UltraViolet SANitizing System for Sterilization of Ambulances Fleets and for Real-Time Monitoring of Their Sterilization Level.

BACKGROUND: The contamination of ambulances with pathogenic agents represents a potential threat for the public health, not only for common pathogens but also for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The aim of this project was to exploits the germicidal effect of the UVC radiation at 254 nm to sanitize the patient's compartment of ambulances with an advanced UltraViolet SANitizing System (UV-SAN) and assess its relevance for avoiding the spread of COVID-19 and other drug resistant pathogens. METHODS: The system is equipped with UVC lamps that are activated when the a

Limiting factors in peak oxygen uptake and the relationship with functional ambulation in ambulating children with spina bifida.

The objective of this study is to interpret the outcomes of peak oxygen uptake (VO(2peak)) in children with SB and explore the relationship between VO(2peak) and functional ambulation using retrospective cross-sectional study. Twenty-three ambulating children with SB participated at Wilhelmina's Children's Hospital Utrecht, the Netherlands. VO(2peak) was measured during a graded treadmill-test. Eschenbacher's and Maninna's algorithm was used to determine limiting factors in reaching low VO(2peak) values. Energy expenditure during locomotion (both O(2) rate and O(2) cost) and percentage of VO(2

A survey-based evaluation of ambulance staff awareness of vitamin D and risk of deficiency in a UK ambulance service.

BACKGROUND: Healthcare professions, shift-working and indoor-working are risk factors for vitamin D deficiency. The aim of this survey was to investigate ambulance staff awareness of vitamin D, and the risks associated with deficiency, to inform the need for appropriate well-being resources. METHODS: A purpose-designed, 20-question survey was developed, based on a validated vitamin D questionnaire, to explore staff vitamin D knowledge and self-health in one UK ambulance service. Disseminated during June/July 2020, survey completion was voluntary, and responses obtained were analysed using des

Ambulation after in-utero fetoscopic or open neural tube defect repair: predictors for ambulation at 30 months.

OBJECTIVES: To compare the ambulatory status of a cohort of children who had undergone prenatal repair of an open neural tube defect (ONTD) using one of two different methods (fetoscopic or open hysterotomy) with that of a cohort who had undergone postnatal repair, and to identify the best predictors of ambulation at 30 months of age. METHODS: This was a retrospective review of a cohort of children who underwent ONTD repair either prenatally (n = 110), by fetoscopic surgery (n = 73) or open hysterotomy surgery (n = 37), or postnatally (n = 51), in a single tertiary hospital between November 2

The Effects of Tropical Climate on the Stability of Emergency Drugs Used in Ambulances Under Real EMS Situations.

OBJECTIVES: Adrenaline (epinephrine), amiodarone, and atropine are included in the 2020 Advanced Cardiac Life Support guideline and are commonly used during cardiopulmonary resuscitation in both prehospital and in-hospital situations. The emergency drug boxes are often exposed to high ambient temperatures and very humid climates in the tropical regions. These accelerated conditions can significantly impact the drug stability. It is crucial to ensure that these medications maintain their potency to provide optimal patient care. METHODS: Three medications, including adrenaline, amiodarone, and

New and emerging drivers of ambulance demand.

BACKGROUND: The number of calls for ambulances in Australia has been rising faster than population growth, but it is unclear what is driving this demand. This study aims to quantify and compare the drivers of emergency ambulance demand in Victoria, Australia, before and after the COVID-19 pandemic, including demographic and clinical factors. METHODS: We conducted a retrospective cohort study of all emergency ambulance calls in Victoria from January 1, 2016, to December 31, 2023. Using negative binomial segmented time series regression models, adjusted for population growth and seasonality, we

Patient Ambulations Predict Hospital Readmission.

Improved functional ability and physical activity are strongly associated with a broad range of positive health outcomes including reduced risk of hospital readmission. This study presents an algorithm for detecting ambulations from time-resolved step counts gathered from remote monitoring of patients receiving hospital care in their homes. It examines the statistical power of these ambulations in predicting hospital readmission. A diverse demographic cohort of 233 patients of age 70.5±16.8 years are evaluated in a retrospective analysis. Eleven statistical features are derived from raw time s

Smartphone monitoring of in-ambulance vibration and noise.

