Interventions for anal canal intraepithelial neoplasia.

BACKGROUND: Anal intraepithelial neoplasia (AIN) is a dysplasia of the anal transitional epithelium that is associated with human papillomavirus (HPV) infection. High-grade lesions have the potential to develop into anal cancer. The incidence and prevalence of AIN and anal cancer has been increasing over the last decades. Certain groups - including people living with HIV, men who have sex with men (MSM), and those with suppressed immune systems - are at high risk of developing AIN. Targeted excisions using ablative treatments such as electrocauterisation, infrared coagulation, or cryotherapy h

Rectal and anal canal neuroendocrine tumours.

Neuroendocrine tumors (NETs) are rare, representing 0.5% of all newly diagnosed malignancies. Rectal and anal canal (AC) NETs account for less than 1% of all rectal and AC cancers. Review our institutional experience on NET of the rectum and AC, with emphasis on demographic, histological and treatment features and oncologic outcomes. The study group was identified from the Portuguese Regional South Oncological Registry. From 2000 to 2014, 22 patients with rectal or AC NETs were treated at our institution. Medical records were retrospectively reviewed. There were 12 males (54.5%) and 10 females

Interventions for anal canal intraepithelial neoplasia.

BACKGROUND: Anal canal intraepithelial neoplasia (AIN) is a pre-malignant condition of the anal canal transitional epithelium that is associated with human papillomavirus (HPV) infection. The incidence and prevalence of AIN and anal cancer are increasing rapidly in HIV-positive men who have sex with men (MSM). Other groups like HIV-negative MSM, immunosuppressed patients and people affected by other HPV diseases like genital warts and cervical intraepithelial neoplasia (CIN) may also develop AIN. The condition is complicated by its multicentric and multifocal nature and high rates of relapse a

Anal Canal Melanoma: Challenging the Odds.

INTRODUCTION: Anal canal melanoma is a rare form of both anal cancer and skin melanoma, representing 1% of all anal cancers. Diagnosis is often made at an advanced disease stage. Because of late diagnosis and aggressive clinical course, anal canal melanoma has a very poor prognosis. CASE PRESENTATION: We present the case of a 61-year-old female with anal canal melanoma and lung, liver, pelvic and central nervous system (CNS) metastasis, BRAF V600E mutated. Combined BRAF/MEK inhibition and CNS radiation therapy were able to achieve excellent local and distant disease control. CONCLUSION: Anal

Anal canal gastrointestinal stromal tumour (GIST).

SummaryGastrointestinal stromal tumours (GISTs) are malignant mesenchymal tumours that originate from the interstitial pacemaker cells of Cajal. They are particularly rare, comprising only 5% of all GISTs, and often present at an advanced stage. Treatment for these tumours is still a topic of debate due to their low incidence and insidious location. A woman in her mid-70s presented with rectal bleeding and anal discomfort. A 4.5×4 cm anal GIST was diagnosed. A local excision was performed and the patient was further treated with tyrosine kinase inhibitors. At a 6-month follow-up, the MRI showe

Endobronchial metastases of anal canal carcinoma.

Endobronchial metastases (EBM) secondary to extrapulmonary primary tumours are rare. The most common solid malignant tumours associated with EBM are breast, renal and colorectal carcinomas. This case report describes the first documented case of EBM from anal canal carcinoma. This neoplasm constitutes less than 1% of colorectal tumours. The clinical presentation of this entity is variable, being asymptomatic in more than 50% of cases. Generally, EBM are diagnosed in advanced stages and the survival after the diagnosis is poor. It is therefore an exceptional clinical manifestation in which bron

Treatment of epidermoid anal canal cancer.

Between 1968 and 1979, 183 patients with invasive epidermoid cancer of the anal canal were treated at Institut Curie. There was 156 women, 27 men with a mean age of 67 +/- 11 years (range 40 to 85 years). The initial height of the tumor was less than 4 cm (65 patients), 4 to 6 cm (98 patients), and more than 6 cm (20 patients). All the patients received radiotherapy, either preoperatively or as curative procedure. Twenty-five patients received preoperative radiotherapy, and there was no residual tumor in 9 biopsies obtained at operation. Four local recurrences were observed. One hundred fifty-

Anal canal pressures are low in women with postpartum anal fissure.

BACKGROUND: Anal sphincter hypertenia is commonly thought to underlie development of anal fissure, yet anal fissure is particularly common after childbirth, a time when anal canal pressure may be reduced. This paradox was investigated by a prospective study of the effect of parturition on the pelvic floor. METHODS: Anal manometry was performed 6 weeks before and after delivery in 209 primigravid women with no pre-existing history of anorectal disease. Postpartum studies only were performed on a further 104 primiparae. Anal fissure was diagnosed by history and direct examination. RESULTS: Som

Computer-generated profiles of the anal canal in patients with anal fissure.

