Emphysematous aortic arch aneurysm infected with Salmonella: A case report.

Infected aortic aneurysm is a relatively rare disease that is extremely difficult to manage, resulting in a poor prognosis. We rescued a patient with Salmonella-infected aortic arch aneurysm surrounded with a specific and massive emphysema, despite experiencing aortic rupture, including delayed esophageal perforation after surgery.

An Infected Popliteal Aneurysm Simultaneously Treated with Resection and Revascularization.

Infected popliteal aneurysm is a high-risk condition that may present as an emergency requiring an urgent attention with acute rupture and sepsis. The management of acute ischemia in the presence of local and systemic sepsis is challenging, and infection control and perioperative management during surgery are important. Here we report successful case of treating a patient with an infected popliteal aneurysm. The infection seemed to arise from the soft tissue surrounding the aneurysm, following cellulitis. Our report also includes a review of the related literature and suggests that devising me

Q fever (Coxiella burnetii) causing an infected thoracoabdominal aortic aneurysm.

We report a patient, which we believe is the first, with a thoracoabdominal aortic aneurysm, Crawford type IV, caused by Q fever (Coxiella burnetii). Treatment consisted of antibiotic therapy started preoperatively and continued postoperatively and an open repair, including resection of the infected aneurysm, replacement with a rifampin-soaked polyester graft, and an omental wrap covering the grafts. After 13 months of follow-up, the patient had no signs of infection, and results of laboratory findings were normal.

[Exsudative pleurisy revealing an infected aneurysm of the abdominal aorta].

INTRODUCTION: The development of a sero-sanguinous pleural effusion is a rare, often overlooked, manifestation of a ruptured infected aneurysm of the abdominal aorta. CASE REPORT: A man of 84years was referred for management of a left-sided pleural effusion associated with symptoms of infection. He had presented two months previously with methicillin resistant staphylococcal septicaemia, the origin of which was a plantar ulcer. The patient was apyrexial and had no abdominal pain. A thoraco-abdominal CT scan without contrast showed a peri-aortic abdominal mass suggesting a tumour. A contrast e

Infected aneurysm of the abdominal aorta due to Listeria monocytogenes.

A 79-yr-old man was known for a year with a deteriorating clinical condition, vague abdominal complaints and an elevated erythrocyte sedimentation rate; he was afebrile. Extensive evaluation revealed no cause for his progressive disease. Eventually an infected aneurysm of the abdominal aorta was diagnosed, from which Listeria monocytogenes was cultured. After resection of the aneurysm the patient recovered initially very well. Regrettably, therapy-resistant chylous ascites developed, and the patient died due to surgical complications following a second laparotomy. Infected aortic aneurysms can

Replacement of the Thoracoabdominal Aorta after Endovascular Abdominal Aneurysm Repair for Ruptured Infected Aneurysm: A Case Report.

A 73-year-old man underwent emergency endovascular abdominal aneurysm repair (EVAR) for a ruptured infected abdominal aortic aneurysm. Two years after EVAR, he was admitted with a spiking fever and left lower back pain. Computed tomography scan revealed not only recurrent graft infection with psoas abscess but also infection around the orifice of the superior mesenteric artery. Because conservative medical therapy with antibiotics could not control the infection, we performed complete removal of the infected stent graft, debridement of psoas abscess, and in situ replacement of the thoracoabdom

Open treatment of infected abdominal aortic aneurysm stemming from perineal infection.

Infected abdominal aortic aneurysms (AAAs) make up a small minority of AAAs yet are characterized by a high fatality rate, largely attributed to their increased risk of aneurysm rupture. This case details a rare presentation of a 56-year-old man that developed Proteus mirabilis bacteremia secondary to a perineal abscess and subsequently experienced a 3 cm growth of his previously stable AAA over an 8 day period. This case underscores the importance of maintaining a heightened suspicion for infected aortic aneurysms in sick patients and highlights the critical role of surgical management in ach

Endovascular treatment of Brucella-infected abdominal aortic aneurysm: A case report.

RATIONALE: In very rare cases, a primary infected abdominal aortic aneurysm (IAAA) is caused by a species of Brucella. In this report, we report such a case that was successfully treated with a novel approach. To the best of our knowledge, this was the first case occurring in China, in which an infection of the abdominal aortic aneurysm was caused by a Brucella species. PATIENT CONCERNS: The clinical findings included high fever, fatigue, and abdominal pain. DIAGNOSES: The diagnosis was confirmed by computed tomography angiography and by bacteriologic isolation from the patient's blood cultu

[Infected abdominal aneurysm with retroperitoneal fibrosis--a case successfully treated conservatively].

