Effects of almitrine bismesylate on the ionic currents of chemoreceptor cells from the carotid body.

Almitrine is a drug used in the treatment of hypoxemic chronic lung diseases such as bronchitis and emphysema because it is a potent stimulant of the carotid bodies in human and different animal species that produces a long-lasting enhancement of alveolar ventilation, ameliorating arterial blood gases. However, the mechanism of action of almitrine remains unknown. We investigated the effect of almitrine on ionic currents of chemoreceptor cells isolated from the carotid body of rat and rabbits by using the whole-cell and inside-out configurations of the patch-clamp technique. Almitrine at conce

Ventilatory responses of the rat to mild hypercapnic stimulation before and after almitrine bismesylate.

In pentobarbital-anesthetized rats, hyperventilatory responses to rectangular mild hypercapnic normoxic gas mixtures inhalation were analysed. Hyperventilation dynamics were characterized by a logarithmic curve whose delay (T0) and half response time (T50-T0) were calculated. These values were established in nervously intact (IR), vagotomized (XT), surgically chemodenervated (CDN) rats and in IR-rats after almitrine bismesylate (ALM) pretreatment. In IR and XT-rats, T0 and T50 - T0 were larger after 0.5 and 1.0% CO2 step-changes than after 1.5% CO2 step-change (10 sec vs 2 sec for T0). In CDN

Effects of almitrine and oxygen on ventilation and gas exchange in patients with chronic respiratory insufficiency.

Effects of a new respiratory stimulant almitrine on lung ventilation and gas exchange were studied in 15 patients suffering from chronic respiratory insufficiency. The variables examined were measured during 4 consecutive periods lasting 30 min each. During the first period initial data were collected. From the second period to the end of the investigation, oxygen, 1.5 1/min, was administered. During the third phase, almitrine was infused at a dose of 0.33 mg/kg. Administration of oxygen increased arterial oxygen tension without change in lung ventilation. Infusion of almitrine increased minut

Effect of almitrine on renal sodium excretion in chemoreceptor denervated spontaneously hypertensive rats.

The effect of almitrine on urinary sodium excretion was investigated in conscious carotid body-, sino-aortic- and sham-denervated SHR. The animals were kept singly in metabolic cages and had free access to food and tap water. Oral application of almitrine (0.5 mg/kg bwt) during five days induced a twofold natriuresis in sham-operated rats. The first natriuretic response on the fourth day of almitrine treatment was observed in animals of all groups indicating that this effect is not chemoreceptor mediated. The second natriuresis seen on the sixth day of the post-treatment period in both sino-ao

Haemodynamic and respiratory effects of post-operative doxapram and almitrine in patients following pneumonectomy.

Respiratory analeptics have been shown to give important increases in pulmonary vascular resistance and pulmonary arterial pressure in hypoxaemic respiratory failure patients, but no studies have been carried out in the post-operative recovery period. The aim of this study was to compare the haemodynamic and respiratory effects of doxapram and almitrine infusion given over 1 h in patients who had just undergone pneumonectomy. Two hours after the end of anaesthesia, three groups of randomly selected patients were therefore given 3 mg kg-1 doxapram (n = 10), 1 mg kg-1 almitrine (n = 10) or place

The effect of almitrine bismesylate on the steady-state responses of arterial chemoreceptors to CO2 and O2 in the cat.

The interaction between almitrine bismesylate, a pharmacological stimulant of peripheral chemoreceptors, and varying levels of oxygen (PO2 50-600 Torr) and carbon dioxide (PCO2 10-65 Torr) on steady state carotid chemoreceptor discharge was investigated in pentobarbitone-anaesthetised cats. Almitrine was given as constant intravenous (50 micrograms/kg per min for 4 min) and intracarotid infusions (4-16 micrograms/kg per min) at different levels of alveolar PO2 and PCO2. Almitrine always excited discharge. The intracarotid infusions at the lower infusion rate (4-8 micrograms/kg per min) and the

Effects of the respiratory stimulant almitrine on breathing and FOS expression in the brain of fetal and newborn sheep.

