The Marriage and Divorce of Little Willy: Psychoanalysis and Addiction.

Psychoanalysis can't treat addicts; Alcoholics Anonymous can't treat neurotics. Contrasting the two proffered therapeutics, they are obviously opposed as a rich discourse and a banality of letters or as individual and group therapy. And yet they emerged as one confused tangle in the cocaine addiction of Sigmund Freud. Finally, both have been condemned as bourgeois constructions, as the discourse of capitalism.

AA, Bill Wilson, Carl Jung and LSD.

Alcoholics Anonymous (AA) is an established resource for people suffering from alcohol use disorder (AUD). However, Bill Wilson, the co-founder of AA, in his second letter to Jung referred to its low success rate. One evidence-based alternative, dating back to the 1950s, is the clinical use of lysergic acid diethylamide (LSD) for treating AUD. Bill Wilson was a strong advocate of using LSD as a preparation for alcoholics who had difficulty grasping the spiritual aspect of the 12-step programme. Bill Wilson wrote a "secret" four-page letter to Carl Jung detailing his own use of LSD and the succ

Chronic opioid therapy for nonmalignant pain in patients with a history of substance abuse: report of 20 cases.

A history of substance abuse is considered by many to be a contraindication to chronic opioid therapy for nonmalignant pain. Twenty patients with a history of chronic nonmalignant pain and substance abuse treated with chronic opioid therapy for a period of more than 1 year were retrospectively evaluated to determine the factors associated with prescription abuse. The prevalence of six aberrant behavioral patterns was assessed to see if these correlated with a history of prescription abuse, as reported by the patient's pain clinic physician. Those who did not abuse opioid therapy were more like

Combining psychosocial treatment with pharmacotherapy for alcohol dependence.

Pharmacotherapy for alcohol dependence is always delivered in a psychosocial context that may affect the outcome of the treatment. The rigorous study of different psychotherapeutic treatments for alcohol dependence has shown several distinct approaches to be effective. This article reviews the combination of alcohol dependence pharmacotherapies, including disulfiram, naltrexone, and acamprosate, with different psychosocial interventions. Many psychosocial interventions for alcohol dependence, including Alcoholics Anonymous, can be integrated successfully with pharmacotherapy. Psychosocial inte

Spirituality, Religiosity and Addiction Recovery: Current Perspectives.

Substance use disorders are an important public health problem with a multifactorial etiology and limited effective treatment options. Within this context, spirituality-based approaches may provide interesting and useful options in managing substance use disorders. This kind of intervention can have positive effects in alleviating some core symptoms associated with substance use, such as aggressiveness. Improvement in cessation rates for alcohol, cocaine and opioid use disorders have also been described in some clinical studies. However, spirituality may not play a beneficial role in some subg

Recent development in alcoholism:sociology.

This chapter reviews several domains of sociological research conducted in the field of alcohol studies during the last decade. The domains are organized within the sociostructural, social constructionist, and distribution of consumption theoretical perspectives. Discussions of the current state of the field and definitional and measurement issues are also included. Within the sociostructural perspective, sociocultural theories, social deviance theories, age, gender, ethnic, and occupational group differences, and other related deviant behaviors are examined. Several examples of applications o

Comorbid alcoholism and depression: treatment issues.

Unless there is decisive professional intervention, people who suffer from both a depressive disorder and alcoholism are at great risk of chronic impairment, both at home and in the workplace; persistent symptomatic misery; and premature death. Untreated alcoholism intensifies depressive states, decreases responsiveness to conventional therapeutics, and increases the likelihood of suicide, suicide attempts, and other self-destructive behavior. During the past decade, evidence has emerged from placebo-controlled studies supporting the utility of tricyclic antidepressants and selective serotonin

Predictors of concerned others' Al-Anon attendance and involvement following adults' entry into treatment for an alcohol use disorder.

BACKGROUND: Alcohol use disorders (AUDs) negatively impact the health of persons consuming alcohol and their concerned others (COs, their family, and friends). It is important to identify factors that affect COs' likelihood of participating in supportive services, such as Al-Anon, that can improve their outcomes. We used the Stress-Strain-Coping-Support model to identify potential predictors of Al-Anon attendance and involvement among COs of adults entering treatment for an AUD. METHODS: Participants were 279 dyads of patients entering residential treatment for AUD and their CO. Outcomes were

Factors associated with integration of people recovering from alcohol dependence syndrome with their family: An exploratory study.

BACKGROUND: Alcohol is the most commonly consumed psycho-active substance in India. People who use substances and are in recovery often face societal discrimination and rejection, such as difficulty in obtaining employment, poor support for treatment, and inter-personal rejection. AIM: To find out the factors affecting the integration of people recovering from alcohol dependence syndrome with their families. MATERIALS AND METHODS: A cross-sectional observational study was conducted using a purposive sampling technique among 50 pairs of alcohol-dependent people and their family members. Data

A meta-review of the evidence on addiction recovery mutual help organizations.