53 year old men suffered from abdominal pain, vomiting, lack of appetite, loss of weight, with ESR > 100 mm Hg and moderate anaemia was presented. Many diagnostic procedures were without results and finally laparotomy was taken. The microscopic examination of specimen of periaortic tissue revealed retroperitoneal fibrosis accompanying atherosclerotic aortae. Corticotherapy and antilipemic drugs were given. After 3 years a complete remission of periaortic pathologic mass was observed. This case could prove the autoimmunological hypothesis of retroperitoneal fibrosis.

Infected abdominal aortic aneurysm: in situ replacement with cryopreserved arterial homograft.

Infected aortic aneurysms are a rare (1.3% of all abdominal aortic aneurysms) but life-threatening disease. At present controversy continues about the specific diagnosis and the best surgical management. We present one case of infected aortic aneurysm treated with in situ reconstruction with cryopreserved arterial homograft. He was a 50-year-old man with recent history of pneumococcal meningitis who is readmitted because he suffered a stroke and during physical examination a pulsatile abdominal mass was discovered. Blood cultures were done and the result was repeatedly negative. Radiological s

Infected aneurysm of middle meningeal artery causing intracerebral hemorrhage: illustrative case.

BACKGROUND: Middle meningeal artery (MMA) aneurysms are rare and typically associated with trauma. Nontraumatic MMA aneurysms are very rare, and their rupture can lead to intracranial hemorrhage. OBSERVATIONS: The authors present a case of a ruptured MMA aneurysm causing intracranial hemorrhage in the temporal lobe, for which immediate hematoma evacuation and aneurysm resection were performed. Pathological examination revealed severe inflammatory infiltration and destruction of the aneurysm wall, consistent with an infected aneurysm. Additionally, they conducted a systematic search and litera

Infected thoracic aortic aneurysm caused by Clostridium ramosum: A case report.

A 73-year-old man undergoing conservative management of acute aortic dissection (Stanford type B) was diagnosed with infected aortic aneurysm based on clinical findings of aortic dilatation and recurrent laryngeal nerve paralysis. We performed total aortic arch replacement with a synthetic graft and omentopexy; and Clostridium ramosum was isolated from resection specimens. Infected aortic aneurysms are rare but may lead to death from aortic dissection or rupture. C. ramosum infection is rare and has seldom been reported, and thus no treatment guidelines, including antibiotic therapy, have been

Hemoptysis secondary to rupture of infected aortic aneurysm- a case report.

BACKGROUND: Massive hemoptysis is a life-threatening condition and can arise as a complication of various conditions. It rarely occurs as a complication of a ruptured thoracic aortic aneurysm. Even rarer are conditions where pseudoanurysms of aorta result due to infection. CASE PRESENTATION: A 30 year-old female patient presented with left sided chest pain, intermittent fever, cough and massive hemoptysis. A pseudo-aneurysm of proximal descending thoracic aorta at the level of the left Subclavian artery was noted over CT scan. Upon performing a left posterolateral thoracotomy, the aneurysm wa

Clostridium septicum-infected aortic aneurysm or graft is a deadly diagnosis.

BACKGROUND: Clostridium septicum is an anaerobic, motile, spore-forming, toxin-producing gram-positive bacillus that can lead to rapidly progressive gas gangrene due to the release of alpha toxin. Aortic aneurysm secondary to C. septicum infection is a rare condition with 60 cases reported in the literature; however, we have recently treated several patients with the condition in our large tertiary care and aortic center. METHODS: Blood and tissue culture results collected between January 2005 and January 2018 and maintained in the microbiology laboratory database at the Cleveland Clinic were

[Complications of infected aneurysm due to Salmonella enteritidis: a case report].

Salmonelloses are becoming more frequented. To date, very few cases of aneurisms or vascular grafts infections have been described. We report the rare case of an extradigestive manifestation, due to Salmonella enteritidis affecting a patient with Behçet disease. The biological diagnosis is based on microorganism isolation from the site of infection but also from blood cultures. The initial treatment combined antibiotherapy and surgical procedure and replacement of the arteriovenous graft.

Ruptured abdominal aortic aneurysm after resection of an infected cardiac myxoma.

A 12-year-old girl with a high fever underwent echocardiography and was found to have a myxoma that arose from the atrial side of the anterior mitral valve leaflet. The tumor was successfully excised. Histologic examination of the tumor showed myxoma cells and an organized thrombus with bacterial colonization. The patient was discharged from the hospital on antibiotic treatment. After remaining asymptomatic for 3 weeks, she was readmitted with acute abdomen. Ultrasonography and magnetic resonance angiography detected intra-abdominal hemorrhaging and a saccular aneurysm of the abdominal aorta.