PURPOSE: The pathophysiology of anal fissure remains poorly understood. This study examines manometric findings in patients with anal fissure with use of a computer-assisted system, which helps to standardize manometric performance as well as generating longitudinal and cross-sectional profiles of the anal canal. METHODS: Water-perfused, eight-channel, computer-assisted manometry was performed on 12 patients with chronic anal fissure and compared with 12 age-matched and sex-matched controls. RESULTS: Mean maximum average resting pressure (MARP) was 120.5 mmHg in patients and 82.6 mmHg in con

Anal canal pressure in anal fissure before and after internal sphincterotomy.

PURPOSE: To assess whether the anal canal pressure is increased in patients with a chronic anal fissure and to examine the effects of internal anal sphincterotomy (IASy) on this pressure. MATERIALS AND METHOD: Anorectal manometry was performed in 28 patients in whom an anal fissure had been diagnosed by means of physical examination and anuscopy. After IASy the manometric measurement was repeated on the 15th postoperative day and compared with the value obtained in twelve healthy volunteers. RESULTS: In patients with chronic anal fissure, the mean maximal resting pressure (MRP) was found to

Characteristics of anal canal cancer in Japan.

Anal canal cancer (ACC) has been reported to be an uncommon cancer in Japan, as in the USA, Europe, and Australia. This retrospective multi-institutional study was conducted to clarify the characteristics of ACC in Japan. First, the histological ACC type cases treated between 1991 and 2015 were collected. A detailed analysis of the characteristics of anal canal squamous cell carcinoma (SCC) cases was then conducted. The results of the histological types revealed that of the 1781 ACC cases, 435 cases (24.4%) including seven cases of adenosquamous cell carcinomas were SCC and 1260 cases (70.7%)

SMILE-like lesion in the anal canal.

Stratified mucin-producing intraepithelial lesion (SMILE) is an intraepithelial lesion with overlapping features of the high-grade squamous intraepithelial lesion (HSIL) and adenocarcinoma in situ (AIS). Currently, it is well described in the cervix. We present a case showing similar SMILE-like lesions in the polypectomy specimen from the anal canal along with invasive adenocarcinoma components. This lesion showed an immuno-profile characteristic of a SMILE lesion described in the cervix, such as p63 negativity, high ki67 index, and nuclear positivity for p16. It might be arising from the Huma

Squamous cell carcinoma of the anal canal.

Anal squamous cell carcinoma (SCC) is a rare cancer and accounts for approximately 4% of all cancers of the lower alimentary tract. The dominant etiology is infection with human papilloma virus (HPV), which is the most common sexually transmitted disease in the United States. Integration of HPV DNA into the host genome seems to be the driving mechanism behind carcinogenesis. Vaccines directed against oncogenic HPV serotypes exist, and their utility for preventing anal neoplasia is under investigation. Additional risk factors for developing SCC include HIV infection, anal receptive intercourse,

[Recommendations for the management of cancers of the anal canal].

Anal canal carcinomas remain rare, but their management has improved recently. The PET-CT is now used as a standard at the first diagnosis and after relapses. The introduction of intensity-modulated irradiation techniques makes it possible to better conform the pelviperineal and inguinal volumes, improving the homogeneity of the irradiation while sparing some pelvic structures, thus reducing acute and late effects. Nevertheless, the conversion from 3D to intensity-modulated radiotherapy needs a specific and careful approach, mainly for the management of the perineal region, where relapses and

Adenocarcinoma of the anal canal - a systematic review.

AIM: The prognosis of anal adenocarcinoma is poor and the management remains controversial. We carried out a literature review to identify current practice in the management of anal adenocarcinoma. METHOD: A systematic review of the literature was performed for studies in the English language published between 1950 and 2011. All those that focused on the management of anal adenocarcinoma were considered for inclusion. RESULTS: Sixteen retrospective observational studies were identified; no randomized trials were found. Most of the studies contained small numbers of patients due to the rarity

[Adenocarcinoma of the anal canal. Narrative review].

Adenocarcinoma (ADC) of the anal canal is a rare disease comprising only 5% of all anorectal neoplasias and 1.5% of all gastrointestinal tumours. The World Health Organisation classifies anal ADC into 3 types: the first may arise from the mucosa of the transitional zone in the upper canal, the second from the anal glands (ducts) and the last can develop in the environment of a chronic anorectal fistula. Patients with ADC of the anal canal have high rates of pelvic failure, distant metastasis, and lower overall survival than patients with epidermoid carcinoma. Because of limited case reports ab

Neoplasms of anal canal and perianal skin.

Tumors of the anus and perianal skin are rare. Their presentation can vary and often mimics common benign anal pathology, thereby delaying diagnosis and appropriate and timely treatment. The anatomy of this region is complex because it represents the progressive transition from the digestive system to the skin with many different co-existing types of cells and tissues. Squamous cell carcinoma of the anal canal is the most frequent tumor found in the anal and perianal region. Less-frequent lesions include Bowen's and Paget's disease, basal cell carcinoma, melanoma, and adenocarcinoma. This arti
