Family health history and health behaviors in Alaska native and American Indian people.

BACKGROUND: Family history of diseases among American Indian and Alaska Native (AIAN) people may influence health. METHODS: We examine the prevalence of family health history among a cohort of AIAN people (n= 10,374) enrolled in the Education and Research Towards Health (EARTH) Study. We evaluate the association between having a positive family history and health behaviors to determine if those reporting a family history were more likely to report lifestyles that put them at risk of developing these health conditions. RESULTS: Among participants, 17.7% reported not knowing their family histo

Trajectories of child protective services contact among Alaska Native/American Indian and non-Native children.

BACKGROUND: Contact with child protective services (CPS) functions as an independent marker of child vulnerability. Alaska children are an important population for understanding patterns of CPS contact given high rates of contact overall and among specific demographic groups. OBJECTIVE: We aimed to identify longitudinal trajectory classes of CPS contact among Alaska Native/American Indian (AN/AI) and non-Native children and examine preconception and prenatal risk factors associated with identified classes. PARTICIPANTS AND SETTING: We used data from the Alaska Longitudinal Child Abuse and Ne

Higher Human Papillomavirus Vaccination Coverage Among American Indian and Alaska Native Adolescents and Rural Adolescents in Alaska.

PURPOSE: Human papillomavirus (HPV) related cancer incidence is higher among American Indian/Alaska Native (AI/AN) women than among White women. Limited access to health care or preventive services in rural communities might increase cancer risk among AI/AN people. To understand disparities in Alaska, this study compares rural/urban HPV vaccination rates of AI/AN with non-AI/AN people. METHODS: Immunization rate data were extracted for each year during 2018-2023. Immunization rates from the Alaska Immunization Information System (VacTrAK) were compared between AI/AN and non-AI/AN people. RES

Occupational and environmental exposures among Alaska Native and American Indian people living in Alaska and the Southwest United States.

Most occupational and environmental research describes associations between specific occupational and environmental hazards and health outcomes, with little information available on population-level exposure, especially among unique subpopulations. The authors describe the prevalence of self-reported lifetime exposure to nine occupational and environmental hazards among 11,326 American Indian and Alaska Native (AI/AN) adults enrolled in the Education and Research Towards Health (EARTH) Study in the Southwest U.S. and Alaska. The top three hazards experienced by AI/AN people in Alaska were petr

Vulvovaginal Candidiasis Among American Indian and Alaska Native People, Indian Health Service, 2016-2022.

Vulvovaginal candidiasis (VVC) is an infection caused by the yeast Candida that affects more than 50% of women in their lifetime. We aimed to describe VVC among American Indian/Alaska Native (AI/AN) girls and women who were receiving care in the Indian Health Service (IHS) system during 2016-2022. We calculated the annual VVC prevalence per 1000 IHS user population and examined underlying medical conditions, previous diagnoses, and antifungal treatment. Among 6 million female patient-years, 70 766 patients had ≥1 VVC diagnosis code (2022 prevalence: 14.2 per 1000 IHS user population). Frequent

Potentially preventable hospitalizations among American Indian and Alaska Native adults, 2016-2021.

OBJECTIVE: Potentially preventable hospitalizations are inpatient admissions for a standard set of selected acute illnesses and chronic conditions that might have been avoided with preventive care or outpatient management. During 2010-2012, Alaska Native adults had higher rates of potentially preventable hospitalizations compared to other adults in Alaska. We evaluated potentially preventable hospitalizations among American Indian/Alaska Native (AI/AN) adults in the United States during 2016-2021. METHODS: We used hospital discharge data from the Indian Health Service National Patient Informa

Review of built and natural environment stressors impacting American-Indian/Alaska-Native children.

Children's exposures to chemical and non-chemical stressors from their everyday environment affects their overall health and well-being. American-Indian/Alaska-Native (AI/AN) children may have a disproportionate burden of stressors from their built and natural environments when compared to children from other races/ethnicities. Our objectives were to identify chemical and non-chemical stressors from AI/AN children's built and natural environments and evaluate their linkages with health and well-being outcomes from the peer reviewed literature. Library databases (e.g. PubMed) were searched to i

Place and sexual partnership transition among young American Indian and Alaska native women.

