Unique occupational hazards of Alaska: animal-related injuries.

OBJECTIVE: During 1992-2000, an average of 40 fatal occupational injuries and 12,400 nonfatal occupational injuries and illnesses related to animals were recorded each year in the United States, most involving domestic farm animals. Although Alaska has a relatively small farming industry, it supports several industries that require workers to regularly be in contact with animals. This study examines the pattern and characteristics of animal-related occupational injuries in Alaska. METHODS: Two data sources were accessed: the Alaska Trauma Registry for nonfatal injuries requiring hospitalizati

Prevalence of autoimmune liver disease in Alaska Natives.

OBJECTIVE: There is limited information on the prevalence of autoimmune liver disease in nonwhite populations. We conducted a population-based study on the prevalence of autoimmune liver diseases in Alaska natives. METHODS: Clinical records from 1984 to July, 2000 were reviewed to identify Alaska natives with autoimmune hepatitis (AIH), primary biliary cirrhosis (PBC), primary sclerosing cholangitis, autoimmune cholangitis, and overlap syndromes of two of the above. AIH was defined as definite or probable, based on criteria established by the International Autoimmune Hepatitis Group. The diag

Drinking and drinking-related problems among Alaska natives.

Alcohol use has adversely affected many aspects of the Alaska Native community. To a large extent, overcoming the problem of alcohol abuse may require that Alaska Natives craft individual and community solutions to detrimental health, social, and economic conditions and instill new patterns of living that inhibit alcohol abuse. An example of this approach is the Alaska Federation of Natives' "sobriety movement," a grassroots campaign to promote sobriety that emphasizes traditional values and lifestyles. The use of "healing" or other traditional methods may help Alaska Natives both recover from

Alaska Native cancer update: incidence rates 1989-1993.

Five-year average annual age-adjusted cancer incidence rates for Alaska Natives (Eskimos, Indians, and Aleuts) for the most recent period (1989-1993) are compared to rates of 20 years earlier. Rates for all cancers combined increased 28 and 25% in men and women, respectively, during the 25-year interval. Increases were seen in men in cancers of the lung, prostate, and colon and in women for cancers of the lung, breast, and corpus uteri. Rates are also compared to data from the the National Cancer Institute's Surveillance, Epidemiology, and End Results (SEER) program for United States whites. R

Factors Associated with Vaccine Hesitancy in the State of Alaska.

BACKGROUND: Vaccine hesitancy (VH) is a major public health problem which includes not only concerns about vaccine, but often includes beliefs not aligned with evidence. The etiology of VH is complicate, including genuine concerns about vaccine safety to beliefs in disproven or unsubstantiated theories. Understanding VH includes determining the factors associated with people most likely to be susceptible. OBJECTIVES: To identify characteristics of persons more likely to have VH based on perceived vaccine safety. DESIGN: We conducted a cross sectional, targeted online survey of 1,024 responde

Rabies in Alaska, from the past to an uncertain future.

Rabies is a serious zoonotic disease with significant public health consequences in the circumpolar North. Recent studies have advanced our understanding of the disease ecology in Alaska. In this paper, we review historical records of rabies in Alaska ranging from the late nineteenth century to the present, analyse the public health impact in the state and review studies on disease ecology before assessing challenges and anticipated altered disease dynamics in the face of a rapidly changing North. Rabies is a disease that has been present in Alaska continuously for over 100 years. It is mainta

Mercury in fishes of Alaska, with emphasis on subsistence species.

In the north, the presence of mercury (Hg) in food leading to chronic exposure is a scientific, economic and political issue. Guidelines have been established for the safe consumption of fish containing Hg, however, adherence to these guidelines must be weighed against the health benefits of consuming fish, such as from the omega-3 polyunsaturated fatty acids, vitamins and minerals. Alaskan Natives generally consume much more fish than the national average. This review summarizes and synthesizes the significant amount of data that has been generated on Hg in Alaska fish, particularly those con

Clinical outcomes for newborn intensive care in Alaska.

