Real-Time Ambulatory ECG Does Not Expedite Care.

BACKGROUND: Ambulatory ECG (AECG) monitoring is pivotal to the diagnosis of arrhythmias and can be performed with near "real-time" notification of abnormalities. There are limited data on the relative benefit of real-time monitoring compared with traditional Holter monitoring. METHODS AND RESULTS: This is a retrospective observational analysis of University of Utah Health patients who underwent ambulatory ECG studies from 2010 to 2022. The study cohort was stratified by patients with an ambulatory ECG that provides real-time event notification (non-Holter) versus those who do not (Holter). Th

Needle phobia--changing venepuncture practice in ambulatory care.

Needle phobia is a term used in practice to describe an anticipatory fear of needle insertion. A proportion of children display high levels of fear, pain and behavioural distress when exposed to, or anticipating, needle insertion. A difficult routine venepuncture in our ambulatory care unit led staff to review practice and develop a three-step approach to overcoming 'needle phobia': relaxation, control and graded exposure. These developments have resulted in the unit becoming a local referral centre for children and young people between the ages of 5-19 years with this problem. Time and skill …

Assessing technical performance at diverse ambulatory care sites.

The purpose of the large study reported here was to develop and test methods for assessing the quality of health care that would be broadly applicable to diverse ambulatory care organizations for periodic comparative review. Methodological features included the use of an age-sex stratified random sampling scheme, dependence on medical records as the source of data, a fixed study period year, use of Kessner's tracer methodology (including not only acute and chronic diseases but also screening and immunization rates as indicators), and a fixed tracer matrix at all test sites. This combination of…

Examining racial disparity in psychotic disorders related ambulatory care visits: an observational study using national ambulatory medical care survey 2010-2015.

BACKGROUND: One of the most consistent research findings related to race and mental health diseases is the disproportionately high rate of psychotic disorder diagnoses among people of color, specifically people of African descent. It is important to examine if a similar pattern exists among specific psychotic disorders. We aimed to examine the racial/ethnic differences in ambulatory care visits diagnosed with schizophrenia-spectrum disorders (SSDs). METHODS: We analyzed data from the National Ambulatory Medical Care Survey (NAMCS) 2010-2015. The study sample included physician office-based vi…

The impact of Medicaid managed care on hospitalizations for ambulatory care sensitive conditions.

OBJECTIVE: To determine whether Medicaid managed care is associated with lower hospitalization rates for ambulatory care sensitive conditions than Medicaid fee-for-service. We also explored whether there was a differential effect of Medicaid managed care by patient's race or ethnicity on the hospitalization rates for ambulatory care sensitive conditions. DATA SOURCES/STUDY SETTING: Electronic hospital discharge abstracts for all California temporary assistance to needy families (TANF)-eligible Medicaid beneficiaries less than age 65 who were admitted to acute care hospitals in California betw…

[Primary health care and hospitalizations in ambulatory care sensitive conditions in Catalonia].

BACKGROUND: Ambulatory Care Sensitive Conditions (ACSC) are a set of selected codes of hospital discharge diagnosis intended to measure avoidable hospitalizations. Primary Health Care Services may avoid the hospitalizations due to ACSC by applying any of the following interventions, that are characteristics of this level of care: a) primary prevention; b) secondary prevention, and c) tertiary prevention and rehabilitation. METHODS: Observational, cross-sectional pilot study on 248,174 hospital discharges, from a population of 2,248,704 inhabitants in 161 Basic Health Care Units (BHCU), record…

Ambulatory care in the public sector: perinatal care in Los Angeles County.

In planning its response to the increasing demand for perinatal services and the increasing rate of infant mortality and low-birthweight infants the Los Angeles County Department of Health Services faces a formidable set of tasks. Initiatives already in place are attempting to deal with some of the most pressing problems. However, increased services must be made available to enable the increasing number of women to access the system of care, and further outreach is necessary to encourage more women to begin prenatal care early in pregnancy. The county needs to increase the availability of peri

Dizziness in primary care. Results from the National Ambulatory Medical Care Survey.

This study utilized data from the 1981 and 1985 National Ambulatory Medical Care Surveys to study dizziness, a frequently troublesome patient problem in family practice. These data indicate that family physicians and general practitioners see nearly 45% of all outpatients with dizziness. In primary care practices (family physicians, general practitioners, and general internists) the frequency of dizziness as a presenting complaint rises steadily with age, so that nearly 7% of patients aged 85 years and older present with that symptom. Women complain of dizziness more frequently than men, with

Do Payment Methods Incentivize Screening for Unhealthy Alcohol Use in Ambulatory Care Settings? Evidence from the US National Ambulatory Medical Care Survey.

Despite recommendations, screening for unhealthy alcohol use occurs infrequently in US ambulatory care. One barrier to screening often cited by physicians is misalignment of incentives, but it is unclear which financial arrangements contribute to or alleviate this misalignment. This analysis investigates how determinants of income-both patient care revenue and physician remuneration-encourage or discourage guideline-concordant alcohol screening. Cross-sectional data from the US National Ambulatory Medical Care Survey 2015, 2016, 2018, and 2019 are pooled. Covariate-adjusted logistic regression…

Defining ambulatory care sensitive conditions for adults in Portugal.

BACKGROUND: Ambulatory Care Sensitive Conditions (ACSCs) are health conditions for which adequate management, treatment and interventions delivered in the ambulatory care setting could potentially prevent hospitalization. Which conditions are sensitive to ambulatory care varies according to the scope of health care services and the context in which the indicator is used. The need for a country-specific validated list for Portugal has already been identified, but currently no national list exists. The objective of this study was to develop a list of Ambulatory Care Sensitive Conditions for Port…

Trends in Ambulatory Health Care Usage for Adult Acne.

BACKGROUND: The prevalence of acne is increasing among young children but not well characterized in older patients. OBJECTIVE: To determine whether ambulatory health care for adult acne is increasing and whether the average age of adult acne patients seeking care has been increasing over time. METHODS: We performed a retrospective analysis of data from the 1993-2010 National Ambulatory Medical Care Survey for acne visits in ages 25 and older. RESULTS: Average age of adult acne patients increased by 0.13 years per year over the study period. Visits per 100,000 population declined in the 25 t
