Participant ascertainment is differentially related to phenotypic characteristics and alcohol-related genetic liability in a sample with severe alcohol use disorder.

BACKGROUND: Alcohol use disorder (AUD) is a common substance use disorder associated with a range of sociodemographic, behavioral, and genetic factors. The current study characterizes variation in such factors as a function of ascertainment strategy in a sample of individuals with a lifetime history of severe AUD. METHODS: Participants (N = 10,804) were recruited through substance use treatment facilities or through online outreach/advertisement in the United States and completed a survey that assessed a range of alcohol-related variables, sociodemographics, psychopathology, and personality.

Prazosin-Related Reductions in Daily Craving Mediate Treatment-Related Daily Alcohol Use Outcomes in Alcohol Use Disorder.

BACKGROUND: Prazosin has been shown to reduce drinking, particularly in individuals with alcohol withdrawal symptoms, possibly due to reducing craving as shown in experimental studies. However, it is unclear whether prazosin decreases daily changes in alcohol craving and whether such changes drive prazosin-related changes in alcohol intake in the treatment of alcohol use disorder (AUD). METHODS: Using an intensive daily-level longitudinal design embedded within a 12-week, double-blind, placebo-controlled, randomized clinical trial of prazosin (16 mg/day) versus placebo (PBO) for the treatment

Xenon gas as a potential treatment for opioid use disorder, alcohol use disorder, and related disorders.

Xenon gas is considered to be a safe anesthetic and imaging agent. Research on its other potentially beneficial effects suggests that xenon may have broad efficacy for treating health disorders. A number of reviews on xenon applications have been published, but none have focused on substance use disorders. Accordingly, we review xenon effects and targets relevant to the treatment of substance use disorders, with a focus on opioid use disorder and alcohol use disorder. We report that xenon inhaled at subsedative concentrations inhibits conditioned memory reconsolidation and opioid withdrawal sy

Self-medication of mood disorders with alcohol and drugs in the National Epidemiologic Survey on Alcohol and Related Conditions.

BACKGROUND: Using alcohol or drugs to reduce emotional distress (self-medication) has been proposed as an explanation for the high comorbidity rates between anxiety and substance use disorders. Self-medication has been minimally studied in mood disorders despite equally high rates of alcohol and drug use. METHODS: Data came from the National Epidemiologic Survey on Alcohol and Related Conditions (NESARC), a large (n=43,093, age 18 years and older) nationally representative survey of mental illness in community-dwelling adults. Prevalence rates of self-medication were determined for DSM-IV moo

A relational model of wellbeing-centered recovery in alcohol use disorder.

Wellbeing is a key element among the prevailing definitions of recovery from alcohol use disorder. However, there is an increasing need to develop a unified framework for recovery conceptualization and characterization of its components, as well as the mechanisms to achieve it. The objective of the study was to test a model relating the dimensions involved in recovery. A total of 348 participants with different periods of abstinence (range:1 month-28 years) were assessed using self-reports of psychological well-being, quality of life, negative emotionality and coping strategies. Statistical an

Age-related cognitive decline is accelerated in alcohol use disorder.

This study aimed to examine potential cognitive impairments in patients with alcohol use disorder (AUD), and explore the factors affecting them. We recruited 97 inpatients with AUD, showing superficially normal cognitive function (mini-mental state examination score ≥24) for this study. We assessed cognitive function after a 4-week post-abstinence period using the Brief Assessment of Cognition in Schizophrenia-Japanese version (BACS-J). Relationships between BACS-J subcategory/composite raw scores and Z-scores (deviation from standard data in healthy Japanese) and background factors such as ag

Impulse-control disorders in alcoholics are related to sensation seeking and not to impulsivity.

Impulse-control disorders (ICD) include intermittent explosive disorder, kleptomania, trichotillomania, pyromania and pathological gambling. Several studies have suggested that the incidence of pathological gambling and impulsive violent behavior is substantially higher in alcohol-dependent patients than in the general population. The association between ICD and alcoholism, as well as personality characteristics such as sensation seeking and impulsivity, has never been systematically studied. The present study compared the levels of impulsivity and sensation seeking in age- and sex-matched gro

Alcohol use disorder accelerates age-related cognitive decline.

BACKGROUND: Alcohol use disorder (AUD) is an established contributor to neurocognitive impairment and a major modifiable risk factor for dementia. However, the extent to which lifetime AUD interacts with aging to accelerate subtle cognitive trajectories prior to overt clinical dementia remains unclear. OBJECTIVE: To investigate whether a lifetime history of DSM-5 AUD and correlated indices of alcohol consumption are associated with accelerated age-related cognitive decline in middle-aged and older adults. METHODS: Participants (N = 1,421) were drawn from the Collaborative Study on the Geneti

Impact of bariatric surgery on alcohol-use disorder and alcohol-related liver disease: A retrospective study.