Transferring sick premature infants between hospitals increases the risk of severe brain injury, potentially linked to the excessive exposure to noise, vibration and driving-related accelerations. One method of reducing these levels may be to travel along smoother and quieter roads at an optimal speed, however this requires mass data on the effect of roads on the environment within ambulances. An app for the Android operating system has been developed for the purpose of recording vibration, noise levels, location and speed data during ambulance journeys. Smartphone accelerometers were calibrat

Ambulance Crash in a Rural Area of Thailand.

BACKGROUND: Ambulance crashes delay patient transfer and endanger patients, ambulance crews, and other road users. In low- and middle-income countries, where motor vehicle crash rates are typically high, ambulances have a high risk of being involved in a crash. This case report describes an ambulance crash in Thailand to elucidate modifiable problems in current protocols and practices of emergency medical services. CASE REPORT: In November 2016, a 28-year-old male driver of an ambulance died in a crash while transferring a female patient with dizziness to a rural hospital. The driver and anot

Calcaneus deformity in the ambulant patient with myelomeningocele.

We reviewed our experience of tibialis anterior transfer and anterior release for calcaneus deformity in 46 feet of 26 ambulant patients with myelomeningocele. At an average follow-up of 8.4 years (2 to 17.6) there were 89% who had satisfactory results; 64% of the patients having tibialis anterior transfers were able to stand on their toes. Hip abductor power was a good predictor of a functional transfer. Pre-operative trophic ulceration of the heel increased from 3.2% to 33% if surgery was delayed. Secondary deformities, two-thirds of them into valgus, developed in 76% of feet.

Multicentre trial of piroxicam in ambulant rheumatology.

A total of 8299 patients suffering from rheumatoid arthritis, osteoarthrosis, ankylosing spondylitis or non-articular rheumatism were included in a large non-comparative multicentre study. In all, 98% of the patients reported some functional disability at baseline. At the final visit, only 20% had more pronounced incapacity. Pain, as assessed by the physician, was reduced by 44%. Overall, 70% of the patients having had recent treatment with a NSAID (n = 5489) rated current piroxicam treatment as better. The toleration of the piroxicam treatment was good. A total of 17% of the patients reported

Stroke symptoms and the decision to call for an ambulance.

BACKGROUND AND PURPOSE: Few acute stroke patients are treated with alteplase, partly because of significant prehospital delays after symptom onset. The aim of this study was to determine among ambulance-transported stroke patients factors associated with stroke recognition and factors associated with a call for ambulance assistance within 1 hour from symptom onset. METHODS: For 6 months in 2004, all ambulance-transported stroke or transient ischemic attack patients arriving from a geographically defined region in Melbourne (Australia) to 1 of 3 hospital emergency departments were assessed. Emergency ambulance dispatch: is there a case for triage?

Emergency telephone calls for an ambulance (999 calls) are usually dealt with first-come first-served. We have devised and assessed criteria that ambulance dispatch might use to prioritize responses. Data were collected retrospectively on consecutive patients presenting to an accident and emergency (A&E) department after a 999 call. An unblinded researcher abstracted data including age, date, time, caller, location, reason for call and A&E diagnosis and each case was examined for ten predetermined criteria necessitating an immediate ambulance response--namely, cardiac arrest; chest pain; short

A portable infusion system for the ambulant child.

A portable infusion system has been developed that can maintain a continuous intravenous or nasogastric infusion in a child for prolonged periods of time. Three children have received infusions of several months' duration over a period of two years. They were able to remain ambulant and the system has been maintained by parents at home. A minature peristaltic rotary pump is carried on the back, which infused fluid from an IV solution pack via an indwelling silicone rubber catheter. During IV infusion two bacterial filters are used in series. The first removes the particles normally present in
