation-related experiences, intimate partner violence (IPV), substance use, and anxiety/depressive symptoms among deaf and hard-of-hearing adults. Data came from an anonymous online survey completed by 140 participants. Descriptive analyses showed that participants reported childhood maltreatment across multiple domains, with sexual abuse reported most frequently. More than one-quarter screened positive for IPV victimization, and a substantial proportion screened positive for elevated anxiety and/or depressive symptoms. Some participants also repo

Adults with adverse childhood experiences report greater coronavirus anxiety.

BACKGROUND: Adults with early life adversity exhibit heightened response to threat signals in the environment, which makes them vulnerable to developing stress-related mental health problems, including anxiety disorders. Yet, the impact of the COVID-19 pandemic on adults who have experienced early life adversity is understudied. Recently, researchers have characterized dysfunctional cognitions about the pandemic, which are associated with negative mental health outcomes, as coronavirus anxiety. We conducted a study to examine the relation between exposure to early life adversity, perceived thr

Adverse Childhood Experiences, Stress, and Burnout in Nursing Students.

BACKGROUND: Students may enter a nursing program with a history of trauma and chronic stressors that affect their well-being. Understanding and addressing chronic stress in nursing students may mitigate nurse burnout. This study examined correlations with adverse childhood experiences, stress, burnout, and student wellness. METHOD: Baseline measurements on well-being and stress-related topics using validated questionnaires were obtained for incoming nursing students in a 2-year baccalaureate nursing program in the Midwest. RESULTS: Nursing students (n = 272) reported more psychological flexi

Adverse Childhood Experiences and Antidepressant Therapy: A Systematic Review.

Adverse childhood experiences (ACEs) increase depression risk and may affect antidepressant (ADM) treatment outcomes; however, cumulative research on each ACE in conjunction with ADM outcomes is lacking. This review aims to examine the relationship between ACEs and ADM. We examined PsychInfo and PubMed/MEDLINE databases from 2010-2025 and identified 26 publications that met review criteria (i.e., peer-reviewed and concurrently examined ACEs and ADM). ACEs were associated with an increased propensity to use ADMs, an interaction between DNA methylation and ADM efficacy, an increased risk of suic

Racial Differences in Adverse Childhood Experiences: Timing and Patterns.

INTRODUCTION: Traditional research on adverse childhood experiences (ACEs) often retrospectively assesses ACEs and treats ACEs as a composite score, potentially overlooking the distinct effects of specific ACE patterns and their critical timing, which may influence health outcomes differently. It is crucial to explore variations in the patterns and timing of ACE exposure across racial/ethnic groups to improve ACE screening and intervention strategies. METHODS: In 2023, this study analyzed data of the Future of Families and Child Wellbeing Study (FFCWS) initially conducted in 1998. FFCWS sampl

Caregiver Adverse Childhood Experiences and Pediatric Infection-Related Outcomes.

BACKGROUND: Higher caregiver-adverse childhood experiences (ACEs) have been associated with multiple adverse pediatric outcomes. However, no studies have examined links between caregiver ACEs and infectious outcomes like antibiotic prescriptions or infection-related clinical encounters. METHODS: We conducted a retrospective cohort study including patients from 2 pediatric primary care sites, serving predominantly non-Hispanic Black, publicly insured populations. Our outcomes were antibiotic prescriptions and infection-related ambulatory clinical encounters for children 0-3 years old. We captu

The State of the Science on Adverse Childhood Experiences.

The Adverse Childhood Experiences (ACEs) study was conducted to advance understanding of psychological trauma in early life as a possible determinant of adult health. In the past decade, there has been a movement to use the ACEs research questionnaire in a variety of clinical settings to screen individuals and assess their trauma score. But critics argue that the ACEs questionnaire was never intended to be used for individual-level screening, and even that harm can be done by using the questionnaire for this purpose. In the meantime, researchers have developed a protective factor questionnaire

Adverse childhood experiences and allostatic load: A systematic review.

Adverse Childhood Experiences (ACEs) are stressful and/or traumatic experiences associated with an increased lifetime risk of negative health outcomes. The Allostatic Load (AL) is a measure of multisystem dysregulation, resulted by chronic stress. We systematically reviewed the English language literature on the association between ACEs and AL to identify the clinical risk profile, with the exclusion of reviews and preclinical studies. Searches covered the publication period up to the 1st of February 2022 and identified 25 studies in which ACEs such as maltreatment, abuse, poverty, psychologic
