Lumen areas and homothety factor influence airway resistance in COPD.

The remodelling process of COPD may affect both airway calibre and the homothety factor, which is a constant parameter describing the reduction of airway lumen (h(d): diameter of child/parent bronchus) that might be critical because its reduction would induce a frank increase in airway resistance. Airway dimensions were obtained from CT scan images of smokers with (n=22) and without COPD (n=9), and airway resistance from plethysmography. Inspiratory airway resistance correlated to lumen area of the sixth bronchial generation of right lung, while peak expiratory flow correlated to the area of t

Hypersomnolence, insomnia and the pathophysiology of upper airway resistance syndrome.

OBJECTIVE: In order to test the hypothesis that upper airway resistance syndrome (UARS) is merely an extension of the pathophysiology of obstructive sleep apnea/hypopnea (OSA/H) to less severe pharyngeal collapse during sleep, we compared the severity of hypersomnolence and the prevalence of insomnia in UARS patients to the patterns observed for OSA/H patients. Our goal was to determine whether the severity of hypersomnolence and the prevalence of insomnia observed in UARS patients could have been predicted from the patterns observed among OSA/H patients. METHODS: We performed a retrospective

Variability of specific airway resistance in patients with laryngeal hemiplegia.

OBJECTIVES: This study was designed to analyze whether respiratory flows and specific airway resistance (sRaw) depend on the degree of breathiness and on the position of the paralyzed vocal fold in laryngeal hemiplegia. METHODS: We performed a prospective study involving 55 patients affected by laryngeal hemiplegia. RESULTS: The paralyzed fold was in an intermediate position in 18 cases and in a paramedian position in 37. Breathiness was estimated with the GRBAS scale, and the patients were divided into four groups: B0 (12 patients), B1 (14), B2 (16), and B3 (13). Spirometry was used to meas

The association of upper airway resistance with periodic limb movements.

STUDY OBJECTIVES: We hypothesized that the upper airway resistance syndrome (UARS), the component event being a respiratory effort related arousal (RERA), and periodic limb movements in sleep (PLMS), the component event being repetitive, stereotyped extremity movements occurring in a periodic fashion, were associated in certain patients. DESIGN: Invasive polysomnography using Pes and full facemask pneumotachography was used to identify RERA's in patients. Periodic limb movements (PLM) were scored according to standard criteria and as associated with RERA if the movement occurred between the P

Recognition and surgical management of the upper airway resistance syndrome.

Patients with upper airway resistance syndrome (UARS) have clinical signs and symptoms of excessive daytime somnolence (EDS) in the absence of obstructive sleep apnea. These patients have increased upper airway resistance, reflected by an elevated intrathoracic pressure measurement, despite a normal respiratory disturbance index (RDI). Physical findings often include excessive palatal tissue and narrowing of the oropharynx and hypopharynx. Nine patients with UARS who received surgical treatment were prospectively evaluated. The four men and five women had signs of EDS, with or without snoring.

Density and frequency dependence of resistance in early airway obstruction.

The forced oscillation technique is a noninvasive and effort-independent test used to characterize the mechanical impedance of the respiratory system. The aim of this study was to assess the sensitivity of this method in detecting early airway abnormalities caused by smoking or occupational hazards. Respiratory mechanical parameters by the forced oscillation technique and maximal expiratory flow volume (MEFV) curves were obtained in a group of 82 workers from a gas manufacturing plant. In addition, the variations in the frequency dependence of total respiratory resistance between air breathing

Changes in nasal airway resistance associated with rapid maxillary expansion.

Records consisting of nasal resistance measurements, postero-anterior radiographs, and dental casts were obtained on seventeen patients before they underwent rapid maxillary expansion. These records were retaken after maximum expansion of the appliance and after 3 months of retention. Measurements of nasal resistance, binasal cavity width, and maxillary first molar width were made for each subject at each stage of treatment. The following conclusions were derived: 1. Rapid maxillary expansion produced a significant reduction in nasal resistance measured at both 0.50 L. per second and 0.25 L. p

Orofacial-cervical alterations in individuals with upper airway resistance syndrome.