BACKGROUND: Mutual help organizations (MHOs) for alcohol and substance use disorders are peer-based programs offering recovery-specific social support. A wide-ranging, diverse body of published MHO literature examines their clinical and public health utility. We conducted a meta-review ("overview of reviews") to synthesize this evidence and identify trends, limitations, and gaps across existing syntheses. METHODS: We searched five electronic databases (August 2023) using tailored search strings to identify reviews of MHOs. We coded review characteristics and quality ratings (A Measurement Too

Twelve-step facilitation in non-specialty settings.

Participation in the twelve-step mutual-help organization, Alcoholics Anonymous, has proven to be an effective means of helping individuals with alcohol dependence achieve lasting sobriety. Although many patients choose to attend AA of their own accord, clinicians' facilitation of AA involvement ("Twelve-Step Facilitation" [TSF]) has shown to substantially increase the likelihood that patients will become engaged with these freely available resources. Importantly, many individuals with alcohol dependence never seek help from addiction specialists, yet often encounter other health professionals

Effects of long-term AA attendance and spirituality on the course of depressive symptoms in individuals with alcohol use disorder.

Alcohol use disorder (AUD) is associated with depression. Although attendance at Alcoholics Anonymous (AA) meetings predicts reductions in drinking, results have been mixed about the salutary effects of AA on reducing depressive symptoms. In this single-group study, early AA affiliates (n = 253) were recruited, consented, and assessed at baseline, 3, 6, 9, 12, 18, and 24 months. Lagged growth models were used to investigate the predictive effect of AA attendance on depression, controlling for concurrent drinking and treatment attendance. Depression was measured using the Beck Depression Invent

Addiction careers and careers in addiction.

Research into resocialization of criminals or into recovery from substance dependence has stressed the need to give up the deviant identity and lifestyle. However, addiction careers, crime, and other types of deviant behavior require a wide range of skills. Such competence may facilitate the alternative or simultaneous pursuit of "respectable" careers in treatment, prevention, research, or policy matters. Former alcoholics fill important positions in the service structures of Alcoholics Anonymous, current or former users are appreciated as partners in AIDS prevention programs, and drug addicts

General aspects of the treatment of alcoholic hepatitis.

General measures for treating patients with alcoholic hepatitis (AH) are similar irrespective of the disease severity. Alcohol abstinence is the cornerstone of treatment and can be achieved with appropriate social support, Alcoholics Anonymous and sometimes pharmacological therapy. Alcohol withdrawal should be anticipated and treatment initiated to prevent this complication. Treatment for complications of cirrhosis should be as for any other patient with cirrhosis. AH patients are particularly prone to infections and malnutrition. These should be identified and treated appropriately using broa

Benefits and costs of alcoholic relationships and recovery through Al-Anon.

BACKGROUND: Codependence is an ambiguous and disputed term often used to characterize both those who maintain relationships with alcoholics and those who seek help through resources such as Al-Anon Family Groups. OBJECTIVES: The purpose of this article is to better understand non-pathological reasons for maintaining alcoholic relationships and for help-seeking by detailing the costs and benefits of those choices. METHODS: The costs and benefits both of remaining in an alcoholic relationship and of seeking help in Al-Anon were identified through a review of available research on alcoholic fam

Altered states of consciousness therapy. A missing component in alcohol and drug rehabilitation treatment.

Attaining altered states of consciousness is described as a basic human motive. The substance dependent population is distinguished from other populations because they pursue these states destructively by inappropriate use of alcohol and drugs. Despite a body of literature supporting the benefits of altered states of consciousness, alcohol and drug rehabilitation treatment programs fail to address this motive because of social disapproval, means-end confusion, and inadequate staff training. The authors maintain that Alcoholics Anonymous directs its members toward an altered state of consciousn

The intoxicated child.

Psychiatric disorders and substance abuse occur frequently together, which leads to difficulty in assessment and treatment. An awareness of the prevalence and manifestations of psychiatric diagnoses is essential for the quality treatment of adolescents who abuse substances. The clinician should keep up-to-date on psychopharmacologic interventions [65, 83]. Frequently, the use of psychiatric medications, such as antidepressants, mood stabilizers, and psychostimulants, is beneficial. Care must be taken, however, to avoid potential interactions between the illicit drugs and the prescribed medicat

Alcoholism treatment in the United States. An overview.

On any given day, more than 700,000 people in the United States receive alcoholism treatment in either inpatient or outpatient settings. For many of those patients, detoxification--with or without pharmacotherapy--is the first step of treatment. The major behavioral approaches currently used in alcoholism treatment include cognitive-behavioral therapy, motivational enhancement therapy, and Alcoholics Anonymous (AA) or related 12-step programs. Clinical studies, such as the Project MATCH trial, have compared the effectiveness of these approaches. Overall, that study detected no significant diff