BACKGROUND AND AIMS: Increased risk of alcohol dependence is recognised following bariatric surgery. Modified gastrointestinal anatomy and neurohormonal profiles lead to altered alcohol metabolism, peak blood alcohol concentrations and enhanced reward circuits and cravings. For patients with metabolic associated fatty liver disease, bariatric surgery has been shown to reduce liver fibrosis; however, the impact of bariatric surgery on alcohol consumption and consequent liver fibrosis has not been well-characterised using validated measures. This study examined the changes in alcohol consumption

Genetic and environmental influences on the progression from alcohol use disorder to alcohol-related medical conditions.

BACKGROUND: Medical conditions related to alcohol use disorders (AUD) represent a substantial public health concern. However, only a subset of individuals with AUD develop these conditions and the extent to which genetic and environmental factors that are shared with AUD, versus those distinct from it, contribute to this progression has not yet been determined. METHODS: Using data from Swedish national registries for a cohort born from 1932 to 1970 (N = 1,319,214, 48.9% women), we conducted twin-sibling biometric model fitting to examine the genetic and environmental sources of variance that

The comorbidity of alcoholism and personality disorders in a clinical population: prevalence rates and relation to alcohol typology variables.

This study assessed prevalence rates and overlap among Diagnostic and Statistical Manual of Mental Disorders (3rd ed., revised; DSM-III-R; American Psychiatric Association, 1987) personality disorders in a multisite sample of 366 substance abusers in treatment. In addition, the relation of antisocial personality disorder (APD), borderline personality disorder (BPD), and paranoid personality disorder (PPD) to alcohol typology variables was examined. Structured diagnostic interviews and other measures were administered to participants at least 14 days after entry into treatment. Results indicate

The frequency of alcoholism and alcohol related brain disorders in various regions of Oslo city. An autopsy study.

The autopsy materials from the 5-year period 1983-1987 from Ullevål and Aker Hospitals in Oslo were reviewed. All cases with alcoholism were recorded and the histological sections were re-examined with regard to the presence of Wernicke's encephalopathy and alcoholic cerebellar atrophy. The two materials were compared statistically in order to see whether any difference in the frequency of alcoholism and alcohol related brain disorders could be demonstrated between the geographic areas served by these two hospitals. The study revealed a significantly higher percentage of alcoholics in the Aker

Hospital Readmission and Mortality Among Alcohol-related Cancer Survivors With an Alcohol Use Disorder Diagnosis.

OBJECTIVES: Unhealthy alcohol use, which includes alcohol use disorder (AUD), is common among cancer survivors and can lead to negative health outcomes. Prior research found that an AUD diagnosis is associated with hospital readmission up to 180 days after discharge among alcohol-related cancer survivors. This study investigates whether AUD diagnosis is associated with hospital readmission 30 and 90 days after discharge and in-hospital mortality. METHODS: We conducted a retrospective cohort study using the all-payer Nationwide Readmissions Database (NRD) from 2017 to 2020. We identified hospi

Utilisation of alcohol-related treatment after a first alcohol use disorder diagnosis in Hamburg, Germany.

BACKGROUND: A variety of treatment options for people with alcohol use disorder (AUD) exist. Surveys estimate that 1 in 10 people with AUD utilise treatment, but real-world treatment pathways remain covert. This data-linkage study seeks to characterise treatment utilisation patterns to identify gaps in treatment access and delivery in Germany. METHODS: Linking individual-level data from three sources (statutory health insurance, pension funds, outpatient addiction care services) identified seven alcohol-related treatment types delivered in outpatient (brief psychiatric consultation; formal ps

The prominence of smoking-related mortality among individuals with alcohol- or drug-use disorders.

INTRODUCTION AND AIMS: Even though individuals with substance-use disorders have a high prevalence of tobacco smoking, surprisingly little is known about smoking-related mortality in these populations. The current retrospective cohort study aims to address this gap. DESIGN AND METHODS: The study sample included cohorts of individuals hospitalised in California between 1990 and 2005 with alcohol- (n = 509 422), cocaine- (n = 35 276), opioid- (n = 53 172), marijuana- (n = 15 995) or methamphetamine-use (n = 36 717) disorders. Executive functioning in children with Fetal Alcohol Spectrum Disorders: profiles and age-related differences.

The goal of this project was to examine the profile of executive function (EF) deficits and age-related differences among children with FASD. Twenty-nine children with FASD (8 to 16 years of age) completed 8 tests from the Delis-Kaplan Executive Function System (D-KEFS). They had difficulty on many components of EF including cognitive flexibility, inhibition, some measures of verbal fluency, abstract thinking, deductive reasoning, hypothesis testing, problem solving, and concept formation. A distinctive profile emerged with performance being poorest on the card sorting test and relatively high

Drinking motives as mediators in the relation between personality disorder symptoms and alcohol use disorder.

Research shows high comorbidity between Cluster B Personality Disorders (PDs) and Alcohol Use Disorders (AUDs). Studies of personality traits and alcohol use have identified coping and enhancement drinking motives as mediators of the relation among impulsivity, negative affectivity or affectivity instability, and alcohol use. To the extent that certain PDs reflect extreme expression of these traits, drinking motives were hypothesized to mediate the relation between PD symptoms and presence/absence of an alcohol use disorder (AUD). This hypothesis was tested using a series of cross-sectional an