Recent clinical outcomes for the Newborn Intensive Care Unit (NICU) at Providence Alaska Medical Center based on Alaska Neonatology's Clinical Outcomes Database are presented. There has been a decrease in overall mortality, with much of the improvement occurring in babies from 22 to 25 weeks gestation in the years 1998--2002. There has also been a decrease in the incidence of severe intraventricular hemorrhage / periventricular leukomalacia. Earlier discharge of babies has also been documented, as measured by post conceptual age at discharge. No improvement in the incidence of retinopathy of p

Mortality of American Indian and Alaska native infants.

Accurate determination of infant mortality rates among Indians in seriously hampered by variations in the identification of Indian persons and use of different subsets of the Indian population for various purposes. Lack of consistency in the reporting of racial origin on birth and death records is a source of substantial error. Because of these factors, more than the usual care must attend comparisons and inferences drawn from data in which these differences are present. At present, it would seem prudent to regard all data about American Indians as provisional. Even though Indian infant mortal

Epidemiology of hepatitis B in two Alaska communities.

In 1973, epidemiologic and serologic data related to hepatitis B infection were collected from the residents of two remote Alaskan Eskimo villages located in an area of high hepatitis incidence. A total of 418 sera were tested by solid-phase radioimmunoassay for heaptitis B surface antigen (HBsAg) and antibody to that antigen (anti-HBs). The overall infection prevalence of 54.8% in the two villages included a 13.9% prevalence of HBsAg and a 40.9% prevalence of anti-HBs. Families containing an individual with HBsAg had significantly higher infection prevalence than those without an antigen carr

Increase in Disseminated Gonococcal Infections - Alaska, 2023-2024.

Disseminated gonococcal infection (DGI) is a rare but serious complication of Neisseria gonorrhoeae infection that can result in severe illness or death. In April 2024, the Alaska Section of Epidemiology (SOE) identified an increase in suspected DGI cases in Anchorage, Alaska, prompting enhanced surveillance. The state's gonorrhea surveillance system and electronic health records were used to review all likely and verified DGI cases reported to SOE during January 2023-December 2024. Thirty-five DGI cases were identified, accounting for 0.35% (eight of 2,289) of all gonorrhea cases in 2023 and

Increasing colorectal cancer screening among Alaska Native peoples living in remote areas of Alaska: a multitarget stool DNA cluster randomized controlled trial.

BACKGROUND: Increased colorectal cancer (CRC) screening is needed to address high Alaska Native (AN) CRC rates. The study objective was to evaluate high-intensity vs. medium-intensity patient outreach using multi-target stool DNA (mt-sDNA) or colonoscopy. METHODS: A community-level, cluster-randomized clinical trial (April 29, 2021, through September 15, 2024) in 46 rural/remote Alaska communities. Participants were asymptomatic AN adults aged 45-75 with no family or personal history of CRC who were due for screening. High-intensity: Up to six patient navigator telephone calls with follow-up

Coccidioidomycosis Among American Indians and Alaska Natives, 2001-2014.

BACKGROUND: American Indians and Alaska Natives (AI/ANs) may be uniquely vulnerable to coccidioidomycosis given the large population residing in the Southwestern United States. We describe coccidioidomycosis-associated hospitalizations and outpatient visits during 2001-2014 in the Indian Health Service (IHS) system and compare hospitalizations with data from the Agency for Healthcare Research and Quality's National (Nationwide) Inpatient Sample (NIS). METHODS: We identified hospitalizations in the IHS and the NIS and outpatient visits in the IHS using International Classification of Diseases,

Heart disease mortality among Alaska Native people, 1981-2007.

OBJECTIVES: Historically, Alaska Native (AN) people have exhibited low overall rates of heart disease mortality compared with the U.S. white (USW) population. We compared AN and USW heart disease mortality rates during the 27-year period from 1981 through 2007. METHODS: We compared AN and USW heart disease mortality rates overall and by gender, age, and disease subtype. We calculated age-adjusted rates for AN people for three nine-year periods from 1981 through 2007 and compared them with the rates for USW people. RESULTS: AN people > or = 35 years of age had a significantly lower rate of he
