BACKGROUND: Lumbar epidural analgesia (LEA) is commonly used for labor analgesia but up to 13% of epidural catheters fail and require replacement. Combined spinal-epidural analgesia is associated with a lower catheter failure rate. Few data exist regarding catheter replacement rates after dural-puncture epidural (DPE). We conducted a retrospective analysis comparing catheter failure rates between epidural and DPE techniques.
METHODS: This retrospective single-center trial reviewed all labor neuraxial analgesia procedures among 18 726 women across five years, and identified 810 DPE and 2667 LE

Both epidural and combined spinal epidural (CSE) analgesia can provide maternal pain relief during labor. Currently, there are few data comparing the risks and complications of these two techniques. We recorded the incidence and severity of anesthetic-related complications in 1022 laboring parturients. Ninety-eight women opted for either no or parenteral analgesia, 388 chose epidural, and 536 requested CSE analgesia. Women choosing CSE analgesia most often received an intrathecal injection of sufentanil 10 micrograms at the time of epidural catheter insertion. The epidural catheters were then

Repeated annual audit cycles revealed an unacceptable failure rate of 38%-46% in epidural analgesia for surgical patients in our organisation. Reasons for failure included unilateral block, missed segments and catheter migration. In spite of interventions to remedy the situation, the success rate could not be improved. The aim of the initiative outlined in this article was to improve the efficacy of epidural analgesia and to reduce the failure rate. We found that following the appropriate training and assessment, nurse-administered diamorphine top-ups are a safe and effective way to improve th

We report a case of Bell's palsy following delivery in a parturient who had successful epidural analgesia. Most reported cases have occurred after spinal anaesthesia or inadvertent dural puncture complicating epidural analgesia. Review of the literature revealed few cases of Bell's palsy associated with uncomplicated epidural analgesia. In this report we discuss the controversy surrounding the aetiology and possible mechanism of onset of this unpleasant complication.

Epidurally administered clonidine has been reported to produce postoperative analgesia. To assess the efficacy, safety, and appropriate dose of epidural clonidine for post-cesarean section analgesia, we designed a double-blind, placebo-controlled study. Sixty women were randomly assigned to receive epidural administration of saline bolus followed by 24-h saline infusion, 400-micrograms clonidine bolus followed by 10 micrograms/h clonidine infusion, or 800-micrograms clonidine bolus followed by 20 micrograms/h clonidine infusion. Supplemental analgesia was provided with patient-controlled iv mo

BACKGROUND: Studies have shown increased incidence of severe vaginal lacerations (third and fourth degree) in women under the influence of epidural analgesia. This increase has been attributed to the increased the use of operative vaginal delivery (OVD), with attendant increased risk of laceration. Although mild and moderate vaginal lacerations requiring suturing are clinically significant, their relationship to epidural analgesia has not been extensively studied.
OBJECTIVE: The purpose of this study is to examine relationships between epidural analgesia in laboring women and vaginal lacerati

We present a case of transient segmental neurological reaction secondary to administration of local anaesthetics for epidural analgesia in a 42 year-old woman. An epidural catheter was introduced between thoracic intervertebral interspace 9 and 10. The patient received morphine, lidocaine, bupivacaine and ropivacaine. After 14 hours she experienced symptoms with burning intense pain, hyperalgesia with the upper limit just below the chest and the lower limit in the midfemur. The symptoms disappeared after 36 hours.

Achieving optimal pain relief after lung transplantation (LT) is often difficult, because both systemic analgesics and regional techniques have specific advantages and disadvantages. Uncontrolled pain impedes repeated powerful coughs and hinders valid respiratory excursions and graft expansion; these consequences markedly increase the risk of pulmonary complications. Thoracic epidural analgesia (TEA) is a universally accepted method of pain management after unilateral or bilateral thoracotomy, and in clinical experience it has proven to be the best option after LT. The combination of epidural

Epidural analgesia was used 22 times in three female addax (Addax nasomaculatus). The addax were physically restrained in a drop-floor chute and sedated with acepromazine (0.14-0.34 mg/kg). Epidural analgesia was induced with 2% lidocaine (0.17-0.38 mg/kg) via a 20-ga 1-inch needle placed at the sacrococcygeal intervertebral space or the first intercoccygeal intervertebral space. Adequate analgesia was attained to allow manipulation of the reproductive tract and transvaginal ultrasound-guided oocyte collection.

