Priorities for 'out-of-hours' home-based palliative care for professionals, patients, and family caregivers: A qualitative interview study.

BACKGROUND: People with advanced illness at home, and their families, rely on 'out-of-hours' services provided by community, primary and specialist palliative care services. Home is commonly expressed as the preferred place to be cared for and die, and an increasing proportion of people are dying at home, but what constitutes 'good' care is poorly understood from the combined perspectives of healthcare professionals and patients and family caregivers. OBJECTIVE: To understand the convergence and divergence of the perspectives of healthcare professionals with those of patients and family careg

Point-of-care measurement of C-reactive protein promotes de-escalation of treatment decisions and strengthens the perceived clinical confidence of physicians in out-of-hours outpatient emergency medical services.

OBJECTIVES: Out-of-hours outpatient emergency medical services (OEMS) provide healthcare for patients with non-life-threatening conditions in need for urgent care when outpatient practices are closed. We studied the use of point-of-care-testing of C-reactive protein (CRP-POCT) at OEMS. DESIGN: Cross-sectional questionnaire-based survey. SETTING: Single centre OEMS practice in Hildesheim, Germany (October 2021 to March 2022). PARTICIPANTS: OEMS physicians answering a questionnaire immediately after performing CRP-POCTs (CUBE-S Analyzer, Hitado) on any patients. PRIMARY AND SECONDARY OUTCOME

A comparison of daytime and out of hour mental health presentations to Beaumont Hospital Emergency Department between 2018 and 2020.

BACKGROUND: In 2018, there were more than 12,000 self-harm presentations to emergency departments (EDs) in Ireland with 50% occurring between 7p.m. and 3a.m. There is evidence that the assessment quality and follow-up is variable across clinicians. To address this, The National Clinical Programme for the Management of Self-Harm in the ED (NCPSH 2016) was developed to set clear standards. AIM: Our aim was to compare diagnosis and management of patients presenting to Beaumont Hospital (BH) ED, across a 3-year period, by the on-site Liaison Psychiatry Service, during normal working hours, to the

Identifying delirium in older adults presenting to a primary care out-of-hours (OOH) service: a retrospective cohort study.

INTRODUCTION: Out-of-hours (OOH) services provide urgent primary care outside normal General Practice (GP) hours, serving patients who cannot wait for routine service. Delirium is commonly associated with acute illness, causes distress and leads to poor outcomes. However, little is known about delirium presentations in OOH services. We aimed to investigate using records from an OOH service in South-West England. METHODS: The database contained 33 345 contacts with patients ≥65 years attending the OOH service. We screened consultations during April and July 2019, and January 2020 using an auto

Initiation of veno-arterial extracorporeal membrane oxygenation (VA-ECMO) for cardiogenic shock during out of hours versus working hours is not associated with increased mortality.

BACKGROUND: Initiation of veno-arterial (VA) Extracorporeal Membrane Oxygenator (ECMO) is associated with severe complications. It is unknown whether these adverse consequences occur more often after initiations during out of hours service compared to working hours. METHODS: All patients receiving VA-ECMO for cardiogenic shock between 2009 and 2020 were categorized into a working hours group (between 8 am and 5 pm on weekdays) and an out of hours service group (between 5 pm and 8 am, or between Friday 5 pm and Monday 8 am). Primary outcome was all-cause mortality at 30 days. Secondary outcome

Catheterization in the community: converting to the Foley catheter tray.

With the aim of minimizing clinical incidents and improving clinical practice, the Cardiff and Vale University Health Board carried out a trial of a Foley catheter tray to be carried by district nurses as emergency boot stock. As part of the trial all nurses had to undergo training on the Foley catheter tray prior to use. The district nurses using the tray reported improvement in confidence, efficiency, convenience and patient experience. Owing to the success of the initiative, use of the Foley catheter tray as emergency boot stock is now standard practice for District Nursing/Acute Response a

Ludwig's angina: a multidisciplinary concern.

