Necrotizing Fasciitis Resulting from an Anastomotic Leak after Colorectal Resection.

One of the most feared complications in colorectal surgery is an anastomotic leak (AL) following a colorectal resection. While various recommendations have been proposed to prevent this potentially fatal complication, anastomotic leaks still occur. We present a case of an AL resulting in a complicated and fatal outcome. This case demonstrates the importance of high clinical suspicion, early recognition, and immediate management.

Esophagovisceral anastomotic leak. A prospective statistical study of predisposing factors.

An esophagovisceral anastomotic leak is a life-threatening postoperative complication, especially in the mediastinum. Of the 242 patients who underwent intrathoracic esophagogastric anastomosis for esophageal carcinoma (182 patients) and adenocarcinoma of the cardia (60 patients) between January 1980 and June 1985, 14 (5.8%) had esophageal anastomotic leakage and two died (0.8%). Various clinical and biologic parameters and aspects of operative technique were studied prospectively and analyzed statistically to identify possible factors responsible for leaks. Both bivariate and multivariate sta

Case Report: Intrahepatic bile duct injury during adhesiolysis mimicking anastomotic leak.

Intrahepatic bile duct injury (IHBDI) is a rare but serious postoperative complication, typically associated with hepatobiliary procedures or direct hepatic trauma. We present the case of a 79-year-old male with prior distal gastrectomy and Billroth II reconstruction who underwent emergency gastrointestinal surgery following blunt abdominal trauma. Postoperatively, gross bilious output from surgical drains raised concern for anastomotic leakage; however, re-laparotomy revealed bile extravasation from the segment 3 liver surface, consistent with peripheral IHBDI most plausibly arising from a co

Esophageal microbiome correlates with post-esophagectomy anastomotic leak in cancer patients.

BACKGROUND: Despite continuous improvement in long-term survival after esophagectomy, potential serious post-operative complications, such as anastomotic leaks (ALs), still occur. Several risk factors for ALs have been proposed, including environmental factors. Our main objective was to examine the correlation of esophageal tumor microbiome composition and functional profile with ALs. Additionally, we analyzed the microbiome of esophageal tumors and their potential correlation with clinical features of the patients. MATERIALS AND METHODS: Surgical specimens of esophageal tumors and adjacent n

Score prediction of anastomotic leak in colorectal surgery: a systematic review.

OBJECTIVE: Predicting the risk of anastomotic leak (AL) is of importance when defining the optimal surgical strategy in colorectal surgery. Our objective was to perform a systematic review of existing scores in the field. METHODS: We followed the PRISMA checklist (S1 Checklist). Medline, Cochrane Central and Embase were searched for observational studies reporting on scores predicting AL after the creation of a colorectal anastomosis. Studies reporting only validation of existing scores and/or scores based on post-operative variables were excluded. PRISMA 2020 recommendations were followed. Q

Diagnostic Modalities for Early Detection of Anastomotic Leak After Colorectal Surgery.

INTRODUCTION: Anastomotic leak (AL) remains a severe complication following colorectal surgery, leading to increased morbidity and mortality, particularly in cases of delayed diagnosis. Existing diagnostic methods, including computed tomography (CT) scans, contrast enemas, endoscopic examinations, and reoperations can confirm AL but lack strong predictive value. Early detection is crucial for improving patient outcomes, yet a definitive and reliable predictive test, or "gold standard," is still lacking. METHODS: A comprehensive PubMed review was focused on CT imaging, serum levels of C-reacti

Infected Haematoma Fistulating Through Adjacent Colorectal Anastomosis Mimicking an Anastomotic Leak.

Anastomotic leakage is a well-understood major complication of colorectal surgery and carries significant implications for patient morbidity and mortality. However, an infected collection fistulating through an otherwise healthy colorectal anastomosis can mimic an anastomotic leak and warrants different management to a primary anastomotic leak. Such a presentation is undocumented in the current literature. A 42-year-old man underwent an elective perineal rectosigmoidectomy (Altemeier's procedure) for full-thickness rectal prolapse. Early postoperative computed tomography (CT) imaging demonstra

Esophageal anastomotic leak does not affect ability to receive adjuvant treatment.