Almitrine is a piperazine derivative known to stimulate breathing in the adult but cause apnea in fetal sheep. In fetal sheep (127-133 d gestation; term = 147 d) we confirmed this finding, but found that almitrine (4 mg/kg, i.v. or intra-arterial) had a biphasic effect, briefly stimulating and then suppressing breathing movements for at least 3 h. In 2- to 3-d-old (n = 4) and 7- to 14-d-old (n = 4) lambs almitrine increased both tidal volume and breath frequency, increased arterial partial pressure of oxygen and pH, and decreased partial pressure of carbon dioxide. The changes of tidal volume,

Long-lasting hyperventilation induced by almitrine: evidence for a specific effect on carotid and thoracic chemoreceptors.

In anesthetized dogs, almitrine (0.5-3 mg/kg i.v.) induced a dose-dependent increase in respiratory rate and ventilation. The aortic and carotid chemoreceptors were involved in the effects of almitrine. Section of both carotid sinus nerves and vagus nerves abolished the effects of the drug on respiration. The respiratory response did not occur in dogs with bilateral lesions of the nucleus of the solitary tract. The electrical activity of chemoreceptor fibres was increased. Perfusion of almitrine into the carotid artery stimulated respiration. Inhalation of pure oxygen shifted the dose-response

Changes in ventilation, breathing pattern and acid-base balance of conscious rabbits following intravenous almitrine.

Five conscious rabbits each received a single injection of almitrine bismesylate via a marginal ear vein. Measurements were made of ventilation and breathing pattern using a barometric method, and blood gases, pH and lactate concentration ([Lac]) were measured from arterial blood samples throughout the hour following the injection. Almitrine caused an immediate increase in ventilation through an increase in tidal volume (VT), frequency (f) showing a small decline. After 15 min f increased to above control level and VT declined. Slower, deeper breathing was reinstated by 40 min post-injection.

The effect of almitrine on the steady-state ventilatory response to carbon dioxide at rest and during exercise in man.

Almitrine has potential as a tool for testing the physiological role of the peripheral chemoreceptor. The effects of almitrine on CO2 chemosensitivity were studied at rest and during light exercise using a constant inflow technique that avoids the hyperoxia of rebreathing methods. The steady-state ventilatory response to CO2 was measured in two groups of six normal men before and 150 min after 100 mg oral almitrine bismesylate or placebo. One group was studied at rest, the other while pedalling at 50 W. The resting group showed a significant increase in CO2 response slope after almitrine when

Effects on the rabbit carotid body of stimulation by almitrine, natural high altitude, and experimental normobaric hypoxia.

Rabbits were given intraperitoneal injections of almitrine in ascending doses for 5 weeks. They were compared with a control group and with a group of rabbits which had been exposed from birth to the natural hypobaric hypoxia found at Cerro de Pasco (433 m) in the Peruvian Andes. A further group of animals was placed in an experimental normobaric chamber for either 3 or 6 months to subject them to the same degree of hypoxia as that occurring in Cerro. The carotid bodies of the rabbits in all these groups were processed for light and electron microscopy, and examined both qualitatively and quan

Intravenous almitrine bismesylate reversibly induces lactic acidosis and hepatic dysfunction in patients with acute lung injury.

BACKGROUND: Intravenous almitrine, which augments hypoxic pulmonary vasoconstriction, is used for short-term improvement of arterial oxygenation. However, recent research has suggested a potentially harmful effect on lactate metabolism and hepatic function. METHODS: Arterial oxygenation, hemodynamic parameters, plasma lactate, and hepatic function were monitored prospectively in 25 patients with acute lung injury (defined as a ratio of arterial oxygen pressure to inspiratory oxygen fraction < or = 150 mmHg) who where treated with intravenous almitrine. In 21 of 25 patients, acute lung injury

Almitrine bismesylate modulates the susceptibility of guinea pig pulmonary artery to catecholamines, PGF2 alpha and adenosine.

The action of almitrine bismesylate, a potent ventilatory stimulant drug, was studied on the contractile responses to some putative transmitters of carotid body (dopamine, noradrenaline, PGF2 alpha, adenosine) in segments of guinea pig pulmonary arteries. Almitrine (10 mumol/l) was capable of mitigating the contractions evoked by dopamine (0.3 mmol/l), noradrenaline (1 mumol/l) and PGF2 alpha (10 mumol/l) as well. In vessels precontracted with noradrenaline, almitrine (added to the nutrient solution of vessels before applying noradrenaline) potentiated the adenosine-induced initial contractile
