PURPOSE: Headache at altitudes has had an incidence of 25-62% through many related studies. Many reasons are identified concerning headache at altitudes such as acute mountain sickness (AMS), sinus headache, migraine, tension type headache, and frontal tension headache. This study tried to compare different types of headache among trekkers on Mount Damavand, a 5671m mountain, Iran, to find their incidence and related symptoms and signs.
METHODS: Through a cross-sectional study, we evaluated headache incidence and its correlation to AMS among people who climbed Mount Damavand. Lake Louise Scor

BACKGROUND: Travelling to high altitudes (sleeping above 2500 m) carries a risk of acute mountain sickness (AMS). Proper pre-acclimatization strategies, such as time spent at simulated altitude, can significantly lower this risk. However, the optimal duration of pre-acclimatization remains unknown. Therefore, we examined published research on how different types and durations of pre-acclimatization at simulated altitude influence the AMS risk.
METHODS: A literature search was conducted to find controlled studies investigating the effects of normobaric or hypobaric simulated altitude exposures

Cross stress adaptation (CA) refers to the induction of physiological adaptation to one stimulus, such as cold water, to adaptively influence the physiological response to another novel stimulus, such as hypoxia. This study sought to determine if cold-hypoxia CA, induced by repeat cold-water immersion (CWI) and compared with a thermoneutral water (TN) control, could reduce resting and submaximal exercise physiologic strain, improve exercise performance, and decrease the incidence and severity of acute mountain sickness (AMS) symptoms during a 29 h exposure to hypobaric hypoxia (∼4,100 m). High-altitude cerebral edema (HACE) and acute mountain sickness (AMS) are neuropathologies associated with rapid exposure to hypoxia. However, speculation remains regarding the exact etiology of both HACE and AMS and whether they share a common mechanistic pathology. This review outlines the basic principles of HACE development, highlighting how edema could develop from 1) a progression from cytotoxic swelling to ionic edema or 2) permeation of the blood brain barrier (BBB) with or without ionic edema. Thereafter, discussion turns to the available neuroimaging literature in the context of cyto

High-altitude heart disease, a form of chronic mountain sickness, has been well established in both Tibet and Qinghai provinces of China, although little is known regarding this syndrome in other countries, particularly in the West. This review presents a general overview of high-altitude heart disease in China and briefly summarizes the existing data with regard to the prevalence, clinical features, and pathophysiology of the illness. The definition of high-altitude heart disease is right ventricular enlargement that develops primarily (by high-altitude exposure) to pulmonary hypertension wit

Acute mountain sickness (AMS) usually occurs after 6-12 hours of acute exposure to altitudes above 2,500 m. If there is no further altitude gain, it normally resolves spontaneously within a day or two. However, it may, in rare cases, progress to life-threatening cerebral edema. High-altitude pulmonary edema (HAPE) is a non-cardiogenic edema that is often preceded by symptoms of AMS. The major preventive measure is slow ascent. Acetazolamide and dexamethasone are effective in preventing AMS, while nifedipine is effective only against HAPE. Immediate descent and/or the administration of oxygen i

AIM: Native Tibetans have adapted to chronic hypoxia, yet remain susceptible to Chronic Mountain Sickness (CMS), including high-altitude polycythemia (HAPC) and high-altitude pulmonary hypertension (HAPH). CMS prevalence rises with altitude, but mechanisms remain unclear. We aim to investigate biomarker profiles in native Tibetans with and without CMS across altitudes, including extreme altitude >5,000m.
METHODS: We enrolled 91 native Tibetans across 4 altitudes (500 m/3,680 m/4,750 m/5,373 m), categorized into healthy, HAPC and HAPH groups. Seventeen biomarkers were quantified via Luminex. B

In April-May 1996 a Norwegian expedition climbed Mount Everest from the Tibetan side, Eight out of the ten members reached the summit. The expedition did not experience any serious incidents but took part in the rescue and treatment of climbers from other expeditions. Eleven cases are reported. Four of these were fatal. Cerebral oedema and frostbite were the most common problems in addition to hypothermia and exhaustion. The author also discusses acute mountain sickness, including clinical features, treatment and prophylaxis.