Although relatively uncommon, Ludwig's angina is a potentially life-threatening infection of the floor of the mouth and neck. There is a danger of airway obstruction by swelling in the area and displacement of the tongue, and patients are at risk of deterioration. There are many factors thought to place patients at an increased risk of developing the condition. These include recent dental treatment, dental caries or generally poor dentition, chronic disease such as diabetes, alcoholism and malnutrition, and patients with compromised immune systems (eg AIDS, organ transplantation). The consequences of out-of-hours hip fracture surgery: insights from a retrospective nationwide study.

PURPOSE: The study aimed to investigate the association between out-of-hours surgery and postoperative mortality in hip fracture patients. Furthermore, internal fixation and arthroplasty were compared to determine if a difference could be observed in patients operated with these techniques at different times during the day. METHODS: All patients above 18 of age years in Sweden who underwent hip fracture surgery between 2008 and 2017 were eligible for inclusion. Pathological fractures, non-operatively managed fractures, or cases whose time of surgery was missing were excluded. The cohort was s

Health care assistants in out of hours community palliative care: multiple qualitative organisational case studies.

BACKGROUND: Globally, the role, title and preparation of the unregulated worker is unstandardized, yet increasing evidence suggests they play an important role in the provision of palliative care. In the United Kingdom, a Health Care Assistant (HCA) is an unregulated healthcare worker who provides support to patients under the supervision of registered professionals across clinical settings. Whilst the provision of hospice out-of-hours (OOH) community palliative care is considered a key priority internationally; little is known about the role and contribution of the HCA in the provision of suc

Vital signs of the systemic inflammatory response syndrome in adult patients with acute infections presenting in out-of-hours primary care: A cross-sectional study.

BACKGROUND: Signs of the systemic inflammatory response syndrome (SIRS) - fever (or hypothermia), tachycardia and tachypnoea - are used in the hospital setting to identify patients with possible sepsis. OBJECTIVES: To determine how frequently abnormalities in the vital signs of SIRS are present in adult out-of-hours (OOH) primary care patients with suspected infections and assess the association with acute hospital referral. METHODS: We conducted a cross-sectional study at the OOH GP cooperative in Nijmegen, the Netherlands, between August and October 2015. GPs were instructed to record the

Exploring expressed concerns and uncanny feeling in patients with shortness of breath calling out-of-hours primary care.

BACKGROUND: Patients contacting out-of-hours primary care (OHS-PC) with shortness of breath (SOB) are often concerned. Sometimes, they also have an uncanny feeling; existential anxiety that something is wrong in their body. How concerns and uncanny feeling are related to critical medical conditions that cause SOB is unknown. We therefore explored the relation between expressed concerns and researcher's judged uncanny feeling among patients who contact OHS-PC for SOB with potential life-threatening events (LTEs) as the outcome. METHODS: This is an explorative cross-sectional study. We analysed

Reasons for encounter by different levels of urgency in out-of-hours emergency primary health care in Norway: a cross sectional study.

BACKGROUND: Frequencies of reasons for encounter (RFEs) in emergency primary care out-of-hours (OOH) services are relevant for planning of capacities as well as to target the training of staff at casualty clinics. We aimed to present frequencies of RFEs in the different organ systems, and to identify the most frequent RFEs at different urgency levels. METHODS: We analyzed data on RFEs in Norwegian OOH services. International Classification of Primary Care (ICPC-2) RFE codes were recorded in all contacts to eight representative OOH casualty clinics in 2014 and 2015 covering 20 municipalities w

Is Cauda Equina Surgery Safe Out-of-Hours? A Single United Kingdom Institute Experience.

BACKGROUND: Cauda equina syndrome (CES) can have devastating neurological sequelae if surgical treatment is delayed. However, out-of-hours surgery (weekdays from 6:00 pm to 8:00 am and all weekend operations) can potentially result in higher rates of intraoperative complications, resulting in worse outcomes. In the present study, we have described our outcomes for patients with CES during an 8-year period (December 2011 to October 2019) with the aim of assessing the risk of out-of-hours surgery. METHODS: We performed a retrospective analysis of inpatient events and outcomes at 6 months of fol
