Lip Augmentation with Autografts and Allografts.

Lip augmentation is a popular cosmetic procedure aimed at enhancing lip volume, symmetry, and contour, which significantly influences overall facial aesthetics.Historically, lip enhancement dates back centuries, with various cultures emphasizing the allure of fuller lips. Lip Augmentation with alloplastic implants like silicone and Gore-Tex was favored for a time for their versatility and consistency, but modern practice has increasingly favored biologically derived implants due to their natural integration, reduced complications, and desirable texture. Autologous tissues, such as superficial

Lymphocytic Airway Inflammation in Lung Allografts.

Lung transplant remains a key therapeutic option for patients with end stage lung disease but short- and long-term survival lag other solid organ transplants. Early ischemia-reperfusion injury in the form of primary graft dysfunction (PGD) and acute cellular rejection are risk factors for chronic lung allograft dysfunction (CLAD), a syndrome of airway and parenchymal fibrosis that is the major barrier to long term survival. An increasing body of research suggests lymphocytic airway inflammation plays a significant role in these important clinical syndromes. Cytotoxic T cells are observed in ai

Chronic rejection of human face allografts.

Face vascularized composite allografts (FVCAs) have helped patients with severe facial disfigurement, with acute rejection now largely controlled through iatrogenic immunosuppression. However, little is known regarding the incidence and mechanism(s) of more long-term pathologic alterations in FVCAs that may affect function and graft durability. Protocol surveillance biopsy specimens for up to an 8-year interval in 7 patients who received FVCAs at our institution revealed histopathologic evidence of chronic rejection. Clinical manifestations included features of premature aging, mottled leukode

Evaluation of pancreatic allografts with sonography.

OBJECTIVES: The purposes of this study were to develop a protocol for evaluating pancreas allografts, to describe a method for successfully studying pancreatic transplants, and to determine whether the resistive index (RI) of the splenic artery is a useful differentiator between complications. METHODS: We retrospectively analyzed clinical, surgical, procedural, and radiologic reports in 51 consecutive patients undergoing 182 sonographic examinations during a 4.5-year period. Complications included splenic vein thrombosis, rejection, and pancreatitis. We obtained RIs in normal and complication

Medullary ray injury in renal allografts.

Non-immune injury leading to interstitial fibrosis and tubular atrophy (IF/TA) in renal allografts has various etiologies, but pathological means of verification have yet to be developed. Medullary ray injury (MRI) is a pathological feature of many non-immune injuries inducing IF/TA and pathological determination of calcineurin inhibitor (CNI) toxicity proceeding to striped fibrosis. We investigated the contribution of CNI toxicity to MRI and other non-immune etiologies related to IF/TA. In this study MRI is defined as fibrosis and inflammation localized exclusively to the medullary ray. Thirt

Rejection and acceptance of corneal allografts.

PURPOSE OF REVIEW: Corneal transplantation is successful in the short-term, but the long-term prognosis has not improved over the past 20 years. Here, we review recent findings that may contribute to improved corneal allograft survival. RECENT FINDINGS: A better understanding of the molecular pathways affecting corneal graft survival has led to more targeted approaches to immune modulation. Costimulatory molecule blockade, inhibition of chemokine-chemokine receptor interactions, modulation of apoptotic pathways, and reduction of corneal neovascularization and lymphangiogenesis have been shown

Chronic rejection of human kidney allografts.

Human chronic rejection is an uncommon feature of chronic allograft nephropathy, the major cause of late graft loss in kidney transplantation. Chronic rejection is a complex process, mediated, at least in part, by immunological mechanisms. T cells with donor, tissue or pathogen specificities that react with donor cells damage the graft tissue in synergy with antibodies. The cellular and humoral components of the immune system may be preformed or appear at both early and late time points following kidney transplantation. Both slowly contribute to the development of vasculopathy, glomerulopathy,

Clinical report on stentless mitral allografts.

Three patients aged 43, 64 and 54 years (two females and one male) underwent allograft mitral valve replacement including the subvalvular apparatus in 1984 at the Department of Cardiovascular Surgery, University of Kiel. Transpapillary epicardial suture technique was used to support the graft-native papillary muscle union site. Early postoperative course was uneventful in all three patients. There was no early or late postoperative death. Trivial mitral regurgitation was recorded by color Doppler echocardiography but it was not progressive. The regurgitant volume measured at the postoperative

Venous allografts for critical limb ischaemia.