INTRODUCTION: Studies that assess the upper airways in sleep-related breathing disorders have been performed only in patients with obstructive sleep apnea syndrome who seek medical attention. Therefore, in addition to the need for population studies, there are no data on the orofacial-cervical physical examination in subjects with upper airway resistance syndrome. OBJECTIVES: To compare the orofacial-cervical examination between volunteers with upper airway resistance syndrome and without sleep-related breathing disorders. METHODS: Through questionnaires, physical measurements, polysomnograp

A passive quantitative measurement of airway resistance using depth data.

The Respiratory Syncytial Virus (RSV) is the most common cause of serious lower respiratory tract infections in infants and young children. RSV often causes increased airway resistance, clinically detected as wheezing by chest auscultation. In this disease, expiratory flows are significantly reduced due to the high resistance in patient's airway passages. A quantitative method for measuring resistance can have a great benefit to diagnosis and management of children with RSV infections as well as with other lung diseases. Airway resistance is defined as the lung pressure divided by the airflow.

Evaluation of Airway Resistance in Children with Juvenile Idiopathic Arthritis.

Background: Pneumonitis, pleuritis, and pulmonary interstitial infiltration have been described in patients with juvenile idiopathic arthritis (JIA). However, the pulmonary involvement of JIA is not often clinically apparent. There are few studies based on pulmonary function in children having only a diagnosis of JIA. The aim of the present study is to determine whether children with JIA have airway resistance and flow impairments measured by easily applied interrupter technique. Method: We performed interrupter resistance (Rint) measurements in children with JIA and in healthy control subject

Vitamin A deficiency increases airway resistance following C-fiber stimulation.

Vitamin A deficiency (VAD) remains an important problem in the developing world where environmental air pollution is increasing. Because the coincidence of these factors could increase the prevalence of asthma in these regions, airway reactivity to methacholine (MCh), capsaicin, and sulfur dioxide was studied in VAD rats. The impedance to a small volume oscillation was analyzed to determine airway resistance (R(aw)) and lung elastance (H). VAD rats demonstrated larger increases in R(aw) and H after the administration of methacholine, and in R(aw) after administration of capsaicin or sulfur dio

Technique for assessing nasal airway resistance in patients treated prosthetically.

This investigation was designed to compare nasal airway resistance (Rnaw) values by use of "head-out" plethysmography and standard posterior pressure-flow rhinometry. Fourteen adults who had prostheses because of velopharyngeal inadequacy caused by a variety of etiologic conditions were examined. Each patient performed rest breathing and sustained /m/ productions to develop Rnaw values for comparison. The results revealed increased Rnaw and decreased cross-sectional nasal airway area and derived velopharyngeal orifice area when the appliances were in place. The two techniques for assessing Rna

Compensatory mechanisms induced by high oropharyngeal airway resistance in rats.

Acid base balance changes were observed during 72 h following bilateral nasal obstruction in rats. Mouth breathing caused acute respiratory acidosis and marked aerophagia, leading to spontaneous death of the experimental animal 80 to 90 h postoperatively. Stenotic oropharyngeal airway, due to palatal-epiglottic approximation, is supposed to be responsible for respiratory insufficiency in the nose obstructed rats. The compensatory changes in respiratory mechanics caused by high oropharyngeal airway resistance, together with some possible reflex changes, may have caused either air swallowing or

The effect of rapid maxillary expansion on nasal airway resistance.

The purpose of this study was to evaluate changes in nasal resistance to airflow in persons undergoing rapid maxillary expansion and to reevaluate the responses at a 1-year follow-up. Nasal resistance measurements, assessed in four modes (natural state, anterior nares dilation with Tygon tubing, following administration of decongestant, and nares dilation with tubing and decongestant), were taken on a group of 38 patients receiving rapid maxillary expansion and compared with a control group not receiving expansion. Thirty-three of the patients were reevaluated 9 to 12 months after expansion wa

Spontaneous Breathing Through Increased Airway Resistance Augments Elastase-Induced Pulmonary Emphysema.

INTRODUCTION: Resistive breathing (RB), the pathophysiologic hallmark of chronic obstructive pulmonary disease (COPD), especially during exacerbations, is associated with significant inflammation and mechanical stress on the lung. Mechanical forces are implicated in the progression of emphysema that is a major pathologic feature of COPD. We hypothesized that resistive breathing exacerbates emphysema. METHODS: C57BL/6 mice were exposed to 0.75 units of pancreatic porcine elastase intratracheally to develop emphysema. Resistive breathing was applied by suturing a nylon band around the trachea t
