BACKGROUND: Epidural analgesia (EA) with local anaesthetic is considered to play a key role after colorectal surgery. However, its effect on postoperative recovery is still a matter of debate.
METHODS: A systematic review of randomized controlled trials comparing postoperative EA and parenteral opioid analgesia after colorectal surgery was performed. The effect on postoperative recovery was evaluated in terms of length of hospital stay, pain intensity, duration of postoperative ileus, incidence of postoperative complications and side-effects.
RESULTS: Sixteen trials published between 1987 an

A series consisting of 32 women undergoing total abdominal hysterectomy received a standard narcotic-free anaesthetic. For the first 24 h postoperatively, eight were given the standard regimen of intramuscular morphine sulphate whilst the other three groups received continuous subcutaneous infusions of either morphine sulphate, ketamine hydrochloride or the two drugs combined. The amount of time they were pain free, the incidence of sleep and nausea, together with cardiovascular and respiratory changes were recorded. All three subcutaneous regimens produced significantly more pain-free reading

BACKGROUND: Intravenous patient-controlled analgesia (IVPCA) and patient-controlled epidural analgesia (PCEA) were studied in terms of analgesic efficacy, respiratory function and side effects after thoracic surgery for 24h. PCEA using fentanyl and bupivacaine as compared to IVPCA using morphine provides better pain relief both at rest and during coughing and is associated with fewer side effects.
AIMS: To compare IVPCA and PCEA in terms of analgesic efficacy, respiratory function and side effects after thoracic surgery.
SETTINGS AND DESIGN: Tertiary care teaching hospital. Prospective, rand

Providing anesthesia and analgesia for mouse subjects is a common and critical practice in the laboratory setting. This practice is necessary for performing invasive procedures, achieving prolonged immobility for sensitive imaging modalities (magnetic resonance imaging, for instance), and providing intra- and post-procedural pain relief. In addition to facilitating the procedures performed by the investigator, the provision of anesthesia and analgesia is crucial for the preservation of animal welfare and for humane treatment of animals used in research. Furthermore, anesthesia and analgesia ar

Pain and related disability remain a major social and therapeutic problem. Comorbidities and therapies increase drug interactions and side effects making pain management more compounded especially in the elderly who are the fastest-growing pain population. Multimodal analgesia consists of using two or more drugs and/or techniques that target different sites of pain, increasing the level of analgesia and decreasing adverse events from treatment. Paracetamol enhances multimodal analgesia in experimental and clinical pain states. Strong preclinical evidence supports that paracetamol has additive

PURPOSE OF REVIEW: To explore the data for and against the use of the various components of multimodal analgesia in cranial neurosurgery.
RECENT FINDINGS: Postcraniotomy pain is a challenging clinical problem in that analgesia must be accomplished without affecting neurologic function (i.e. 'losing the neurologic exam'). The traditional approach with low-dose opioids is often insufficient and can cause well recognized side effects. Newer multimodal analgesic approaches have proven beneficial in a variety of other surgical patient populations. The combined use of multiple nonopioid analgesics

Coxibs can be regarded as an effective way of postoperative pain treatment with proven analgesic and opioid-saving effects. When comparing the opioid-saving effect after the large surgical interventions, COX-2 inhibitors are not inferior to NSAIDs and surpass paracetamol. The combination of coxibs and opiate receptors antagonists, as well as epidural analgesia is effective in the frames of multimodal analgesia. The reasonability of coxibs and paracetamol combination is questionable. In patients at risk of gastrointestinal complications development, but with none cardiovascular risk, COX-2 inhi

BACKGROUND: A forceps delivery may be indicated when a fetus fails to progress to delivery, or when delivery needs to be expedited in the second stage of labour. Effective analgesia is required to ensure that the woman is comfortable throughout the delivery, to allow the obstetrician to safely perform the procedure. It is currently unclear what the most effective and safe agent or method is to provide pain relief during forceps delivery.
OBJECTIVES: To assess the effectiveness and safety of different analgesic agents and methods available for forceps delivery for women and their babies.

Painful procedures in the neonatal intensive care unit are common, undertreated, and lead to adverse consequences. A stepwise approach to treatment should include pain recognition, assessment, and treatment, starting with nonpharmacologic and progressing to pharmacologic methods for increasing pain. The most common nonpharmacologic techniques include nonnutritive sucking with and without sucrose, kangaroo care, swaddling, and massage therapy. Drugs used to treat neonatal pain include the opiates, benzodiazepines, barbiturates, ketamine, propofol, acetaminophen, and local and topical anesthetic

BACKGROUND AND OBJECTIVES: The study of pain in neonatology is important because pain and stress mean suffering and discomfort for newborns and, despite it, very little has been done to minimize them. In this revision we discuss: prevention of pain, non-pharmacological and pharmacological treatment, and sedation in newborns.
CONTENTS: Several non-pharmacological measures can be taken to prevent pain in Neonatal Intensive Care Units, and to humanize and reduce the stress on the environment for patients and their families. Pain treatment in the newborn consists of non-pharmacological (non-nutri

Centroneuraxis analgesia has a place in the management of critically ill patients. With the safety of the intrathecally placed microcatheter now in question, epidural blockade is more suitable in the intensive care unit setting. The different mechanisms of action and the resultant synergy of local anesthetic agents and opioids, as well as their different side effects, are outlined. The contraindications to neuraxial blockade are discussed, and the role of epidural analgesia in the management of flail chest syndrome is addressed in detail.

An ideal analgesic for labour would preferably be non-invasive, as effective as spinals and epidurals without their attendant complications and is safe to mother and child and should not complicate the labour process. Analgesia for labouring women ranges from the use of opioid injections to invasive methods, chiefly epidural injections. Each has its advantages and drawbacks. This article provides a review of analgesic methods and techniques for labouring women. It focuses mainly on the role of epidurals, how it is utilised by anaesthetists and the differing methods of drug delivery through the

The current trends of pain treatment from anesthesiological viewpoint mark considerable progress. The ever increasing incidence of injuries in childhood (traffic accidents and home injuries alike), the different mechanism involved, and the multiple damage to organs and systems make mandatory prompt assessment of the condition, diagnosing and adequate therapeutic approach. Analgesia as an essential element of anesthesia is routinely applied to pediatric trauma cases.

Despite physiological advances and recent progress in pain relief, early analgesia for patients with acute abdominal pain is not a conventional endpoint. In clinical practice, priority is often given to diagnosis and management decisions. There are few controlled trials to settle the issue and opinions are still divided. recent studies suggest than early and effective analgesia in acute abdomen does not interfere with diagnosis, and even facilitates initial examination. Various modes of analgesia can be considered.

Endogenous analgesia has been identified in mammals, but little is known about suppression of tonic pain following trauma or disease. Birds suffer from gouty arthritis which can be induced experimentally by intra-articular injection of sodium urate (SU) crystals. SU injection into the ankle joint of the chicken tested in cages resulted in pain-coping behaviour (one-legged standing, sitting) together with severe lameness. Birds kept and tested in large pens showed significantly less pain-coping behaviour, while birds tested in novel pens showed either complete analgesia or marked hypoalgesia, t

This lecture outlines the current pharmacological concepts of sedation and analgesia in the critically ill patient requiring intensive care therapy. The primary goal is to sedate such patients so that they tolerate mechanical ventilation, as well as the therapeutic and diagnostic procedures that are routinely performed on an intensive care ward. The pharmacological regimen comprises both anxiolytic and analgesic drugs. So far, no drug combination has been identified as the ideal therapy; the standard deviation of successful analgosedation is large due to an unpredictable individual response. B