Multiple challenges expose American Indian and Alaska Native (AIAN) women to high-risk sexual partnerships and increased risk for HIV/STI. Using a unique sample of sexually-active young AIAN women (n = 129), we examined characteristics of last three partners and whether transitional partnerships were associated with different risk profiles, including where partners met, lived, and had sex. Respondents were more likely to have met their previous or current secondary partner (P2) at a friend's or family setting (versus work or social setting) (AOR = 3.92; 95 % CI 1.31, 11.70). Posttraumatic stress disorder and symptoms among American Indians and Alaska Natives: a review of the literature.

PURPOSE: American Indians and Alaska Natives (AI/ANs) experience high rates of trauma and posttraumatic stress disorder (PTSD). We reviewed existing literature to address three interrelated questions: (1) What is the prevalence of PTSD and PTSD symptoms among AI/ANs? (2) What are the inciting events, risk factors, and co-morbidities in AI/ANs, and do they differ from those in the general U.S. population? (3) Are studies available to inform clinicians about the course and treatment of PTSD in this population? METHODS: We searched the PubMed and Web of Science databases and a database on AI/AN

The effect of visual impairment on vision-related quality of life in American Indian/Alaska Natives.

PURPOSE: To determine if National Eye Institute Visual Function Questionnaire (NEI-VFQ-25) scores decrease with worsening visual acuity (VA) in American Indian/Alaska Natives (Al/AN), as well as determine the other associated explanatory factors for vision-related quality of life. METHODS: The study included 414 randomly selected AI/AN tribal members aged 40 years or older from the Pacific Northwest. We excluded candidates who were deceased, seriously ill, had dementia, or otherwise were unable to perform subjective testing such as visual field testing. The participants completed the NEI-VFQ-

Research and educational approaches to reducing health disparities among American Indians and Alaska Natives.

Scimitar syndrome is a rare congenital anomaly characterized by anomalous pulmonary venous drainage to the inferior vena cava, causing a left-to-right shunt. Six patients with scimitar syndrome were diagnosed in our hospital between 2002 and 2008. There were 4 girls and 2 boys; 4 < 5 kg in weight, 2 < 8 kg in weight. Scimitar syndrome was suspected in 5 cases because of dextroversion, and diagnosed by color Doppler echocardiography in all 6 when a scimitar vein was detected entering the inferior vena cava. Computed tomography confirmed the diagnosis in all patients. Two patients had horseshoe

Causes of visual impairment and common eye problems in Northwest American Indians and Alaska Natives.

OBJECTIVES: Little information exists regarding the causes of visual impairment and the most common eye problems in American Indians/Alaska Natives. METHODS: We randomly sampled American Indians/Alaska Natives older than 40 years from 3 tribes within the Northwest region. RESULTS: We found a higher prevalence of visual impairment and normal-tension glaucoma, as well as a lower prevalence of ocular hypertension, in American Indians/Alaska Natives compared with previous results in other racial/ethnic groups. CONCLUSIONS: American Indians/Alaska Natives have a need for vision correction. Futur

Midline congenital anomalies: the estimated occurrence among American Indian and Alaska Native infants.

While congenital anomalies have been identified as the second leading cause of infant mortality among American Indians and Alaska Natives, limited information exists concerning the morbidity of such malformations. This study was undertaken to address this concern. Using data from the national hospital discharge database of the Indian Health Service, for the years 1980-1988, morbidity rates of seven, relatively common and easily identifiable midline malformations among liveborn infants in this minority population were estimated. The seven congenital anomalies and the estimated rates per 10000 b

Multi-level determinants of vaccination of the American Indian and Alaska Native population: a comprehensive overview.

CONTEXT: American Indians and Alaska Natives (AIANs) are historically disadvantaged, losing 20 million (95%) of their population largely through epidemics since 1,520 and continuing lower overall vaccination coverage than other races. Determinants of this lower coverage are underexamined. METHODS: Among peer-reviewed relevant articles since 1968, 39 studied AIANs solely; 47 drew general population samples, including AIANs. We employed rigorous economic definitions and framework of Individual Decision-Making Under Uncertainty. The Social-Ecological model identified determinants and mechanisms

Grief and Historical Trauma Among American Indian and Alaska Native Populations: A Cluster Analysis.

American Indian and Alaska Native (AI/AN) communities face significant social, economic, and health disparities rooted in historical traumas stemming from colonization. These disparities manifest in disproportionate rates of poverty, unemployment, and early mortality among AI/AN populations. High rates of premature deaths contribute to a persistent cycle of grief and loss, perpetuating mental and physical health problems. The current study explores the relationships between grief, historical trauma, and psychological and physical health outcomes among AI/AN individuals who have experienced the