BACKGROUND: Esophageal anastomotic leak is associated with high morbidity and mortality and potentially lethal if not recognized promptly and treated aggressively. While many studies have attempted to delineate the surgical techniques associated with lower rates of anastomotic leak, few have attempted to determine its long term effects on adjuvant therapy and patient quality of life. METHODS: We reviewed our prospective 350 patient esophageal-gastric database and found 194 esophageal anastomoses performed from 1994 to 2013. Leaks were classified based on timing, severity, and location. We the

CT scan-based modelling of anastomotic leak risk after colorectal surgery.

AIM: Prolonged ileus, low-grade fever and abdominal discomfort are common during the first week after colonic resection. Undiagnosed anastomotic leak carries a poor outcome and computed tomography (CT) scan is the best imaging tool for assessing postoperative abdominal complications. We used a CT scan-based model to quantify the risk of anastomotic leak after colorectal surgery. METHOD: A case-control analysis of 74 patients who underwent clinico-radiological evaluation after colorectal surgery for suspicion of anastomotic leak was undertaken and a multivariable analysis of risk factors for l

Trans-oral irrigation for the management of cervical esophagogastric anastomotic leak.

Over a period of 8 years, 15 of the 65 patients who had transhiatal esophagectomy (THE) for esophageal diseases and cervical esophagogastrostomy had cervical oesophagogastric anastomotic leak. Seven of the 18 patients (38.9%) with corrosive esophageal strictures (CES) due to acid burns had anastomotic leaks while 2 out of 6 patients (33.3%) and 6 out of 41 patients (14.6%) were the incidences of anastomotic leaks among alkali burnt and carcinoma of the esophagus. Anastomotic leaks were more commonly associated with surgery for CES. They were managed by trans-oral irrigation with water after in

Pancreaticojejunal anastomotic leak after pancreaticoduodenectomy--multivariate analysis of perioperative risk factors.

The records of 131 consecutive patients with periampullary carcinoma who underwent pancreaticoduodenectomy within a 12-year period were reviewed to determine the perioperative risk factors of pancreaticojejunal (PJ) anastomotic leak. Twenty-one PJ leaks were identified, for a frequency of 16% (21 of 131); 19% (4 of 21) of these patients eventually died of PJ leak-related complications. A total of 23 items of perioperative data, presumed as risk factors predisposing to PJ leak, were examined. By univariate analysis, advanced age, prolonged duration of untreated jaundice, deep jaundice, decrease

Techniques for Diagnosing Anastomotic Leaks Intraoperatively in Colorectal Surgeries: A Review.

Colorectal cancer is one of the most common surgically curable malignancies worldwide, having a good prognosis even with liver metastasis. This improved patient outcome is marred by anastomotic leaks (AL) in operated patients of colorectal cancer despite a microscopically margin-negative resection (R0). Various risk factors have been attributed to causing this. Preoperative non-modifiable factors are age, male sex, cancer cachexia, and neoadjuvant chemo-radiotherapy, and modifiable factors are comorbidities, peripheral vascular disease, anemia, and malnutrition. Intraoperative risk factors inc

Endoscopic vacuum-assisted closure of upper intestinal anastomotic leaks.

BACKGROUND: Management of intrathoracic anastomotic leaks remains an interdisciplinary challenge. Established treatment options include percutaneous drainage, endoscopic closure, or even surgical revision. All these procedures are associated with high morbidity and mortality rates. OBJECTIVE: We report a new, effective endoscopic treatment option for intrathoracic esophageal anastomotic leaks by using an endoscopic vacuum-assisted closure system. PATIENTS: Two patients with intrathoracic anastomotic leaks after esophagectomy and gastrectomy were included. METHODS: Surgical reinterventions f

Esophagoenteric anastomotic leaks: treatment with fluoroscopically guided balloon dilatation.

OBJECTIVE: Disruption of anastomosis soon after esophagectomy and esophagoenterostomy is a relatively common complication that leads to chronic enterocutaneous fistulous drainage through the surgical wound in the lower part of the neck or upper part of the chest. It is believed that narrowing of the anastomosis by postsurgical edema and granulation tissue forces the flow of swallowed saliva through the disrupted anastomosis and contributes to the maintenance of the leakage. We evaluated the role of fluoroscopically guided balloon dilatation for treatment of these esophagoenteric anastomotic le