The effectiveness and safety of 5 mg of epidurally administered morphine for postoperative analgesia was determined in 276 healthy women undergoing cesarean delivery. Overall pain relief, time to administration of additional analgesic medications, and adverse side effects were evaluated. Epidural injection of 5 mg of morphine provided good to excellent pain relief lasting 24 to 36 hours for 83% of patients. Also, review of hospital records for a subset of 34 patients revealed that requirements for additional systemic analgesics were markedly less when postoperative pain relief was provided by

The maternal complications associated with 1,000 consecutive obstetric epidurals are described. Although the analgesia provided was very satisfactory, complications were common. A dural puncture rate of 1-7 per cent resulted in 13 patients suffering from "spinal headache". Blood vessel puncture occurred in 45 patients, while the presence of unblocked segments and unilateral block was troublesome. There were no serious sequelae, but it is suggested that the provision of an epidural service for women in labour requires the immediate availability of an experienced anaesthetist to attend the labou

To justify its place as the preferred method of pain relief in labour, CSE must demonstrated a clear superiority over epidural analgesia. Looking at the relative efficacy of the two techniques failure rates appear to be equal. Speed of onset may be faster with an initial spinal injection although perhaps only clinically relevant in advanced labour where the quality of analgesia may sometimes be better. CSE would not seem to offer significant advantage in terms of mode of delivery or the ability to ambulate. The side effects of the technique are somewhat more concerning as CSE would appear to c

BACKGROUND: Epidural analgesia and dural puncture epidural (DPE) analgesia are widely used techniques for alleviating labour pain. This meta-analysis compared clinical outcomes between parturients receiving epidural analgesia vs DPE analgesia for labour pain.
METHODS: Medical databases were searched to identify randomised controlled trials comparing epidural analgesia with DPE analgesia in labouring parturients published up to October 2024. Results were pooled using an inverse variance random-effects model, and 95% prediction intervals were calculated. Clinical outcomes were used as defined b

BACKGROUND: Neuraxial analgesia is the most effective method for labor pain relief. Continuous epidural analgesia (CEA) provides reliable analgesia but is associated with delayed onset, while traditional combined spinal-epidural analgesia (CSE) offers rapid pain relief at the expense of increased maternal side effects. Modified CSE techniques, typically involving reduced intrathecal drug doses and/or optimized epidural maintenance strategies have been introduced to balance rapid onset with improved safety, but comparative real-world evidence remains limited.
METHODS: In this multicenter retro

BACKGROUND: Epidural analgesia is a central nerve block technique achieved by injection of a local anaesthetic close to the nerves that transmit pain, and is widely used as a form of pain relief in labour. However, there are concerns about unintended adverse effects on the mother and infant. This is an update of an existing Cochrane Review (Epidural versus non-epidural or no analgesia in labour), last published in 2011.
OBJECTIVES: To assess the effectiveness and safety of all types of epidural analgesia, including combined-spinal-epidural (CSE) on the mother and the baby, when compared with

BACKGROUND: Epidural abscess is a serious complication of epidural block. Because of its low incidence, the risk factors and the symptoms and cause of epidural abscess related to epidural anesthesia and analgesia are not well known by anesthesiologists.
METHODS: A computer-assisted search of the literature on epidural catheter-related abscess was performed to describe the clinical course and bacteriology of this complication, to determine possible risk factors, and to assess the index of suspicion among physicians.
RESULTS: Forty-two patients with a catheter-related epidural abscess were ide