Every year, millions of people travel to altitudes above 2,500 m for work or leisure, risking acute mountain sickness (AMS). The role of fluid retention in AMS development is controversial. This study examines the ability of simple hydration markers, collected within hours of arrival at high-altitude (HA), to predict AMS development. In 32 healthy, AMS susceptible individuals (female: 17; mean age: 38 ± 12 years), body mass (BM) and AMS severity were monitored over 21 h at HA (3,650 m). Fluid intake and toilet visits were recorded. BM changes within 3 and 9 h correlated with AMS severity the n

More than 140 million people worldwide live >2500 m above sea level. Of them, 80 million live in Asia, and 35 million live in the Andean mountains. This latter region has its major population density living above 3500 m. The primary objective of the present study is to review the physiology, pathology, pathogenesis, and clinical features of the heart and pulmonary circulation in healthy highlanders and patients with chronic mountain sickness. A systematic review of worldwide literature was undertaken, beginning with the pioneering work done in the Andes several decades ago. Original articles w

BACKGROUND: Exposure to high altitude may unpredictably lead to acute mountain sickness (AMS). The purpose of this study was to identify the predictors of AMS at low altitude using exercise stress echocardiography (ESE).
METHODS: A total of 40 healthy adults were enrolled and underwent comprehensive supine bicycle ESE at low altitude, including pulmonary vascular resistance (PVR), right ventricular area index at the end of diastole, B-lines, and inferior vena cava (IVC) diameter. All subjects ascended to 3,600 m within 24 hours. The risk factors for AMS were screened using least absolute shri

PURPOSE: Hypoxia has been shown to reduce energy intake and lead to weight loss, but the underlying mechanisms are unclear. The aim was therefore to assess changes in eating after rapid ascent to 4,559 m and to investigate to what extent hypoxia, acute mountain sickness (AMS), food preferences and satiation hormones influence eating behavior.
METHODS: Participants (n = 23) were studied at near sea level (Zurich (ZH), 446 m) and on two days after rapid ascent to Capanna Margherita (MG) at 4,559 m (MG2 and MG4). Changes in appetite, food preferences and energy intake in an ad libitum meal were

Peripheral chemoreflex function was studied in high-altitude (HA) natives at HA, in patients with chronic mountain sickness (CMS) at HA, and in sea-level (SL) natives at SL. Results were as follows. 1) Acute ventilatory responses to hypoxia (AHVR) in the HA and CMS groups were approximately one-third of those of the SL group. 2) In CMS patients, some indexes of AHVR were modestly, but significantly, lower than in healthy HA natives. 3) Prior oxygenation increased AHVR in all subject groups. 4) Neither low-dose dopamine nor somatostatin suppressed any component of ventilation that could not be

PURPOSE: Intermittent exogenous ketosis via ketone monoester (KE) ingestion has been shown to enhance physiological responses and reduce acute mountain sickness (AMS) symptoms during acute normobaric hypoxia. Its effects during early acclimatization to terrestrial high-altitude, however, remain unclear.
METHODS: Thirty-four participants were randomised to an intermittent exogenous ketosis (IEK, n=17) or placebo (PLA, n=17) group and underwent a near sea level trial (295 m) without supplementation, followed by a four-day sojourn at 3375 m during which they received regular KE (IEK) or placebo

OBJECTIVE: Acute mountain sickness (AMS) is defined by patient-reported symptoms using the Lake Louise Score (LLS), which provides limited insight into any possible underlying central nervous system (CNS) dysfunction. Some evidence suggests AMS might coexist with altered neural functioning. Cognitive impairment (CI) may go undetected unless a sens

OBJECTIVES: Elevated optic nerve sheath diameter on sonography is known to correlate with increased intracranial pressure and is observed in acute mountain sickness. This study aimed to determine whether optic nerve sheath diameter changes on ascent to high altitude are associated with acute mountain sickness incidence.
METHODS: Eighty-six healthy adults enrolled at 1240 m (4100 ft), drove to 3545 m (11,700 ft) and then hiked to and slept at 3810 m (12,500 ft). Lake Louise Questionnaire scores and optic nerve sheath diameter measurements were taken before, the evening of, and the morning afte

Traveling to high altitudes for entertainment or work is sometimes associated with acute high altitude pathologies. In the past, scientific literature from the lowlander point of view was primarily based on mountain climbing. Sea level scientists developed all guidelines, but they need modifications for medical care in high altitude cities. Acute Mountain Sickness, High Altitude Pulmonary Edema, and High Altitude Cerebral Edema are medical conditions that some travelers can face. We present how to diagnose and treat acute high altitude pathologies, based on 51 years of high altitude physiology

Individuals who reside at high altitudes for extended periods or those who visit these regions briefly frequently experience high-altitude response, which triggers a series of physiological and pathological changes in the body, ultimately causing altitude sickness. One of the most critical features of high-altitude environments is hypoxia. Recent studies have demonstrated that hypoxia-inducible factor 1 (HIF-1) plays a central role in mediating the body's response to hypoxic conditions at high altitudes. HIF-1, a heterodimeric transcription factor composed of an oxygen-sensitive subunit α (HIF