In as many as 25% of the patients with critical lower limb ischaemia, a suitable autogenous vein graft is not available. Patency rate of prosthetic grafts is disappointing at the infrageniculate level. Recently, there is renewed interest in the use of preserved venous allografts as an alternate conduit in patients with threatened ischaemic limbs. From 1991 to 1993, the authors revascularized 12 ischaemic limbs with venous allografts (10 preserved at 4 degrees C, 2 Varivas grafts). The indication for operation was rest pain in five patients and tissue necrosis in seven. Nine patients had one or

Fresh and frozen articular cartilage allografts.

Fresh and frozen cartilage allografts both function well to relieve joint pain due to cartilage injury. Fresh cartilage allografts appear to remain alive for many months and are most applicable to small injuries such as osteochondritis dissecans, traumatic defects, and osteonecrosis. Frozen cartilage allografts have been used mostly for treatment of bone tumors which most often involve the subchondral and the metaphyseal areas of the long bones, particularly around the knee and hip. Frozen cartilage allografts appear to survive well and maintain joint space for several years. Deterioration of

Natural history of autografts and allografts.

The clinical outcome of bone grafting procedures depends on many factors, including type and fixation of the bone graft as well as the site and status of the host bed. Bone grafts serve one or both of two main functions, as a source of osteogenetic cells and as a mechanical support. Autografts, both cancellous and cortical, are usually implanted fresh and are often osteogenetic, whether by providing a source of osteoprogenitor cells or by being osteoinductive. The latter is a process whereby the transplanted tissue induces mesenchymal cells of the recipient to differentiate into osteoblastic c

Clinical performance of decellularized cryopreserved valved allografts compared with standard allografts in the right ventricular outflow tract.

BACKGROUND: Although decellularized cryopreserved valved allografts (DCAs) have reduced immunogenicity, proof of clinical superiority over standard cryopreserved allografts (SCAs) is lacking. To assess functional results and durability, we studied a group of patients with DCAs implanted between 2000 and 2005 and compared them with a similar group with SCAs. METHODS: From July 2000 until January 2005, 47 patients underwent insertion of a DCA between the right ventricle and pulmonary arteries. The DCA patients were compared with 47 age-matched and diagnosis-matched controls receiving SCAs. All

The Use of Allografts in Ankle Injuries.

Ankle sprains can damage the lateral ankle ligament complex, resulting in lateral ankle instability or intra-articular cartilage damage, like osteochondral defects (OCD). Both ankle instability and OCD can lead to ankle osteoarthritis, resulting in substantial disability and negatively impacting patients' quality of life. Treating these lesions could be challenging, and various surgical procedures have been described in the literature, including allografts, which have gained popularity and shown promising results in the last few years. The main advantages of allografts include the absence of d

[Vascularized composite allografts in France: An update].

Vascularized composite allografts (VCA) encompass the face, upper limb, trachea, penis, abdominal wall, and, more recently, uterus transplants. They offer unique reconstructive possibilities to overcome the limitations of traditional reconstructive techniques. Unlike solid organ transplants (heart, liver, kidney, lung, etc.), VCA is not generally performed in a life-threatening situation but aims to improve quality of life, at the cost of a major constraint to its expansion: the need for lifelong immunosuppressive treatment. Nevertheless, VCA is considered one of the five most important innova

Partial decellularization eliminates immunogenicity in tracheal allografts.

There is currently no suitable autologous tissue to bridge large tracheal defects. As a result, no standard of care exists for long-segment tracheal reconstruction. Tissue engineering has the potential to create a scaffold from allografts or xenografts that can support neotissue regeneration identical to the native trachea. Recent advances in tissue engineering have led to the idea of partial decellularization that allows for the creation of tracheal scaffolds that supports tracheal epithelial formation while preserving mechanical properties. However, the ability of partial decellularization t

Mechanical properties of de-epithelialized tracheal allografts.

BACKGROUND: Many studies have shown that mucosa-treated tracheas can be used for allotransplantation without the use of immunosuppressants. However, no literature has reported whether the transplanted trachea has normal biomechanics. In this study, fresh dog trachea was used as a control to determine the mechanical properties of allograft trachea after mucous membrane removal. METHODS: Cervical tracheas (~10 cm in length, consisting of the cartilage rings down to the tracheal carina) and the surrounding connective tissues were removed from 20 mixed-breed dogs and then divided into 4 groups wi
